IONIS-PKKRx a Novel Antisense Inhibitor of Prekallikrein and Bradykinin Production.

Kallikrein is the key contact system mediator responsible for the conversion of high-molecular-weight kininogen into the inflammatory vasodilator peptide bradykinin, a process regulated by C1-esterase inhibitor (C1-INH). In hereditary angioedema (HAE), genetic mutations result in deficient or dysfunctional C1-INH and dysregulation of the contact system leading to recurrent, sometimes fatal, angioedema attacks. IONIS-PKKRx is a second-generation 2'-O-(2-methoxyethyl)-modified chimeric antisense oligonucleotide, designed to bind and selectively reduce prekallikrein (PKK) mRNA in the liver. IONIS

Factor XII and prekallikrein promote microvascular inflammation and psoriasis in mice.

BACKGROUND AND PURPOSE: Psoriasis is an autoimmune inflammatory skin disease, featuring microvascular abnormalities and elevated levels of bradykinin. Contact activation of Factor XII can initiate the plasma kallikrein-kinin cascade, producing inflammation and angioedema. The role of Factor XII in psoriasis is unknown. EXPERIMENTAL APPROACH: The effects of deficiency of Factor XII or its enzymatic substrate, prekallikrein, were examined in the imiquimod-induced mouse model of psoriasis. Skin microcirculation was assessed using intravital confocal microscopy and laser Doppler flowmeter. A nove

A case of prekallikrein deficiency resulting in severe recurrent mucosal hemorrhage.

Although most cases of prekallikrein deficiency are asymptomatic, a few reports link severe prekallikrein deficiency with thrombotic phenomena and recurrent pregnancy loss. Here, we present a case of a Jamaican female patient with prekallikrein deficiency, associated with a severe bleeding diathesis. A low level of plasma prekallikrein activity, along with the presence of C529Y mutation, confirmed the diagnosis in our patient. Some animals with prekallikrein deficiency were shown to have moderate bleeding tendencies. Furthermore, the first patient with prekallikrein deficiency from the African

Cardiovascular diseases in congenital prekallikrein deficiency: comparison with other chance-associated morbidities.

: To compare the prevalence of cardiovascular diseases with other chance-associated morbidities in patients with congenital prekallikrein deficiency. Patients with prekallikrein deficiency were gathered from two time unlimited PubMed searches and from personal files. Inclusion criteria were prekallikrein level less than 15% of normal; correction of aPTT on long incubation times; prolonged aPTT corrected by normal plasma or serum; normal prothrombin time and normal FXII and FXI. Acquired forms were excluded. Out of 106 patients, we have found that 45 patients had at least one chance-associated

Hypotension associated with prekallikrein activator (Hageman-factor fragments) in plasma protein fraction.

Thirteen lots of plasma protein fraction made by one manufacturer were implicated in 23 recent reports of hypotension in surgical patients. Four of these patients required resuscitation after rapid administration of the product in the postoperative period. All implicated lots had prekallikrein-activator activity but low levels of bradykinin and kallikrein. The prekallikrein activator was identified as Hageman-factor fragments by molecular weight (35,000 as estimated by gel chromatography), isoelectric point (4.2 to 4.4), and inhibition by antibody to Hageman factor. These data suggest that Hag

Severe Prekallikrein Deficiency Associated with Low Level of Factor XII: A Case Report.

Hereditary deficiency of plasma prekallikrein (PPK) is a rare autosomal recessive disease. The affected patients are often asymptomatic and diagnosed incidentally during preoperative investigations or during hospitalization by isolated prolongation of activated partial thromboplastin time (aPTT). In this article, we report, a 46-year-old woman who was candidate for two invasive procedures (thyroid FNA and hysterectomy) and underwent preoperative evaluation. Due to prolonged aPTT with normal PT she was referred to the IBTO reference coagulation laboratory for specific coagulation assays. Ultima

Plasma Prekallikrein Is Associated With Carotid Intima-Media Thickness in Type 1 Diabetes.

The hypothesis that plasma prekallikrein (PK) is a risk factor for the development of vascular complications was assessed in a study using the Diabetes Control and Complications Trial (DCCT)/Epidemiology and Diabetes Interventions and Complications (EDIC) cohort of subjects with type 1 diabetes. The circulating levels of plasma PK activity were measured in the plasma of 636 subjects with type 1 diabetes (EDIC years 3-5). Common and internal carotid intima-media thickness (IMT) were measured by B-mode ultrasonography in EDIC years 1 and 6. Plasma PK levels were positively and significantly asso

Prekallikrein deficiency presenting as recurrent cerebrovascular accident: case report and review of the literature.

We report the case of a woman with history of hypertension and hyperlipidemia presenting with recurrent episodes consistent clinically with cerebrovascular accidents (CVA), and MRI changes suggestive of ischemia versus vasculitis as their cause. No anatomical neurological, rheumatic, cardioembolic, or arteriosclerotic etiologies could be determined by extensive workup. Incidentally, the patient was found to have prolonged activated Partial Thromboplastin Time (aPTT) and a normal Prothrombin Time (PT); further testing revealed a prekallikrein deficiency. Since no other cause for the CVAs was es

Extracellular carbonic anhydrase mediates hemorrhagic retinal and cerebral vascular permeability through prekallikrein activation.

Excessive retinal vascular permeability contributes to the pathogenesis of proliferative diabetic retinopathy and diabetic macular edema, leading causes of vision loss in working-age adults. Using mass spectroscopy-based proteomics, we detected 117 proteins in human vitreous and elevated levels of extracellular carbonic anhydrase-I (CA-I) in vitreous from individuals with diabetic retinopathy, suggesting that retinal hemorrhage and erythrocyte lysis contribute to the diabetic vitreous proteome. Intravitreous injection of CA-I in rats increased retinal vessel leakage and caused intraretinal ede

Prekallikrein deficiency in a 15-year-old boy with Ménière's disease: a case report.

Congenital prekallikrein deficiency is a rare disorder in which there is an in vitro clotting defect despite absence of bleeding or thrombotic tendency. In this report, a 15-year-old boy with an unexpected markedly prolonged activated partial thrombin time, a normal prothrombin time, and without personal nor familial history of bleeding or thrombosis is presented. Laboratory investigation revealed a severe prekallikrein deficiency. This case highlights the importance of following a diagnostic algorithm to establish the correct diagnosis. Moreover, by selecting appropriate laboratory tests, unn

[The state of the blood prekallikrein-kallikrein system in patients with acute arterial obstruction of legs].

Distinct activation of the blood prekallikrein-kallikrein system was detected in patients with acute thromboses and embolism of aorta bifurcation and of leg magistral arteries. The rate of activation depended on the form of main disease responsible for acute arterial obstruction as well as on severity of legs ischemic impairments. Measurement of the blood prekallikrein-kallikrein system parameters enabled to evaluate more efficiently the severity of initial steps of legs ischemic impairments in patients with acute arterial occlusions as well as to predict the restorative operations success.

Activation of kinin-prekallikrein system after infusion of factor VIII concentrates in hemophilic patients.

Possible changes in the kinin-prekallikrein system after infusion of factor VIII concentrate were studied in seven patients with severe hemophilia. The functional and immunological activities of factors XI and XII, prekallikrein, high molecular weight kininogen, and C-1-esterase inhibitor were measured before and at 0.5, 2, 6, and 12 hours after infusion of 50 U/kg of factor VIII concentrate. A significant decrease (P less than 0.05) in the functional and immunological activities of prekallikrein was observed at 0.5 and 2 hours after factor VIII administration. The other proteins did not chang

Coagulation management in endovascular aortic repair in a patient with congenital prekallikrein deficiency: a case report.

Prekallikrein deficiency is a rare autosomal recessive coagulation disorder that prolongs the activated clotting time. Guidelines for safe intraoperative coagulation control in patients with prekallikrein deficiency undergoing endovascular aortic repair are limited. We present a case of a 70-year-old man with prekallikrein deficiency who underwent endovascular aortic repair and required intraoperative anticoagulation control. Intraoperative anticoagulation with heparin and protamine was successfully managed using the activated clotting time measurements after preoperative administration of two

Description and evaluation of a new chromogenic substrate assay kit for the determination of prekallikrein in human plasma.

Because of an increasing interest in the determination of prekallikrein a kit was made for the determination of this plasma proenzyme. The kit consists of 1) a prekallikrein activator of the cephalin-ellagic acid type containing Factor XII and HMW-kininogen to ensure a total activation of the prekallikrein even in pathological plasmas, 2) a buffer which is optimal for both activation and substrate hydrolysis and 3) the chromogenic substrate S-2302. A control plasma is also included. This kit was evaluated by thirteen research groups as well as by ourselves. Both normal and patient plasmas were

A Case of Severe Prekallikrein Deficiency Manifesting as Isolated Activated Partial Thromboplastin Time (aPTT) Prolongation.

We present the case of a 54-year-old female who was admitted to the medicine service for a left foot ulcer with cellulitis requiring hallux amputation. Pre-surgical laboratory findings were significant for an isolated elevation in activated partial thromboplastin time (aPTT). Hematology was consulted for evaluation of prolonged aPTT prior to surgery given potential for bleeding risk. Further laboratory studies obtained were significant for normal factor levels, negative hypercoagulable workup, normal kininogen activity, and severe prekallikrein deficiency. The patient was pre-operatively given

Long-term safety outcomes of prekallikrein (Fletcher factor) deficiency: A systematic literature review of case reports.

Background: Hereditary prekallikrein (Fletcher factor) deficiency is a rare condition characterized by a prolonged activated partial thromboplastin time. Inhibitors of plasma kallikrein have recently been approved for prophylaxis of hereditary angioedema and are under investigation for use in other indications. Objective: We attempted to conservatively assess the impact of long-term inhibition of this pathway by reviewing reported comorbidities in patients with hereditary prekallikrein deficiency. Methods: We searched several medical literature databases for publications that reported data fro

Double Trouble: Combined Prekallikrein and IgA Deficiencies in a Patient Undergoing Orthotopic Heart Transplantation-A Case Report.

Prekallikrein (PK) and selective IgA deficiencies are rare, and their coexistence in a cardiac transplant patient presents unique challenges. These disorders affect anticoagulation monitoring and transfusion safety, necessitating tailored perioperative strategies. We present the case of a 58-year-old female with both PK and IgA deficiency who underwent orthotopic heart transplantation (OHT), complicated by significant postoperative bleeding requiring re-exploration. A multidisciplinary plan included FFP-based IgA desensitization and correction of elevated ACT from PK deficiency. Anti-Xa levels
