Morphological changes of the corneal epithelium after application of topical anaesthetic ointments.

The effect of topical anaesthetic ointments (4% cocaine, 4% xylocaine, 0.5% proparacaine, 0.2% oxybuprocaine) on the corneal epithelium of rabbits was examined using a scanning electron microscope. Even after a single application, the more toxic effect of cocaine compared to the other topical anaesthetics was evident. Cocaine caused disruption of both the plasma membrane and the cytoplasm. After a single application, the other preparations caused a marked decrease in the microvilli and microplicae, disruption of the intercellular spaces and the prominence of the cell nucleus which under normal

Comparison of cycloplegia at 20- and 30-minutes following proxymetacaine and cyclopentolate instillation in white 12-13-year-olds.

CLINICAL RELEVANCE: Reducing the time between drop instillation and refraction reduces the time paediatric patients and young adults spend in practice, facilitating more eye examinations daily. BACKGROUND: The current procedure for paediatric cycloplegic refraction is to wait for at least 30-minutes post-instillation of a cycloplegic before measuring spherical equivalent refraction. This study compared cycloplegia at 20- and 30-minutes following 0.5% proxymetacaine and 1.0% cyclopentolate in 12-13-year-olds. METHODS: Participants were 99 white 12-13-year-olds. One drop of proxymetacaine hydr

Short-term effects of proparacaine on human corneal thickness.

.5% proparacaine HCl is commonly used for topical anaesthesia in ultrasound pachometry prior to refractive surgery. This drug is known to alter corneal epithelial adhesion. Does 0.5% proparacaine result in an alteration in corneal thickness due to changes in the corneal epithelium? Corneal thickness was measured by optical pachometry at 30 sec intervals for 15 min under 3 experimental conditions; 1) 2 drops of artificial tears, 2) 1 drop of 0.5% proparacaine, 3) 2 drops of 0.5% proparacaine. Baseline measurements were recorded before each trial. No significant differences were found between th

Proparacaine Overuse in Corneal Abrasions at the Emergency Department: A Case Series.

Corneal abrasions are among the most common ophthalmic injuries in the emergency department (ED) and primarily present as severe ocular pain. Topical anesthetics provide temporary analgesia, but overuse is associated with complications including further corneal injury, infection, and vision loss. This case series describes three patients who used a 15-mL bottle of 0.05% proparacaine hydrochloride ophthalmic solution after discharge from the ED and returned within three days with corneal injury and pain. Although the use of topical anesthetics is traditionally discouraged by ophthalmologists, p

Does topical phenylephrine, tropicamide, or proparacaine affect macular blood flow?

The acute effect of topical phenylephrine HCl 10%, tropicamide 1% and proparacaine HCl 0.5% on macular capillary blood flow was studied in six healthy human volunteers using the blue field simulation technique. This technique provides a method for quantifying the velocity of leukocytes flowing in one's own retinal macular capillaries. Patients adjusted the velocity of computer simulated leukocytes moving on a CRT screen to match that of their own entoptically perceived leukocytes before instillation of each of the tested preparations and for 35 minutes immediately thereafter A 5% NaCl solution

Postoperative Corneal Injuries: Incidence and Risk Factors.

BACKGROUND: Previous studies of postoperative corneal injury rates relied on provider-initiated incident reports, which may underestimate the true incidence. Postoperative administration of proparacaine eye drops is used almost exclusively to diagnose corneal injury; therefore, identifying instances of administration may provide a better estimate of corneal injuries. We compared proparacaine administration versus provider-initiated reports to determine rates of corneal injury. In addition, potential associations between clinical variables and injury were assessed with a matched case-control st

[Self-medication with local anaesthetics by glaucoma patients using teletonometry].

BACKGROUND: Self-tonometry, a supplementary measurement of the intraocular pressure in ophthalmology by glaucoma patients using an automatic tonometer, will become more and more important in the future. As long as the self-tonometry has to work in the contact modus with the ocular surface, home application of a topical anaesthetic by the glaucoma patient will be a requirement for a successful measurement. So far no severe problems within this controlled self-medication have been seen. Nevertheless, public health authorities believe patient health is put at high risk by the application of local

Eosinophil chemotaxis and anterior uveitis from topical dimaprit and nordimaprit.

Topical application of the H2-histamine receptor agonist, dimaprit (S-[4-N,N-dimethylaminopropyl]isothiourea), produced eosinophil chemotaxis into the anterior segment of rabbit eyes only when an H2-antagonist was co-administered. Nordimaprit (S-[4-N,N-dimethylaminoethyl]isothiourea), a structural homologue of dimaprit that lacked activity at histamine receptors, produced eosinophil chemotaxis whether or not an H2-antagonist was co-administered. Onset of eosinophil chemotaxis began after 2 or more days of treatment, and was accompanied by corneal edema, opacification, and ocular inflammation.

The effect of brimonidine and proparacaine on metabolic enzymes: Glucose-6-phosphate dehydrogenase, 6-phosphogluconate dehydrogenase, and glutathione reductase.

Oxidative stress is to upregulate the pentose phosphate pathway (PPP). The PPP consists of two functional branches, glucose-6-phosphate dehydrogenase (G6PD) and 6-phosphogluconaste dehydrogenase (6PGD). Glutathione reductase (GR) has a significant role in catalyzing an oxidized glutathione form into a reduced form. The purpose of this study is to investigate the effects of brimonidine and proparacaine on the activity of 6PGD, G6PD, and GR enzymes purified from human erythrocytes. Brimonidine displayed considerable inhibition profile against G6PD with IC50 value and KI constant of 29.93 ± 3.56
