For many older patients, surgery is an inevitable component of healthcare. Thorough and effective preoperative patient evaluation is imperative because comorbidities, polypharmacy, functional problems, and socio-economic issues all can affect perioperative decisions and complicate recovery. A methodical assessment includes a history and physical exam, review of prescription and nonprescription medications, evaluation of major organ systems, and discussion with the patient of advance directives. Other major considerations include functional status, the risk level of the surgery to be performed,

The effect of flubendazole in human hydatic disease due to E. granulosus has been studied in 22 patients with one or several hydatic cysts of different sites. Flubendazole has been given orally before surgery for a 10 days mean period, at 4 g daily in adults and 1 g daily in children. Flubendazole has been titrated by radioimmuno-assay in sera, urines and bile taken 12 to 24 hours after the last ingestion and in the hydatic cyst (membranes and liquid). The scolex vitality was determined by direct optic microscopy and by intra-peritoneal mouse inoculation. Seven non-treated patients, without hy

Optimum success may be obtained from those incontinence operations which form one coherent unity with presurgical and postsurgical measures. Full elucidation of the symptom of urinary incontinence has proved to be the most important preoperative necessity. More comprehensive urometric objectivation will be required for problems which cannot be answered by a minimum programme. Time-consuming diagnosis should be, basically, accompanied by conservative incontinence treatment, primarily for three reasons, substantiation of diagnosis, avoidance of meaningless surgery, and improvement of conditions

An increasing proportion of surgical procedures involves elderly and frail patients in high-income countries, leading to an increased risk of postoperative complications. Complications significantly impact patient outcomes and costs, due to prolonged hospitalization and loss of autonomy. Consequently, it is crucial to evaluate preoperative functional status in older patients, to tailor the perioperative plan, and evaluate risks. The hospital environment often exacerbates cognitive impairments in elderly and frail patients, also increasing the risk of infection, falls, and malnutrition. Thus, i

Delirium is a common and serious neuropsychiatric syndrome affecting older adults, particularly after surgery. It manifests in three forms: hypoactive, hyperactive, and mixed, with symptoms ranging from lethargy and inattention to agitation and confusion. Delirium is often misdiagnosed or overlooked, especially in its hypoactive form, which can delay treatment and worsen patient outcomes. Distinguishing delirium from dementia is crucial, as delirium is typically reversible with prompt intervention. This article, presented in a case study format, explores the assessment and management of deliri

Hygiene is becoming more and more important in long-term care facilities. Long-term care facilities are subject to monitoring by the Public Health Service (PHS) and other authorities. For the PHS in Bavaria the Bavarian Health and Food Safety Authority (Landesamt für Gesundheit und Lebensmittelsicherheit, LGL) published a hygiene monitoring concept and there exists an inspection guide developed by a specialist department for nursing homes and institutions for the handicapped (Fachstelle für Pflege und Behinderteneinrichtungen, FQA). Because inspections are performed in multiprofessional teams,

Preoperative echocardiography has long been used as an effective diagnostic tool in the preoperative assessment of surgical candidates. In elderly patients, hip fractures represent a critical medical emergency with a high risk of complications that can occur if not addressed promptly. Unnecessary preoperative investigations may lead to delays and potentially compromise the quality of patient care. The motivation to evaluate the utility of preoperative echocardiography in select clinical scenarios, particularly in low- and middle-income countries (LMICs) where resources are limited, arose from
