PURPOSE: We investigated the intestinal transport of verapamil, chlorpromazine, and propantheline, particularly their P-glycoprotein-mediated secretion.
METHODS: Permeation of rat intestinal segments in vitro was determined using diffusion cells.

A case of a persistent parotid fistula and sialocele following temporomandibular joint surgery is described. This rarely reported complication is postulated to result from pre-existing bucco-masseteric obstruction of parotid outflow causing raised intraductal pressure in the patient. Following unsuccessful conservative management, the patient was cured by a 5-day course of restricted oral intake and propantheline bromide.

In a study of 52 consecutive out-patients the effects of diazepam and dixyrazine were compared with each other and with placebo as premedication for elective gastroscopy. All patients also received topical lidocaine and intravenous administration of propantheline bromide immediately before the examination. No difference was found between diazepam and dixyrazine, but they were both significantly superior to placebo in relieving anxiety, retching, and eructations.

The application of drug ion-selective electrodes as sensors for the direct determination of the released drug in a continuous-flow analyzer for automated dissolution studies is described. Flow-through electrodes, selective to chlorpromazine, amitriptyline, propantheline, cimetidine, and ranitidine, have been constructed and used for the dissolution studies of 18 dosage forms using the rotating basket apparatus. There are numerous methods described in the literature for sialocele treatment. Surgical modalities usually involve an operation with possible facial nerve injury, with the risk of a general anesthetic, and with prolonged hospitalization to be considered. Among non-surgical modalities radiation and laissez-faire are of questionable efficacy. Propantheline bromide, which medically interrupts the parasympathetic control of salivary secretion, has proved a safe, effective means of rapid sialocele control.

A discussion of the various parasympathetic depressants is presented. Belladonna alkaloids include atropine, the prototype, and scopolamine. Synthetic drugs include quaternary ammonium compounds with antimuscarinic activity such as methantheline, propantheline, and other drugs such as isopropamide, pipenzolate methylbromide, and diphemanil methylsulfate. A miscellaneous class of drugs such as hemicholinium, valethamate, and oxybutynin chloride also possesses parasympathetic depressant activity. The pharmacologic properties and clinical usage of these drugs are discussed.

The interactions of the parasympatholytic drugs adiphenin, adiphenin H and propanthelin and the structurally related compounds, diphenhydramine, D-propoxyphene and methadone with bovine serum albumin (BSA) are studied by equilibrium dialysis and ultracentrifugation. The results show that BSA has a few binding sites (n less than 10) for the mentioned drugs. The association between the compounds under study and BSA are relatively weak. The binding constants are in the range of 10(3) l/Mol.

Oxybutynin has less anticholinergic activity than propantheline bromide. Therefore, oxybutynin is often preferentially prescribed for patients at risk of morbid complications of atropine-like adverse effects. We report a case of oxybutynin-induced reflux esophagitis. This probably resulted from the anticholinergic action of oxybutynin, which decreased lower esophageal sphincter tone. We outline a management approach for patients who experience this adverse effect due to oxybutynin or other commonly prescribed medications.
