Advocating for Patients With Inflammatory Bowel Disease: How to Navigate the Prior Authorization Process.

In an effort to manage health care costs and avoid improper medication use, prior authorizations (PAs) have become a standard stipulation required by payers in the determination of medication coverage. For gastroenterologists managing patients, especially those with inflammatory bowel disease (IBD), the PA process is time-consuming and further complicated by 2 additional factors: step therapy requirements and failure of payers to recognize updated IBD treatment pathways. These factors often lead to PA denials and cause treatment delays, which in turn can lead to disease progression, ongoing pa

The association between cost sharing, prior authorization, and specialty drug utilization: A systematic review.

BACKGROUND: Specialty drugs are identified by high monthly costs and complexity of administration. Payers use utilization management strategies, including prior authorization and separate tiers with higher cost sharing, to control spending. These strategies can negatively impact patients' health outcomes through treatment initiation delays, medication abandonment, and nonadherence. OBJECTIVE: To examine the effect of patient cost sharing on specialty drug utilization and the effect of prior authorization on treatment delay and specialty drug utilization. METHODS: We conducted a literature sear

Analysis of prior authorization success and timeliness at a community-based specialty care pharmacy.

BACKGROUND: Specialty medications may require a prior authorization (PA) before a patient can access the medication. Providers often identify PA approval as a burden for the practice. Pharmacists can facilitate the completion of the PA process. OBJECTIVE: The primary objective was to evaluate the time to first PA decision (approval or denial) for dermatologic medications dispensed by a community-based specialty pharmacy. A secondary objective was to compare PA timeliness (time to PA approval and time to first medication fill) between a community-based specialty pharmacy and a dermatology prov

Implementation and workload impact of a Veterans Integrated Service Network (VISN) centralized prior authorization program.

PURPOSE: This retrospective study evaluated the primary objective of whether centralizing prior authorization drug request (PADR) adjudication across 4 Veterans Integrated Service Network (VISN) 21 healthcare systems reduced local pharmacist workload while maintaining or improving timely prior authorization completion. The study was determined to be institutional review board exempt. SUMMARY: In 2019, VISN 21 piloted a centralized PADR program, funded by the Veterans Health Administration Pharmacy Benefit Management. The program aimed to standardize prior authorization processes, promote phar

State Laws Banning Prior Authorization For Medications For Opioid Use Disorder Increased Substantially, 2015-23.

Prior authorization is a barrier to accessing medications for opioid use disorder (MOUD). Although private insurance covered about one-third of patients with OUD in 2023, the understanding of prior authorization prohibitions in private insurance remains limited. We synthesized state laws prohibiting prior authorization for MOUD in private insurance in the US. The number of states with such prohibitions grew from two in 2015 to twenty-two in 2023, with variations in the scope of these prohibitions. Seven states have fully prohibited prior authorization for all MOUD ("full prohibitions") since t

Coverage and Prior Authorization Policies for Medications for Opioid Use Disorder in Medicaid Managed Care.

IMPORTANCE: Medicaid is a key policy lever to improve opioid use disorder treatment, covering approximately 40% of Americans with opioid use disorder. Although approximately 70% of Medicaid beneficiaries are enrolled in comprehensive managed care organization (MCO) plans, little is known about coverage and prior authorization (PA) policies for medications for opioid use disorder (MOUD) in these plans. OBJECTIVE: To compare coverage and PA policies for buprenorphine, methadone, and injectable naltrexone across Medicaid MCO plans and fee-for-service (FFS) programs and across states. DESIGN, SE

Pharmacist-initiated prior authorization process to improve patient care in a psychiatric acute care hospital.

A prior authorization (PA) is a requirement implemented by managed care organizations to help provide medications to consumers in a cost-effective manner. The PA process may be seen as a barrier by prescribers, pharmacists, pharmaceutical companies, and consumers. The lack of a standardized PA process, implemented prior to a patient's discharge from a health care facility, may increase nonadherence to inpatient prescribed medications. Pharmacists and other health care professionals can implement a PA process specific to their institution. This article describes a pharmacist-initiated PA proces

Disparities in AI-Based Prior Authorization for Head and Neck Reconstruction: A Large Language Model Analysis.

INTRODUCTION: Large language models (LLM) are being rapidly integrated into healthcare, particularly to streamline time- and labor-intensive administrative processes. However, the potential for artificial intelligence (AI) systems to demonstrate bias when employed for insurance authorization remains poorly understood. As insurers increasingly adopt AI to make coverage decisions, this study examined bias in LLM-driven prior authorization in otolaryngology, using oral cavity squamous cell carcinoma (SCC) as a case study. METHODS: Using OpenAI's generative transformer GPT-4o, this study assessed

Economic Impact of Anti-Vascular Endothelial Growth Factor Prior Authorization Process From a Large Electronic Database.

Purpose:To determine the economic impact of insurance company prior authorizations (PAs) for anti-vascular endothelial growth factor (anti-VEGF) therapies on society. Methods: PA denial and delay rates were derived from a large electronic database (SamaCare PA). Data were analyzed in a theoretical cost-effectiveness model to calculate the increased costs of PAs (in 2025 U.S. dollars). Results: Of 33 178 total PA requests, the majority were from Medicare Advantage plans (18 769, 58.2%), followed by commercial insurance carriers (10 047, 31.1%) and Medicaid (3438, 10.7%). Commercial carriers had

Addressing health disparities in food allergy: A Position Statement of the AAAAI Prior Authorization Task Force.

Self-reported food allergies (FAs) affect approximately 8% of the US pediatric and approximately 10% of the adult population, which reflects potentially disproportionate increases among ethnically and racially minoritized groups. Multiple gaps and unmet needs exist regarding FA disparities. There is reported evidence of disparities in FA outcomes, and the FA burden may also be disproportionate in low-income families. Low family income has been associated with higher emergency care spending and insecure access to allergen-free food. Pharmacoinequity arises in part as a result of structural raci

Delay in prior authorization of biologic therapy: Another possible cause of healthcare disparity in IBD patients.

BACKGROUND: Biologics, a mainstay in inflammatory bowel disease (IBD) treatment, typically require prior authorization from insurance companies. Multiple studies show that African Americans are less likely to be prescribed biologics. The prior authorization process may perpetuate disparities in healthcare. This study evaluated the approval time for biologics in IBD. METHODS: A chart review of IBD patients seen in a university gastroenterology clinic over 5 years was performed. Patient gender, race, IBD subtype, biologic use, and insurance type were recorded. Insurance type was classified as p

Association of Prior Authorization and Out-of-pocket Costs With Patient Access to PCSK9 Inhibitor Therapy.

IMPORTANCE: Although PCSK9 inhibitors (PCSK9i) were approved in 2015, their high cost has led to strict prior authorization practices and high copays, and use of PSCK9i in clinical practice has been low. OBJECTIVE: To evaluate patient access to PCSK9i among those prescribed therapy. DESIGN, SETTING, AND PARTICIPANTS: Using pharmacy transaction data, we evaluated 45 029 patients who were newly prescribed PCSK9i in the United States between August 1, 2015, and July 31, 2016. MAIN OUTCOMES AND MEASURES: The proportion of PCSK9i prescriptions approved and abandoned (approved but unfilled); mult

A+KIDS, a web-based antipsychotic registry for North Carolina youths: an alternative to prior authorization.

OBJECTIVE: The rise in use of antipsychotics among U.S. children is well documented. Compliance rates with current safety-monitoring guidelines are low. In response, the North Carolina Division of Medical Assistance established the Antipsychotics-Keeping It Documented for Safety (A+KIDS) registry. The initial objectives of the project were to successfully establish a Web-based safety registry and to obtain and evaluate clinical information derived from the registry. METHODS: In April 2011, A+KIDS began asking prescribers of antipsychotics for children age 12 and under to respond to a set of q

Impact of Prior Authorization Suspension on Access to Diabetes Technology in an Underserved Population: A Retrospective Analysis.

OBJECTIVE: Because of a computer-based prior authorization (PA) centralized system failure, Medi-Cal authorities temporarily suspended PA requirements for continuous glucose monitoring (CGM) and insulin pumps, including automated insulin delivery systems, for 18 months. We assessed whether this PA-free period improved access to diabetes technology or impacted glycemic control. RESEARCH DESIGN AND METHODS: We conducted a retrospective chart review of 105 adults with type 1 diabetes in a safety net clinic across two sequential policy periods: PA-required and PA-free. Observational outcomes incl

Medicaid Managed Care Prior Authorization For Buprenorphine Tied To State Partisanship And Health Plan Profit Status, 2018.

Buprenorphine is among the most effective drugs for treating opioid use disorder, yet only a quarter of Americans who need it receive it. Requiring prior authorization has been identified as an important barrier to buprenorphine access. However, the practice remains widespread in Medicaid-the largest insurer of Americans with opioid use disorder. In this study, we examined how prior authorization for buprenorphine is related to plan structure and state political environment, using data on all 266 comprehensive Medicaid managed care plans active in 2018. We found substantial variation in prior

Prior Authorization Requirements for Proprotein Convertase Subtilisin/Kexin Type 9 Inhibitors Across US Private and Public Payers.

BACKGROUND: Proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9is) are an innovative treatment option for patients with familial hypercholesterolemia or clinical atherosclerotic cardiovascular disease who require further lowering of low-density lipoprotein cholesterol. However, the high costs of these agents have spurred payers to implement utilization management policies to ensure appropriate use. We examined prior authorization (PA) requirements for PCSK9is across private and public US payers. METHODS AND RESULTS: We conducted an analysis of 2016 formulary coverage and PA data f

Prior Authorization for Sacral Nerve Stimulation in the Era of WISeR: Evaluation of U.S. Payer Coverage Policies.

INTRODUCTION: Sacral nerve stimulation (SNS) is an established therapy for refractory urinary incontinence, yet the 2025 Wasteful and Inappropriate Service Reduction Model (WISeR) designated it as requiring mandatory prior authorization. No analysis has examined how U.S. payers translate SNS evidence into coverage policies or whether restrictions are evidence-based. We sought to: (1) compare covered and excluded indications, (2) evaluate restriction criteria and conservative therapy requirements, and (3) assess quality of evidence cited. METHODS: This was a systematic, retrospective analysis

Prior authorization restrictions on medications for opioid use disorder: trends in state laws from 2005 to 2019.

RESEARCH OBJECTIVE: Medications for opioid use disorder (MOUDs) - including methadone, buprenorphine, and naltrexone - are the most effective treatments for opioid use disorder (OUD). Historically, insurers have required prior authorization for MOUD, but prior authorization is often reported as a key barrier to MOUD prescribing. Some states have passed laws prohibiting MOUD prior authorization requirements. We sought to identify the frequency of MOUD prior authorization prohibitions in state laws and to categorize types of prohibitions. METHODS: We searched for regulations and statutes presen
