2018 Update on Medical Overuse.

IMPORTANCE: Overuse of medical care is a well-recognized problem in health care, associated with patient harm and costs. We sought to identify and highlight original research articles published in 2017 that are most relevant to understanding medical overuse. OBSERVATIONS: A structured review of English-language articles published in 2017 was performed, coupled with examination of tables of contents of high-impact journals to identify articles related to medical overuse in adult care. Manuscripts were appraised for their quality, clinical relevance, and impact. A total of 1446 articles were id

Worldwide pattern of antibiotic prescription in endodontic infections.

BACKGROUND: Odontogenic infections, and especially endodontic infections, are polymicrobial, involving a combination of Gram-positive and Gram-negative facultative anaerobes and strictly anaerobic bacteria. Therefore, antibiotics can be used as an adjunct to endodontic treatment. However, most chronic and even acute endodontic infections can be successfully managed by disinfection of the root-canal system, which eliminates the source of infection, followed by abscess drainage or tooth extraction, without the need for antibiotics. The literature provides evidence of inadequate prescribing pract

Nonsteroidal Anti-inflammatory Drugs (NSAIDs) and Collision Sports: The Need for Further Education in Rugby.

Rugby is a collision sport and the fastest growing sport in the United States. Nonsteroidal anti-inflammatory drugs (NSAIDs), which can be acquired without prescription, are used by millions of individuals daily. Athletes in collision sports such as rugby overuse them, and the management of these analgesic anti-inflammatory drugs is gaining importance to healthcare providers in sports because of their potential organ toxicities. Athletes, amateur and elite, may misconstrue the perceived advantages of NSAIDs and seem to be unaware of the serious complications arising from their overuse. This ov

Making a case for dental clinical practice guidelines for elective antibiotic use: an institutional retrospective study.

BACKGROUND: Antibiotic overuse disrupts host-microbiome equilibrium, and increases risks of diarrhea, drug allergies, and multi-drug resistance. Dentists are the third largest prescribers of oral antibiotics in the US. Investigating antibiotic prescribing patterns alongside dental procedures may identify opportunities to reduce antibiotic overuse. METHOD: Institutional antibiotic prescriptions (Abx-Rx) dispensed with dental procedures were retrospectively reviewed from 2017-2022. Augmentin® (USAntibiotics, TN) and clindamycin prescriptions were investigated for nonindicated vs. well-reasoned

Medication overuse in a subgroup analysis of phase 3 placebo-controlled studies of galcanezumab in the prevention of episodic and chronic migraine.

INTRODUCTION: Acute medication overuse is prevalent in patients with migraine. METHODS: In three phase 3, double-blind, randomized, placebo-controlled studies, patients with episodic migraine (EVOLVE-1 and EVOLVE-2) or chronic migraine (REGAIN) were randomized 2:1:1 to monthly subcutaneous injections of placebo or galcanezumab 120 or 240 mg for 3 or 6 months. This subgroup analysis evaluated mean changes in the number of monthly migraine headache days in each treatment among patients with versus without baseline acute medication overuse via mixing modelling with repeated measures. RESULTS: T

Effectiveness at long-term of a multidisciplinary intervention in the reduction of overuse of benzodiazepine prescriptions: A cluster controlled trial.

BACKGROUND: The overuse of Benzodiazepines (BZD) has become an area of concern because of its consequences on patients' health and healthcare costs. OBJECTIVE: to determine whether a multicomponent intervention based on a multidisciplinary education program is effective in withdrawal from BZD long-term use, compared to usual care. METHODS: Cluster-randomized, parallel-group trial with health centres as units of randomization and patients as units of analysis. INCLUSION CRITERIA: adults with more than 4 weeks of BZD use, without serious mental disorder, non-terminal, without alcohol dependen

Medication overuse and drug addiction: a narrative review from addiction perspective.

Chronic headache is particularly prevalent in migraineurs and it can progress to a condition known as medication overuse headache (MOH). MOH is a secondary headache caused by overuse of analgesics or other medications such as triptans to abort acute migraine attacks. The worsening of headache symptoms associated with medication overuse (MO) generally ameliorates following interruption of regular medication use, although the primary headache symptoms remain unaffected. MO patients may also develop certain behaviors such as ritualized drug administration, psychological drug attachment, and withd

Anti-CGRP monoclonal antibodies in chronic migraine with medication overuse: real-life effectiveness and predictors of response at 6 months.

BACKGROUND: In daily practice, anti-CGRP monoclonal antibodies (MAbs) may be useful in chronic migraine (CM) with medication overuse (MO), but data is limited. We evaluated their effectiveness in a real-life clinical cohort. METHODS: This is a prospective study conducted in CM patients with and without medication overuse treated with monthly MAbs during 6 months (erenumab/galcanezumab). We collected headache characteristics, including acute medication intake, through an electronic diary. We compared patients (1) with and without MO at baseline, (2) with and without ongoing MO after treatment,

Dependence on Prescription Opioids.

BACKGROUND: The incidence of initial prescriptions of opioids for chronic non-cancer pain rose by 37% in Germany from 2000 to 2010. Prescribing practice does not always conform with the recommendations of current guidelines. In the USA, 8-12% of patients with chronic non-cancer pain are opioid-dependent. METHODS: This review is based on publications retrieved by a selective PubMed search and on the German S3 guideline on the long-term use of opioids in non-cancer pain. RESULTS: Patients must be informed and counseled about the effects and risks of opioids before these drugs are prescribed. A

Burden of migraine with acute medication overuse or psychiatric comorbidities and treatment with CGRP pathway-targeted monoclonal antibodies: A review.

Migraine is a complex and often debilitating neurological disease that affects more than 1 billion people worldwide. It is characterized by moderate-to-intense, throbbing headache attacks that are worsened by activity and is associated with nausea, vomiting, and sensitivity to light and sound. Migraine, ranked the second leading cause of years lived with disability by the World Health Organization, can diminish patients' quality of life and bring significant personal and economic burden. Furthermore, migraine patients with a history of acute medication overuse (AMO) or psychiatric comorbiditie
