OBJECTIVE: Primary graft dysfunction (PGD) remains the leading cause of early morbidity and mortality after orthotopic heart transplantation. Postoperative lactate and the venoarterial carbon dioxide gap (Pv-a CO2) are routinely measured indicators of hypoperfusion, yet their relationship to PGD severity is not well characterized. This study evaluated the association between immediate posttransplant (T0) and 24-hour (T24) biomarker values and the severity of PGD.
METHODS: We performed a single-center retrospective study of adult recipients of orthotopic heart transplantation (May 2020 to May

Background Previous studies suggest that infant heart transplant (HT) recipients are at higher risk of developing severe primary graft dysfunction (PGD) than older children. We sought to identify risk factors for developing severe PGD in infant HT recipients. Methods and Results We identified all HT recipients aged <1 year in the United States during 1996 to 2015 using the Organ Procurement and Transplant Network database. We linked their data to ELSO (Extracorporeal Life Support Organization) registry data to identify those with severe PGD, defined by initiation of extracorporeal membrane oxy

BACKGROUND: Primary graft dysfunction grading is an important prognostic parameter following lung transplantation. Because the current ISHLT consensus classification does not differentiate between mechanically ventilated and extubated patients, we examined whether mechanical ventilation status influences the prognostic value of PGD grades for short- and long-term outcomes.
METHODS: All primary bilateral lung transplants performed at the Medical University of Vienna and Vanderbilt University Medical Center (January 2017 - December 2024) were retrospectively analyzed. PGD was adjudicated per th

Early graft dysfunction following lung transplantation is commonly attributed to primary graft dysfunction (PGD). However, other rare etiologies may present with similar clinical and radiologic features. We report a unique case of a 58-year-old male with idiopathic pulmonary fibrosis who underwent bilateral lung transplantation. The donor was a young adult with traumatic brain injury. Within hours post-transplant, the recipient developed severe hypoxemia and diffuse infiltrates on chest imaging, consistent with Grade 3 PGD. Despite supportive therapy, there was limited improvement. Histopathol

Background: Primary graft dysfunction (PGD) is the leading cause of early mortality after lung transplantation. Albumin is commonly used during lung transplantation to maintain intravascular volume while minimizing total intravenous fluid administration, given the established association between larger intravenous fluid and PGD. However, the direct impact of albumin on PGD remains unclear. Methods: We conducted a single-center retrospective cohort study of lung transplant recipients between 2018 and 2023. We calculated the corrected albumin proportion (cAP), representing the ratio of albumin t

Primary graft dysfunction (PGD) is a major determinant of morbidity and mortality following lung transplantation. Delineating basic mechanisms and molecular signatures of PGD remain a fundamental challenge. This pilot study examines if the pulmonary volatile organic compound (VOC) spectrum relate to PGD and postoperative outcomes. The VOC profiles of 58 bronchoalveolar lavage fluid (BALF) and blind bronchial aspirate samples from 35 transplant patients were extracted using solid-phase-microextraction and analyzed with comprehensive two-dimensional gas chromatography coupled to time-of-flight m

Although primary graft dysfunction (PGD) is fairly common early after cardiac transplant, standardized schemes for diagnosis and treatment remain contentious. Most major cardiac transplant centers use different definitions and parameters of cardiac function. Thus, there is difficulty comparing published reports and no agreed protocol for management. A consensus conference was organized to better define, diagnose, and manage PGD. There were 71 participants (transplant cardiologists, surgeons, immunologists and pathologists), with vast clinical and published experience in PGD, representing 42 he

Primary graft dysfunction (PGD), the most extensively studied phenotype within the evolving spectrum of early graft dysfunction (EGD), remains the leading cause of early mortality after heart transplantation and continues to limit broader utilization of marginal donor organs. Despite advances in donor preservation strategies and perioperative management, the mechanisms underlying PGD remain incompletely understood. Traditionally, PGD has been conceptualized as a manifestation of ischemia-reperfusion injury (IRI) leading to early graft contractile failure. Accumulating evidence suggests that im

There is an urgent need to better understand the pathophysiology of primary graft dysfunction (PGD) to develop point-of-care methods predicting those at risk. We utilized a multiplex multivariable approach to define cytokines, chemokines, and growth factors in patient-matched biospecimens to identify factors predictive of PGD. Biospecimens were collected from patients undergoing bilateral lung transplantation (LTx) from three sites: donor lung perfusate, post-transplant bronchoalveolar lavage (BAL) fluid (2h), and plasma (2h, 24h, 72h, and 1 and 2 wks). A 71-multiplex panel was performed on ea

BACKGROUND: Electrical storm in Chagas disease represents a life-threatening scenario, particularly in Latin America.
CASE SUMMARY: A 62-year-old man developed recurrent ventricular tachycardia a decade after implantable cardioverter-defibrillator placement for Chagas cardiomyopathy. Despite optimized therapy, arrhythmia control was not achieved, culminating in refractory electrical storm. He underwent heart transplantation. Severe primary graft dysfunction developed intraoperatively. Peripheral venoarterial extracorporeal membrane oxygenation was initiated, unloading with an intra-aortic bal

BACKGROUND: Primary graft dysfunction (PGD) is a major cause of morbidity and mortality after lung transplantation (LTx). PGD is graded at static time points, limiting insight into its temporal dynamics. Statistical risk-factor analysis may overlook the multifactorial complexity of PGD. Machine learning (ML) may address this but is constrained by small sample sizes. We aim to introduce a temporal PGD classification, perform ML-based clustering and overcome sample-size limitations by generating synthetic patient data.
METHODS: A prospectively collected database of 794 LTx at University Hospita

OBJECTIVES: Histidine-rich glycoprotein has been reported as an anti-inflammatory glycoprotein that inhibits acute lung injury in mice with sepsis and as a prognostic biomarker in patients with sepsis. We investigated the relationship between plasma concentrations of histidine-rich glycoprotein and the risk of occurrence of primary graft dysfunction.
METHODS: According to the primary graft dysfunction grade at post-transplant 72 h, patients who underwent lung transplantation were divided into three groups: non-primary graft dysfunction group (grade 0-1), moderate primary graft dysfunction gro

Lung allograft recipients have worse survival than all other solid organ transplant recipients, largely because of primary graft dysfunction (PGD), a major form of acute lung injury affecting a third of lung recipients within the first 72 h after transplant. PGD is the clinical manifestation of ischemia-reperfusion injury and represents the predominate cause of early morbidity and mortality. Despite PGD's impact on lung transplant outcomes, no targeted therapies are currently available; hence, care remains supportive and largely ineffective. This review focuses on molecular and innate immune m

BACKGROUND: Primary graft dysfunction is a major cause of early mortality following lung transplantation. The International Society for Heart and Lung Transplantation subdivides it into 4 grades of increasing severity.
METHODS: A retrospective review of the institutional lung transplant database from March 2018 to September 2021 was performed. Patients were stratified into three groups: primary graft dysfunction grade 0 patients, grade 1 or 2 patients, and grade 3 patients. Recipient, donor, and surgical variables were analyzed by logistic regression analysis to identify risk factors for prim

BACKGROUND: Primary graft dysfunction (PGD) and acute kidney injury (AKI) are major early complications of lung transplantation and are associated with increased mortality. Lung injury after PGD can contribute to renal dysfunction; however, the association between PGD and AKI severity has not been thoroughly investigated. We analyzed the association between PGD grading and AKI staging, and the impact of AKI on subsequent changes to chronic kidney disease (CKD), including glomerular filtration rate (GFR), over time.
METHODS: This was a retrospective review of a single-center lung transplantati

BACKGROUND: Primary graft dysfunction (PGD) is the leading cause of early morbidity and mortality after lung transplantation. Accurate prediction of PGD risk could inform donor approaches and perioperative care planning. We sought to develop a clinically useful, generalizable PGD prediction model to aid in transplant decision-making.
METHODS: We derived a predictive model in a prospective cohort study of subjects from 2012 to 2018, followed by a single-center external validation. We used regularized (lasso) logistic regression to evaluate the predictive ability of clinically available PGD pre

BACKGROUND: Heart transplantation (HTx) remains the most effective long-term treatment for advanced heart failure. Primary graft dysfunction (PGD) continues to be a potentially life-threatening early complication. In 2014, a consensus statement released by International Society for Heart and Lung Transplantation (ISHLT) established diagnostic criteria for PGD. We studied the incidence of PGD across the United Kingdom.
METHODS: We analyzed the medical records of all adult patients who underwent HTx between October 2012 and October 2015 in the 6 UK heart transplant centers Preoperative donor an

Primary graft dysfunction (PGD), as characterized by pulmonary infiltrates and high oxygen requirements shortly after reperfusion, is the major cause of early morbidity and mortality after lung transplantation. Donor, recipient and allograft-handling factors are thought to contribute, although new insights regarding pathogenesis are needed to guide approaches to prevention and therapy. Mast cells have been implicated in ischemic tissue injury in other model systems and in allograft rejection, leading to the hypothesis that mast cell degranulation contributes to lung injury following reperfusio
