Impact of the learning curve on term delivery rates following laparoscopic salpingostomy for infertility associated with distal tubal occlusive disease.

The objective of this study was to determine the impact of the learning curve of one surgeon on the term delivery rate following laparoscopic salpingostomy for tubal infertility. This was a retrospective audit of ongoing clinical practice, undertaken in two tertiary level infertility programmes. Subjects in this study were women undergoing surgery for total occlusion of the distal Fallopian tube. The main outcome measure was cumulative term delivery rates. On stepwise life-table analysis the length of infertility, primary and secondary infertility, tubal diameter and whether surgery was perfor

[Exploring the Entrustment Decision Experiences of Preceptors Regarding New Nurses' Independent Patient Care].

BACKGROUND: Within the framework of competency-based medical education, determining whether new nurses are capable of making patient care decisions independently is a critical issue in clinical nursing education. However, there is a lack of in-depth exploration of clinical preceptor experiences related to entrustment decision-making in this context. PURPOSE: This study was designed to explore the experiences of nursing preceptors in terms of developing the confidence necessary in new nurses to make independent clinical decisions during the preceptorship process. METHODS: A qualitative resear

Increasing capacity by moving away from one-to-one clinical supervision: using peer-assisted learning and a group model of student placements in community paediatric speech and language therapy to enable student-led service delivery.

BACKGROUND: Health Education England (HEE) and the Royal College of Speech and Language Therapists (RCSLT) have identified the need to increase placement capacity. Speech and language therapy is a shortage profession in the UK, so services need to consider innovative placement models to increase their placement offers without increasing the time burden on speech and language therapists (SLTs). AIMS: To increase capacity for pre-registration practice-based learning by using peer-assisted learning (PAL) in a group model of student placement to enable student-led service delivery which provides

Connecting the DOTs in Clinical Orientation: The Daily Orientation Tracker.

Newly hired nurses often complete orientation to the acute care setting with multiple preceptors, and communication about orientation progress has the potential to be disjointed. Traditionally, orientation has been tracked using various formats relying on pen and paper. The Daily Orientation Tracker or DOT, created by preceptors and nurse educators, has pushed communication into an accessible, user-friendly format found to be successful for the orientation of new nurses. Information about how the DOT was developed and implemented as well as data describing the empirical success of the DOT are

Establishing the "North Star" for Clinical Education to Accelerate Practice Readiness: Implications for Leaders, Faculty, Educators, Nursing Professional Development Practitioners, and Preceptors.

Ambiguity and confusion persist around the goal and purpose of clinical experiences in nursing education. As a result of this ambiguity and confusion, the potential exists to waste valuable clinical time, to create ineffective learning objectives, to focus on the tasks of nursing rather than the competencies required for nursing practice, and to perpetuate a mismatch of in-person clinical experiences with simulation decreasing the ability to achieve clinical practice readiness. Nurse leaders across academia, regulation and accreditation agencies, and practice settings must establish consensus

Clinical education of nursing students with learning difficulties: An integrative review (part 1).

Schools of Nursing have witnessed an increase in the number of nursing students who struggle with learning difficulties. Support and accommodations are available in academic settings. Because nursing is a practice profession students also learn in clinical settings, which may not have similar support and accommodations. The compatibility of the clinical setting for the education of students with learning difficulties has not been studied. Staff nurses responsible for the clinical education of students and new nurses receive little preparation for their role as educator, and may not feel suppor

Debunking the myth: Does precepting really lower nurse practitioner productivity?

BACKGROUND: The 2021 AACN Essentials emphasize the importance of preparing practice-ready nurses at all levels, making access to high-quality clinical experiences a critical component of nurse practitioner (NP) education. However, the national shortage of NP student preceptors is a significant challenge for NP educational programs that can jeopardize the clinical learning environment. One commonly cited barrier to precepting NP students is the belief that precepting reduces productivity. PURPOSE: The purpose of this pilot study was to determine if precepting NP students was associated with di

Effects of a program to train radiologic technologists to identify abnormalities on mammograms.

PURPOSE: To assess whether radiologic technologists could be successfully trained to identify abnormalities on mammograms. MATERIALS AND METHODS: Eight radiologic technologists from two different institutions took a pretest, underwent an 8-hour training course followed by a period of preceptorship, and then took a posttest. Seven radiologists also took the pretest and posttest, which consisted of evaluating 1,238 two-view screening examinations, including 318 biopsy-proved cancers. RESULTS: Overall, technologists at institution 1 had lower sensitivity (78%) at the pretest, which improved at

The Cost of Homecare Personal Support Worker Turnover.

As demand for homecare services continues to rise, homecare worker retention remains a critical issue for the sector which impacts access to care and operational efficiency. To design evidence-based retention policies within a fiscally constrained context, it is important to understand the sector-specific costs of turnover. Using data from a large homecare provider organization in Ontario, Canada, we have developed the first known homecare Personal Support Workers (PSWs) cost of turnover estimate. We estimate a turnover cost of $22,000 (CAD 2024) per homecare PSW, based on an employer-level ho

Impact of clinical supervision on the mental health nursing workforce: a scoping review protocol.

INTRODUCTION: Mental health nurses work in potentially unpredictable, stressful and complex environments that can lead to burn-out and high staff turnover. Clinical supervision is a formal and professional agreement between two or more people that aims to strengthen individuals' competencies and organisational strengths. Effective clinical supervision has been noted as a method of reducing workplace issues within mental health nursing, but there is not currently a synthesis of evidence in this area. The key objective of this scoping review is to identify, map and analyse the available evidence

Transition Shock and Work Readiness Among Newly Employed Nurses: A Cross-Sectional Study of the Mediating Role of Professional Self-Efficacy.

BACKGROUND: This study examined how professional self-efficacy mediates the relationship between transition shock and work readiness in newly employed nurses. Transition shock challenges novice nurses, affecting their confidence and practice readiness. Research has explored structural and organizational factors contributing to this phenomenon, including workload, time pressure, staffing constraints, role ambiguity, and limited transition support through orientation/preceptorship and supervision. However, understanding of psychological resources that can mitigate these demands is limited. This

Group Clinical Supervision for midwives and burnout: a cluster randomized controlled trial.

BACKGROUND: There are major shortfalls in the midwifery workforce which has been exacerbated by the COVID 19 pandemic. Midwives have high levels of burnout and many, often early career midwives, are planning to leave the profession. There are reports of a poor workplace culture in maternity units, including bullying. Support is essential for the welfare of the workforce to be able to cope with the demands of their jobs. Supportive strategies, such as Clinical Supervision, a recognised approach in healthcare, enable reflection in a facilitated, structured way, and can enhance professional stand

A Structured Curriculum for Interprofessional Training of Emergency Medicine Interns.

Interprofessional education (IPE) has been shown to improve health outcomes and patient satisfaction. IPE is now represented in the Accreditation Council for Graduate Medical Education's emergency medicine (EM) milestones given the team-based nature of EM. The Highland Allied Health Rotation Program (H-AHRP) was developed by residents to enhance and standardize IPE for EM residents in a single hospital setting. H-AHRP was incorporated into the orientation month for interns starting in the summer of 2016. EM interns were paired with emergency department preceptors in registered nursing (RN), re

During my academic career for more than 40 years, I was involved in 18 epidemiological field studies, partially or fully. Among these field studies, four (1. Medical services in remote rural areas in Okinawa, 2. Yusho episode, 3. JICA Onchocerciasis Control Project in Guatemala, and 4. Miyako cohort study in Fukuoka) are introduced in this paper, including the latest situation after the presentation. Through these field works experienced by the author, the following lessons were gained. 1. Strong human reliance between researchers and the targeted population is essential in carrying out epidem
