[Thoracoscopic Esophagectomy with Mediastinal Lymphadenectomy in Prone Position].

Recently, thoracoscopic esophagectomy has been widely accepted as a minimally invasive surgery and a possible standard procedure for esophageal cancer patients. Furthermore, it has an advantage for meticulous lymph node dissection by providing a magnification view and less blood loss. Mediastinal lymphadenectomy, especially upper mediastinal lymph node dissection, is a key point for esophageal cancer surgery because of its high frequency of metastasis and dissection efficacy. To avoid complications such as recurrent laryngeal nerve palsy, pneumoniae, chylothorax and unexpected injury in medias

Prone position for acute respiratory failure in adults.

BACKGROUND: Acute hypoxaemia de novo or on a background of chronic hypoxaemia is a common reason for admission to intensive care and for provision of mechanical ventilation. Various refinements of mechanical ventilation or adjuncts are employed to improve patient outcomes. Mortality from acute respiratory distress syndrome, one of the main contributors to the need for mechanical ventilation for hypoxaemia, remains approximately 40%. Ventilation in the prone position may improve lung mechanics and gas exchange and could improve outcomes. OBJECTIVES: The objectives of this review are (1) to asc

Assistant-based standardization of prone position thoracoscopic esophagectomy.

Thoracoscopic esophagectomy in the prone position (TEPP) might enable solo-surgery in cases requiring resection of the esophagus and the surrounding lymph nodes due to the associated advantages of good exposure of the surgical field and ergonomic considerations for the surgeon. However, no one approach can be for all patients requiring extensive lymphadenectomy. We recently developed an assistant-based procedure to standardize exposure of the surgical field. Patients were divided into 1 of 2 groups:a pre-standardization group (n=37) and a post-standardization group (n=28). The thoracoscopic op

[Prone position in the adult respiratory distress syndrome].

Acute respiratory distress syndrome (ARDS) has been a subject of research in the recent decades since it was defined in the seventies. ARDS is now known to be triggered by pulmonary and non-pulmonary insults which allow chemical agents to be released in the lungs. The resulting damage to the alveolar-capillary membrane causes an impaired ventilation and oxygenation. The aim of the present bibliographic review is to describe all the theories that have been proposed since 1974 to explain how the prone position enhances oxygenation in ARDS patients. Current thought amongst researchers includes ho

Prone position for the prevention of lung infection.

Pulmonary infection is frequent in brain injured patients. It has been identified as an independent predictor of unfavorable neurological outcome, calling for attempts of prevention. We recently evaluated intermittent prone positioning for the prevention of ventilator-associated pneumonia (VAP) in comatose brain injured patients, in a randomized study. 25 patients were included in the prone position (PP) group: they were positioned on prone four hours once daily until they could get up to sit in an armchair; 26 patients were included in the supine position (SP) group. The main characteristics

The anatomical positioning change of retroperitoneal organs in prone and lateral position: an assessment for single-prone position lateral lumbar surgery.

PURPOSE: There are reports that performing lateral lumbar interbody fusion (LLIF) in a prone, single position (single-prone LLIF) can be done safely in the prone position because the retroperitoneal organs reflect anteriorly with gravity. However, only a few study has investigated the safety of single-prone LLIF and retroperitoneal organ positioning in the prone position. We aimed to investigate the positioning of retroperitoneal organs in the prone position and evaluate the safety of single-prone LLIF surgery. METHODS: A total of 94 patients were retrospectively reviewed. The anatomical posi

Monitoring response to prone positioning.

PURPOSE OF REVIEW: The increasing use of prone position, in intubated patients with acute respiratory distress syndrome as well as in patients with acute hypoxemic respiratory failure receiving noninvasive respiratory support, mandates a better definition and monitoring of the response to the manoeuvre. This review will first discuss the definition of the response to prone positioning, which is still largely based on its effect on oxygenation. We will then address monitoring respiratory and hemodynamic responses to prone positioning in intubated patients. Finally, we will also discuss monitori

Treatment of ARDS With Prone Positioning.

Prone positioning was first proposed in the 1970s as a method to improve gas exchange in ARDS. Subsequent observations of dramatic improvement in oxygenation with simple patient rotation motivated the next several decades of research. This work elucidated the physiological mechanisms underlying changes in gas exchange and respiratory mechanics with prone ventilation. However, translating physiological improvements into a clinical benefit has proved challenging; several contemporary trials showed no major clinical benefits with prone positioning. By optimizing patient selection and treatment pr

Prone positioning: is it safe and effective?

Prone positioning has been used as a treatment option for patients with acute lung injury or acute respiratory distress syndrome (ARDS) since the early 1970s. Prone position and extended prone position ventilation have been shown to increase end-expiratory lung volume, alveolar recruitment, and oxygenation in patients with severe hypoxemic and acute respiratory failure. Prone positioning is not a benign procedure, and there are potential risks (complications) that can occur to both the patient and the health care worker. Notable complications that can arise include: unplanned extubation, lines

Prone-positioning therapy in ARDS.

The prone position has been used to improve oxygenation in patients with severe hypoxemia and acute respiratory failure since 1974. All studies with the prone position document an improvement in systemic oxygenation in 70% to 80% of patients with acute respiratory distress syndrome (ARDS), with maximal improvement seen in the most hypoxemic patients. This article reviews data regarding efficacy for use of the prone position in patients with ARDS. Also described is a simple, safe, quick, and inexpensive procedure used to prone patients with severe ARDS on a standard bed in the intensive care un

The Prone-Angled Transpsoas Approach: Utilizing Position, Approach, Navigation, and Neuromonitoring in Prone Lateral Surgery.

BACKGROUND AND OBJECTIVES: Lateral lumbar interbody fusion techniques have advanced toward minimally invasive, anatomy-preserving approaches that enhance sagittal alignment and reduce perioperative morbidity. The most common existing approaches-the prone transpsoas and oblique lumbar interbody fusion-offer distinct advantages with opposing limitations. This manuscript describes a novel prone angled transpsoas (PAT) technique that combines the ergonomic and sagittal benefits of prone positioning with a navigated oblique/angled approach and direct lateral working corridor. This technique aims to

Anticipated response to prone position assessed with lung ultrasound.

Acute respiratory distress syndrome (ARDS) presents a challenge for clinicians due to its high morbidity and mortality. Prone position (PP) has been used as a therapeutic strategy in ARDS patients, demonstrating benefits in respiratory mechanics and gas exchange. In SARSCoV-2-related ARDS (C-ARDS), 80% of patients require PP to alleviate refractory hypoxemia. Lung ultrasound (LUS) has proven useful for evaluating lung reaeration, guiding PEEP titration, and differentiating ARDS phenotypes. The LUS score is a validated tool that correlates well with CT and mechanical parameters. In this case, a

Supine/prone position fixation treatment in cervical cancer radiotherapy.

OBJECTIVE: This study aimed to determine the correlation between bladder volume changes and set-up accuracy in cervical cancer patients undergoing radiotherapy. METHODS: Forty patients who underwent intensity-modulated radiotherapy were divided into two groups based on their position during treatment: group A (supine) and group B (prone). Correlations between bladder volume changes and set-up accuracy were retrospectively analyzed using archived data and image files. RESULTS: The rate of bladder volume change in group A (-3.99% [-24.51-31.53]) was significantly higher (Z = -2.724; P = 0.006)

OBJECTIVE: Transperitoneal laparoscopic adrenalectomy (TLA) is considered the treatment of choice for small and benign adrenal tumors. In the recent years, posterior retroperitoneoscopic adrenalectomy (PRA) has gained popularity due to its advantages over TLA, presenting a shorter surgical time and a faster recovery without increasing complications. Conversely, a greater learning curve is considered because the use of a different and unusual surgical field. Thoracoscopy Approach in Prone Position for Esophagoplasty in Children.

Congenital esophageal stenosis (CES) is a very rare clinical condition found in 1 per 25,000 to 50,000 live births. There are three histological types of CES described: tracheobronchial remnants, fibromuscular stenosis (FMS), and membranous stenosis. The first-line treatment in most cases is the conservative treatment (dilatation with a Savary bougie or balloon), but in some CES types, dilatation may be ineffective or result in esophageal perforation with serious complications or lethal outcome. Resection of the stenotic segment and end-to-end esophageal anastomosis was formerly presented as t

Duration of prone position sessions: a prospective cohort study.

BACKGROUND: Prone position (PP) is highly recommended in moderate-to-severe ARDS. However, the optimal duration of PP sessions remains unclear. We searched to evaluate the time required to obtain the maximum physiological effect, and to search for parameters related to patient survival in PP. METHODS AND RESULTS: It was a prospective, monocentric, physiological study. We included in the study all prone-positioned patients in our ICU between June 2016 and January 2018. Pulmonary mechanics, data from volumetric capnography and arterial blood gas were recorded before prone positioning, 2 h after
