Factors Influencing COVID-19 Vaccine Uptake among Spanish-Speaking Pregnant People.

The coronavirus disease 2019 (COVID-19) pandemic exposed the vulnerability of pregnant women to excess morbidity and mortality, as well as the disproportionate disease burden in certain racial, ethnic, and sociodemographic groups. Vaccine hesitancy represents a major threat to public health, and crafting messages that reach vulnerable groups and address their intersectionality remains a weakness for pandemic preparedness. We sought to investigate factors that influenced vaccine acceptance and social media ad response in a mixed-methods study of Spanish-speaking women living in the rural Wester

Racial Inequities in Breastfeeding Counseling Among Pregnant People Who Use Cannabis.

We examined how breastfeeding advice in the context of cannabis use differed by race and ethnicity. Data from the 2017-2018 PRAMS (Pregnancy Risk Assessment Monitoring System) survey were used to assess differences in breastfeeding guidance related to cannabis use among 1,213 individuals who self-reported cannabis use 3 months before or during pregnancy. A multivariable logistic regression model was specified to examine the extent to which the odds of receiving prenatal advice against breastfeeding if using cannabis differed by self-reported race and ethnicity. We found that non-Hispanic Black

Experiences of pregnant people affected by natural disasters: a qualitative systematic review.

OBJECTIVE: This qualitative systematic review identifies, appraises, and synthesizes existing qualitative evidence on the experiences of pregnant people during natural disasters to provide a deeper understanding of the complex factors shaping these experiences. INTRODUCTION: Support for pregnant people affected by natural disasters must address not only their physical and mental health needs but also the safety and well-being of the fetus. Such support requires consideration of pregnancy-specific risks and must address complex contextual factors, including family relationships, social support

Obstetricians' experiences caring for pregnant people experiencing incarceration who request permanent contraception.

BACKGROUND: Given the historic and contemporary legacy of non-consensual sterilization and concern for coercion among incarcerated populations, routine provision of permanent contraception is discouraged. However, as with non-incarcerated patients, the 58,000 pregnant people who are incarcerated every year have diverse contraceptive goals, including the desire for permanent contraception. Clinicians caring for incarcerated patients must navigate this ethical tension. The perspectives and experiences of obstetricians who perform surgical permanent contraception procedures and who care for incar

Social vulnerability and triage acuity among pregnant people seeking unscheduled hospital care.

OBJECTIVES: This study examines the association between census tract-linked social vulnerability index (SVI) and maternal-fetal triage index (MFTI), a standardized score used to classify obstetric triage visit acuity. METHODS: This retrospective cohort study included patients at 20 weeks of gestational age or greater presenting to a New York City obstetric triage unit from March 2019 to April 2021, analyzing only the first pregnancy per patient. Exclusions included missing SVI or MFTI data and MFTI-5 (scheduled services). The primary exposure was SVI, and the primary outcome was MFTI score at

Preventing Influenza Virus Infection and Severe Influenza Among Pregnant People and Infants.

The landscape of research on the benefits of influenza vaccines and antivirals to protect pregnant persons and infants has increased in recent years, while influenza vaccination rates and antiviral usage have declined. Pregnant people and infants <6 months of age are at increased risk of hospitalization with influenza, making protection of this population essential. Maternal influenza vaccination at any time during pregnancy is the best way to reduce the risk of influenza and severe influenza in both pregnant people and their infants <6 months of age. Influenza antiviral medications for pregna

Defining Appropriate Comparator Populations for Placental Pathology for Pregnant People With HIV.

Background. Widespread adoption of antiretroviral therapy has reduced perinatal transmission of HIV; however, people living with HIV (PWH) have higher rates of preterm birth and hypertensive disorders of pregnancy. The placenta is the critical fetal support organ in pregnancy, and multiple investigations have sought associations in PWH between HIV and placental pathology. However, results have been inconclusive. We posit that selection of control group populations influences the apparent anomalies in placentas from PWH and examined the differences seen between these placentas and those of four

Direct Dispensation of Prenatal Supplements With Iron and Anemia Among Pregnant People.

IMPORTANCE: Postpartum transfusion is the most common indicator of severe maternal morbidity in the US. Higher rates of anemia are associated with a higher blood transfusion rate. OBJECTIVE: To determine if providing, rather than recommending, supplements with iron at prenatal visits in a medically underserved community is associated with improved hematologic indices and reduced blood transfusion. DESIGN, SETTING, AND PARTICIPANTS: In this quality improvement study, patients who delivered between May 13 and December 13, 2020, and thus were provided a prenatal supplement with iron throughout

Predictors of Preexposure Prophylaxis Eligibility among Pregnant People with Opioid Use Disorder.

OBJECTIVE: We seek to evaluate risk factors for eligibility for preexposure prophylaxis (PrEP) among pregnant people with opioid use disorder (OUD). STUDY DESIGN: This is a single-site retrospective cohort study of pregnant people admitted for management of OUD at an urban, tertiary care center from 2013 to 2022. PrEP eligibility was defined based on (1) modified American College of Obstetricians and Gynecologists' (ACOG) 2014 criteria: diagnosis of a sexually transmitted infection (STI), engagement in transactional sex work, intravenous drug use (IVDU), or incarceration and (2) modified 2021

Managing Bias in the Care of Pregnant and Parenting People with Substance Use Disorder.

INTRODUCTION: Unintentional overdose is the leading cause of pregnancy-associated death in Maryland and is preventable. Stigma contributes to birthing peoples' disengagement with the health care system, and health care professionals may participate in stigmatizing processes. We aimed to develop and evaluate a training on stigma and bias related to substance use disorder (SUD) for maternal health care professionals in Maryland. METHODS: We used a community-engaged process to develop a training on stigma and bias related to SUD in pregnancy and implemented it with Maryland maternal health care

Obstetrics and Gynecology Resident Comfort in Caring for Pregnant People with Physical Disabilities.

Pregnant people with disabilities face higher complication rates, yet few guidelines exist on caring for this population. This study evaluates obstetrics and gynecology (OBGYN) residents' comfort in caring for pregnant people with physical disabilities.A 19-question e-survey was developed and piloted for content and face validation. Likert scale was used to assess comfort in caring for pregnant patients with physical disabilities. The e-survey was sent to U.S. OBGYN residents via CREOG-coordinator listserv, a listserv to all U.S. OBGYN residency coordinators, in February 2024, with three remin

State Laws on Substance Use Treatment for Incarcerated Pregnant and Postpartum People.

BACKGROUND: Incarcerated perinatal populations report high rates of substance use in the United States (US). Despite this, substance use disorder (SUD) treatment is not routinely available in carceral settings and state policies related to the provision of substance use screening and treatment are unknown. METHODS: We conducted a systematic search in WestLaw through the end of the 2020 legislative session combining the terms "pregnant" and "postpartum" with terms for incarceration and related terms. The search returned 453 statutes from 43 states. A deductive codebook of various maternal heal

Prenatal Stress and Socioeconomic Status as Predictors of Placental Methylation in Pregnant People.

Despite growing interest in placental epigenetics, the combined impact of prenatal stress and socioeconomic status (SES) on placental methylation is still largely understudied. We conducted a study to examine the associations of prenatal stress and socioeconomic factors (household income, Hollingshead socioeconomic index) with placental methylation. Pregnant people (n = 143; M age = 25.66) in their second or third trimester (M gestational age = 23.11 weeks) completed self-report questionnaires including demographic and socioeconomic information and ratings of perceived stress in the past month

Social media provides support and education for pregnant people when healthcare does not.

INTRODUCTION: The use of social media for health-related reasons is growing, but there is a dearth of research on the mechanisms of support provided. Understanding how social media groups work could improve communications between providers and patients. Preeclampsia (PreE) is a hypertensive disease of pregnancy that has short- and long-term physical and psychosocial effects. The Preeclampsia, Eclampsia & HELLP Syndrome Survivors Global Support Network (PEHSS) Facebook group is an online, international, moderated support group that provides evidence-based information and community support.
