Role of Procalcitonin in Respiratory Infections.

BACKGROUND: Respiratory infections remain a leading cause of morbidity and mortality worldwide, encompassing a broad spectrum of conditions ranging from mild upper respiratory tract infections to severe pneumonia and sepsis. One of the major clinical challenges in their management is differentiating bacterial from viral infections, which has direct implications for appropriate antibiotic use. Overuse and misuse of antibiotics contribute significantly to antimicrobial resistance, making the identification of reliable biomarkers increasingly important. Procalcitonin has emerged as a promising bi

Procalcitonin: A new biomarker for the cardiologist.

Due to its high accuracy for the diagnosis of bacterial infections, the inflammatory biomarker procalcitonin (PCT) is increasingly being used in patients with suspected infection. In patients with infections of the respiratory tract, it allows rapid rule out of bacterial etiology and facilitates decisions pertaining to antibiotic management. A growing body of evidence also supports PCT testing in patients with cardiovascular disorders including, but not limited to, those with shortness of breath, possible heart failure, suspected endocarditis, and acute coronary syndromes. In these clinical si

Procalcitonin as an acute phase marker.

Procalcitonin is a 14-kDa protein encoded by the Calc-1 gene along with calcitonin and katacalcin. The function and regulation of this protein are quite different from those of the other gene products. Blood concentrations of procalcitonin are increased in systemic inflammation, especially when this is caused by bacterial infection. Studies of its behaviour in patients with bacterial sepsis have led to the proposal that it may be a useful marker of systemic bacterial infection, with greater specificity and sensitivity than acute phase proteins such as C-reactive protein.

Procalcitonin and cellulitis: correlation of procalcitonin blood levels with measurements of severity and outcome in patients with limb cellulitis.

BACKGROUND: Procalcitonin levels may be raised in bacterial infections and have been used to guide antibiotic therapy. There is little data on procalcitonin and limb cellulitis. OBJECTIVES: Within a clinical trial of antibiotic therapy, we examined the correlation between clinical observations, blood tests and local measurements of skin damage, with serum procalcitonin levels. METHODS: The data is from a subset of the patients recruited into a clinical trial of antibiotic therapy for cellulitis (clindamycin for cellulitis, NCT01876628) whose procalcitonin levels were correlated with clinical

Case report: tigecycline-induced procalcitonin elevation.

This article reports a case of significant procalcitonin (PCT) elevation induced by tigecycline use. This finding provides clinicians with new insights into the potential adverse effects of tigecycline and highlights the importance of monitoring PCT levels during tigecycline therapy. The patient was a 76-year-old woman with a medical history of gastric cancer surgery and hypertension. She was admitted due to the fracture of internal fixation following corrective surgery for scoliosis. During hospitalization, she underwent removal of the scoliosis fixation rod and replacement with a titanium ro

Procalcitonin kinetics in Legionella pneumophila pneumonia.

Little is known about procalcitonin (PCT) levels in patients with community-acquired pneumonia (CAP) caused by Legionella pneumophila. The aim of the present study was to investigate this infection marker in patients admitted with L. pneumophila pneumonia in relation to conventional inflammatory parameters, severity of pneumonia upon admission and clinical outcome. Eighteen patients admitted with CAP caused by L. pneumophila serogroup 1 were retrospectively examined. PCT measurements were carried out during the first week of admission in addition to measurements of C-reactive protein (CRP), wh

Non-Infectious Causes for Elevated Procalcitonin.

Background and Objectives: Procalcitonin (PCT) is widely used to support the diagnosis of bacterial infection and sepsis, yet clinically relevant elevations also occur in multiple non-infectious conditions. This systematic review aimed to synthesize human evidence on non-infectious causes of elevated PCT and to summarize proposed pathophysiological mechanisms, with the goal of supporting context-based interpretation in clinical practice. Materials and Methods: A systematic search of PubMed/MEDLINE, Embase, Web of Science, and Scopus was performed from inception to 31 July 2025. Human studies p

Procalcitonin: Between evidence and critical issues.

Sepsis is a life-threatening organ dysfunction caused by a dysregulated response of the host to infection. It represents one of the major health care problems worldwide. Unfortunately, the diagnosis of sepsis is challenging for many reasons, including a lack of a sufficiently sensitive and specific diagnostic test. When procalcitonin (PCT) was discovered, it was thought that it could become the best test for identifying patients with sepsis. From the evidence sources in the available literature, it is now clear that the power of PCT in differentiating infectious from non-infectious forms of sy

Procalcitonin elevation suggests a septic source.

Procalcitonin is used as a marker for sepsis but there is little known about the correlation of the procalcitonin elevation with the causative organism in sepsis. All patients aged 18 to 80 years who were admitted to the surgery service from June 2010 to May 2012 and who had a procalcitonin drawn were evaluated. Culture data were reviewed to determine the causative organism. Infections analyzed included pneumonia, urinary tract infection (UTI), bloodstream infection, and Clostridium difficile. Other parameters assessed included reason for admission, body mass index, pressor use, antibiotic dur

[Procalcitonin measurement in adult clinical practice].

PURPOSE: The availability of a sensitive, specific and prognostic marker of bacterial infection is a major issue. More than ten years after the first publication assessing the usefulness of serum procalcitonin dosage (PCT), its exact place in the diagnostic process remained to be defined. The purpose of this review is to point out the indications of PCT measurement in clinical practice in the adult population. RELEVANT DATA: PCT is an early, sensitive but above all specific marker of severe bacterial/parasitic infections. After being studied mostly in critically ill patients, the indications

An Unrecognized Cause of Elevated Procalcitonin Level.

Diabetic Ketoacidosis (DKA) is a preventable yet serious complication of diabetes that is commonly associated with type 1 diabetes but can also occur in other forms of diabetes, including type 2. Infection is a primary cause of DKA and can lead to elevated levels of procalcitonin (PCT), which is a biomarker used to differentiate bacterial infections from non-infectious inflammation. However, some cases of DKA have shown increased PCT levels even in the absence of bacterial infection, and the underlying mechanism of this observation is not fully understood. To gain a better understanding of how

Procalcitonin: A Powerful Rescuer on Surgical Floors.

Necrotizing fasciitis (NF) is a rare but life-threatening medical and surgical emergency. It is characterized by necrosis of the soft tissue leaving the overlying tissue unaffected, which delays the diagnosis and treatment. Delay in recognition of the severity of necrotizing fasciitis might lead to serious morbidity and mortality. Diagnosis of NF relies on strong clinical judgment, predictable by severe pain, erythema, and a presence of air under the skin, but all of them are not always present. Management of NF is prompt surgical intervention and antimicrobial therapy. The effectiveness depen
