Proton therapy for pediatric diencephalic tumors.

Diencephalic tumors tend to be low grade tumors located near several critical structures, including the optic nerves, optic chiasm, pituitary, hypothalamus, Circle of Willis, and hippocampi. In children, damage to these structures can impact physical and cognitive development over time. Thus, the goal of radiotherapy is to maximize long term survival while minimizing late effects, including endocrine disruption leading to precocious puberty, height loss, hypogonadotropic hypogonadism, and primary amenorrhea; visual disruption including blindness; and vascular damage resulting in cerebral vascu

Proton beam therapy in pediatric oncology.

Pediatric caregivers and parents are eager to know the role of proton beam therapy (PBT) in the treatment of children with brain and solid tumors and other diseases for which radiation therapy is indicated. Although the number of children treated with PBT for the most common pediatric tumors is relatively small and outcome data are clearly lacking, modeling radiation dose, volume, and outcomes based on photon benchmark data and clinical experience suggest an advantage for PBT and an opportunity to reduce or eliminate many of the early and late effects of radiation therapy. As the number of cen

Proton-beam therapy for olfactory neuroblastoma.

PURPOSE: To analyze the feasibility and efficacy of proton-beam therapy (PBT) for olfactory neuroblastoma (ONB) as a definitive treatment, by reviewing our preliminary experience. Olfactory neuroblastoma is a rare disease, and a standard treatment strategy has not been established. Radiation therapy for ONB is challenging because of the proximity of ONBs to critical organs. Proton-beam therapy can provide better dose distribution compared with X-ray irradiation because of its physical characteristics, and is deemed to be a feasible treatment modality. METHODS AND MATERIALS: A retrospective re

Feasibility of Applying Proton Minibeam Technology on the Proton Therapy Complex "Prometheus".

UNLABELLED: Spatial fractionation of proton beams is being considered as a new strategy to reduce the probability of side-effects in radiation therapy on the proton therapy complex (PTC) "Prometheus". By collimating the beams to submillimeter dimensions, the dose profile at the target surface consists of alternating high- and low-dose regions. This promotes sparing of normal tissues along the beam path through the activation of various biological mechanisms. The aim of the study was to implement the minibeam method on the PTC "Prometheus" and to experimentally assess the quality of the resulti

Proton Beam Therapy in Liver Malignancies.

PURPOSE OF REVIEW: Proton beam therapy (PBT) allows for improved sparing of surrounding normal tissues compared with X-ray-based radiation therapy. This is especially important in the setting of liver malignancies, where an increase in integral dose leads to a higher risk of radiation-induced liver disease (RILD) as well as close proximity to vital gastrointestinal (GI) organs. RECENT FINDINGS: We have data from multiple centers demonstrating that PBT can safely deliver high, ablative doses of radiation therapy conferring excellent local control with good tolerance of treatment. PBT is an eff

Proton beam therapy in pediatric oncology.

PURPOSE OF REVIEW: The advent of proton beam therapy (PBT) has initiated a paradigm shift in the field of pediatric radiation oncology, with increasing promise to alleviate both short-term and long-term toxicities. Given the dramatic rise in proton therapy centers in the United States, a discussion of the quality of evidence supporting its use in pediatric cancers is warranted. RECENT FINDINGS: Proton radiotherapy appears to decrease the incidence and severity of late effects with the strongest evidence in pediatric brain tumor cohorts that shows benefits in neurocognitive, hearing, and endoc

Proton therapy for maxillary sinus carcinoma.

OBJECTIVES: To compare the dose-volume data of three-dimensional conformal proton therapy (3DCPT) versus intensity-modulated radiotherapy (IMRT) for a paranasal sinus malignancy. METHODS: 3DCPT and IMRT plans were created for a T4N0 maxillary sinus carcinoma. RESULTS: The target volume dose distributions were comparable for 3DCPT and IMRT. The mean and integral doses for all normal tissues were lower for 3DCPT. The maximum doses for both plans to the ipsilateral optic nerve/retina/lens, temporal lobe, pituitary, and brain exceeded tolerance doses. The contralateral parotid, lacrimal gland, a

Proton pump inhibitor therapy and osteoporosis.

Osteoporotic fractures, particularly hip fractures, can have a devastating impact on the well-being of the elderly population. Recently, two population-based observational studies reported a highly important association between the use of potent acid suppressive therapy and an increased risk of hip fractures. The mechanisms underlying such an association are not clear. However, a careful review of the existing evidence seems to suggest that the main physiologic consequences of proton pump inhibitor therapy may each have a theoretical influence on bone metabolism. Specifically, inhibition of th

Conformal proton therapy for prostate carcinoma.

BACKGROUND: The role and optimum dose of radiation to eradicate prostate cancer continues to be evaluated. Protons offer an opportunity to increase the radiation dose to the prostate while minimizing treatment toxicity. METHODS: Six hundred forty-three patients with localized prostate cancer were treated with protons, with or without photons. Treatments were planned with a 3D planning system; patients received 74-75 CGE (Cobalt Gray Equivalent) at 1.8-2.0 CGE per fraction. Patients were evaluated for response to therapy and treatment-related toxicity. RESULTS: The overall clinical disease-fr

Neurovisual outcome following proton radiation therapy.

From February 1981 to January 1984, 20 patients with a tumor of the upper clivus received proton irradiation at the Harvard Cyclotron Laboratory. For 15 patients with known neurovisual status (including visual acuity, color vision, visual field, and fundus examinations) we obtained a cumulative dose-volume histogram (DVH) of the optic nerves (ON) and the optic chiasm. The prescribed tumor doses ranged from 66.6 to 74.4 Cobalt Gray Equivalent (CGE) with a daily fraction size of 1.8 to 2.1 CGE. CGE is used because modulated protons have an RBE of 1.1 compared to 60Co. The follow-up ranged from 3

Biologically Effective Dose-Optimized Multi-Intensity-Modulated Proton Therapy: A Biologically Comparable Alternative to Proton Arc Therapy.

PURPOSE: Proton spot-scanning arc therapy (ARC) is an emerging technique that can enhance high-dose conformity to targets compared with standard intensity-modulated proton therapy (IMPT). Although proton ARC is highly desirable, it is not yet clinically available. Multiple IMPT plans delivered over different fractions and optimized simultaneously provide a practical means to approximate ARC-plan quality. However, existing multiple IMPT approaches are not optimized to be biologically comparable to proton ARC because they neglect the fractionation effect during treatment planning. This work prop

Proton radiography interpretation with artificial intelligence for treatment deviation detection in proton therapy.

BACKGROUND AND PURPOSE: Patient setup errors, anatomical changes, and uncertainties in proton range estimation degrade dose conformity in proton therapy. Adaptive proton therapy (APT) mitigates these deviations by monitoring and adjusting treatment plans. Proton radiography (PR) offers direct proton range information, making it a promising method to detect such deviations. In this study, we developed and evaluated an artificial intelligence (AI) PR tool for automated interpretation and classification of treatment deviations. MATERIALS AND METHODS: Computed Tomography (CT) scans from 32 head-a

Dataset for predicting single-spot proton ranges in proton therapy of prostate cancer.

The number of radiotherapy patients treated with protons has increased from less than 60,000 in 2007 to more than 220,000 in 2019. However, the considerable uncertainty in the positioning of the Bragg peak deeper in the patient raised new challenges in the proton therapy of prostate cancer (PCPT). Here, we describe and share a dataset where 43 single-spot anterior beams with defined proton energies were delivered to a prostate phantom with an inserted endorectal balloon (ERB) filled either with water only or with a silicon-water mixture. The nuclear reactions between the protons and the silico

Impact of and interplay between proton arc therapy and range uncertainties in proton therapy for head-and-neck cancer.

Objective. Proton therapy reduces the integral dose to the patient compared to conventional photon treatments. However,in vivoproton range uncertainties remain a considerable hurdle. Range uncertainty reduction benefits depend on clinical practices. During intensity-modulated proton therapy (IMPT), the target is irradiated from only a few directions, but proton arc therapy (PAT), for which the target is irradiated from dozens of angles, may see clinical implementation by the time considerable range uncertainty reductions are achieved. It is therefore crucial to determine the impact of PAT on r

Comparison of in-field secondary cancer risk based on therapeutic proton dose between proton arc therapy and intensity-modulated proton therapy in lung cancer patients.

BACKGROUND: Proton arc therapy (PAT) is an emerging delivery modality that delivers proton beams through continuous gantry rotation combined with spot scanning. Compared with intensity-modulated proton therapy (IMPT), PAT improves target conformity and spares organs at risk. However, the dynamic multi-angle irradiation pattern may alter the low-dose region distribution: the low-dose bath expands while the integral dose decreases. The net effect on secondary cancer risk, especially in radiosensitive lungs, remains unclear. Quantitative risk assessment is essential for clinical translation of PA

Proton Therapy in the Management of Pancreatic Cancer.

Radiation therapy plays a central role in the treatment of pancreatic cancer. While generally shown to be feasible, proton irradiation, particularly when an ablative dose is planned, remains a challenge, especially due to tumor motion and the proximity to organs at risk, like the stomach, duodenum, and bowel. Clinically, standard doses of proton radiation treatment have not been shown to be statistically different from photon radiation treatment in terms of oncologic outcomes and toxicity rates as per non-randomized comparative studies. Fractionation schedules and concurrent chemotherapy combi
