Preeclampsia and retinopathy of prematurity in preterm births.

OBJECTIVE: The relationship between gestational hypertension, preeclampsia, and the risk of retinopathy of prematurity (ROP) remains unclear. Thus, we used a large cohort database to study the influence of maternal gestational hypertension and preeclampsia on the occurrence of ROP in preterm infants. METHODS: We used data from a previous retrospective cohort study that includes 25,473 eligible preterm neonates. We examined the association between gestational hypertension, preeclampsia, and ROP while controlling for potential confounders by multiple logistic regression analysis. RESULTS: Of t

Late preterm birth: preventable prematurity?

Prematurity is one of the leading causes of infant morbidity and mortality globally. Over the years, however, advances in medicine and technology have enhanced the ability to care for babies at very early gestations. There has also been a shift in the distribution of births away from term/post-term gestations and toward earlier gestational ages. These changes have added to the burden of premature births. The focus of this article is to present both sides of the story, one that highlights the many problems and morbidities faced by this subgroup of premature infants and the other that justifies

[The significance of myopia and birth weight in prematurity].

AIM: The relation between myopia and birth weight of premature newborns was assessed in this study. MATERIAL AND METHODS: There were 90 children (180 eyes) involved in the study, their birth weight ranged between 580 and 2450 g (mean 1698), and myopia ranged from -1.5 to -22.5 D (mean -9.8 D). Children were divided into two groups: group I: 38 children (76 eyes) with birth weight from 580 to 1500 g (mean 1158 g), group II: 52 children (104 eyes) with birth weight from 1501 to 2450 g (mean 2093 g). Mean value of myopia in group I was -10.4 D and -9.4 D in group II. RESULTS: The results were a

A review of premature birth and subclinical infection.

Premature birth causes high rates of neonatal morbidity and mortality. There are multiple causes of preterm birth. This article reviews the evidence linking subclinical infection and premature birth. Although maternal genital tract colonization with specific organisms has been inconsistently associated with preterm birth and/or premature rupture of membranes, some infections have been consistently associated with preterm delivery. The association of histologic chorioamnionitis with prematurity is a consistent finding, but the mechanisms require further study. The relationship between histologi

Paediatric preparation for multiple premature births.

We describe the managment of 2 sets of pre-term quadruplets born to 2 women pregnant concurrently following the induction of ovulation with gonadotrophin. Careful antenatal planning was necessary to provide for each of the 8 pre-term infants a standard of neonatal medical and nursing care comparable to that normally available for singleton pre-term infants.

From premature birth to premature kidney disease: does accelerated aging play a role?

As the limits of fetal viability have increased over the past 30 years, there has been a growing body of evidence supporting the idea that chronic disease should be taken into greater consideration in addition to survival after preterm birth. Accumulating evidence also suggests there is early onset of biologic aging after preterm birth. Similarly, chronic kidney disease (CKD) is also associated with a phenotype of advanced biologic age which exceeds chronologic age. Yet, significant knowledge gaps remain regarding the link between premature biologic age after preterm birth and kidney disease.

The role of complement in preterm birth and prematurity.

Complications of preterm birth (PTB) are the global leading cause of death in children younger than 5 years of age. Almost 15 million children are born prematurely in the world each year. Increasing evidence suggests that labor and delivery have many hallmarks of an inflammatory reaction, where complement activation has an active participation. As one of the most important components of inflammation, the role of complement during labor and PTB is becoming an attractive research target. The complement components C1q and C5b-9 are deposited on fetal membranes and release inflammatory mediators t

Risk Factors for premature birth in a hospital.

OBJECTIVE: to determine the risk factors for premature birth. METHODS: retrospective case-control study of 600 pregnant women assisted in a hospital, with 298 pregnant women in the case group (who gave birth prematurely <37 weeks) and 302 pregnant women who gave birth to a full-term newborn in the control group. Stata software version 12.2 was used. The Chi-square test was used in bivariate analysis and logistic regression was used in multivariate analysis, from which Odds Ratios (OR) and Confidence Intervals (CI) of 95% were derived. RESULTS: risk factors associated with premature birth wer

[Ranking of cerclage in prophylaxis of premature births].

The results of 196 cases after operation for cerclage are reported. In 133 patients the operation was performed because of cervical incompetence with opening of the cervical canal without contractions. In 63 patients a prophylactic operation was performed for anamnestic reasons. In the group of cervical incompetence the operation resulted in 103 (94.5%) deliveries, 12 of which occurred before the 35th week of gestation. Following prophylactic cerclage, 46 (78.0%) pregnancies ended with a delivery. In the latter group, the rate of abortions was four times higher than in the group with cervical

Beyond survival: the lasting effects of premature birth.

Preterm birth, defined as birth before 37 weeks of gestation, is a major public health concern. It affects about 10% of all newborns globally and is the main cause of infant death and morbidity. Prematurity increases the likelihood of respiratory distress syndrome, cerebral palsy, and developmental abnormalities. Furthermore, premature newborns are at risk of acquiring chronic noncommunicable diseases later in life due to interference with organ system development during the in-utero and perinatal period. Because of the greater risk of long-term repercussions, preterm birth should be considere

Association of Maternal Preeclampsia With Infant Risk of Premature Birth and Retinopathy of Prematurity.

IMPORTANCE: Studies report conflicting associations between preeclampsia and retinopathy of prematurity (ROP). This study provides explanations for the discrepancies to clarify the relationship between preeclampsia and ROP. OBJECTIVE: To evaluate the association of maternal preeclampsia and risk of ROP among infants in an unrestricted birth cohort and a restricted subcohort of preterm, very low birth weight (P-VLBW) infants. DESIGN, SETTING, AND PARTICIPANTS: A retrospective review of 290 992 live births within the Intermountain Healthcare System in Utah from January 1, 2001, through Decembe

Increased risk of pulmonary hypertension following premature birth.

BACKGROUND: Pulmonary hypertension (PAH) among children and adults has been linked to premature birth, even after adjustments for known risk factors such as congenital heart disease and chronic lung disease. The aim of this population-based registry study was to assess the risk of PAH following exposure to premature birth and other factors in the decades when modern neonatal care was introduced and survival rates increased. METHODS: Data on pulmonary hypertension and perinatal factors were retrieved from population-based governmental and national quality registers. Cases were adults and child

Serum prohepcidin concentrations at birth and 1 month after birth in premature infants.

BACKGROUND: Premature newborns are vulnerable to iron imbalance, although the iron homeostasis during the perinatal period remains unclear. To clarify the iron metabolism of premature infants, we measured serum prohepcidin concentrations of preterm infants, and analyzed the association with iron parameters. METHODS: Seventy-one (61 preterm and 10 term) infants were enrolled for the study, that had no underlying diseases including asphyxia, bleedings, infection, and anomalies. Serum concentrations of prohepcidin at birth and 1 month after birth were determined by enzyme-linked immunosorbent as
