Pseudallescheria boydii mycetoma in northern New England.

Mycetoma is a chronic subcutaneous fungal infection characterized by tumefaction, draining sinuses, and grains. It is most common in the tropics but occasionally occurs in the United States. We report a case of a mycetoma affecting the foot of a 38-year-old mentally retarded man from northern New England. The causal organism was identified as Pseudallescheria boydii, the most common cause of mycetoma in the United States. The patient showed a partial response to 8 months of ketoconazole therapy.

Arthritis without grains caused by Pseudallescheria boydii.

Severe infection of the knee joint by Pseudallescheria boydii in a 23-year-old female resulted in complete destruction of the cartilaginous surface. The possibility of entry of the fungus through mild abrasions of unperforated skin is discussed. The clinical appearance is compared with that of Scedosporium prolificans. Antimycotic miconazole-itraconazole treatment, applied during a period of 4 months, was successful; no side-effects were noted.

Recurrent Pseudallescheria boydii sinusitis in acute leukemia.

Most episodes of fungal sinusitis in immunocompromised patients are caused by Aspergillus species. To treat such infections, surgical debridement and anti-fungal therapy have been recommended; it is also clear however that an adequate neutrophil count is important in controlling such infections. We report a case in which fungal sinusitis was shown to be due to P. boydii and in which the infection recurred over a period of 2 years during episodes of neutropenia in spite of vigorous surgical and medical therapy.

Isolated sphenoid sinusitis due to Pseudallescheria boydii.

A case of sphenoid sinusitis due to Pseudallescheria boydii is described in a 52-year-old non-immunocompromised woman. Treatment should always involve surgical drainage, and antifungal chemotherapy may be of benefit if there is histological evidence of invasion of surrounding tissue. For P. boydii infection miconazole should be the agent of choice, rather than amphotericin B. For this reason it is important to obtain culture and histological examination of sinus contents if fungal infection is suspected.

Intraspecific diversity of species of the Pseudallescheria boydii complex.

In order to establish intraspecific diversity of Pseudallescheria boydii and Scedosporium apiospermum, and to develop tools for their identification, variability within P. boydii and related species was investigated at different levels of diversity. Sequences of the D1/D2 region of large subunit (LSU) and of the internal transcribed spacer (ITS) region of ribosomal DNA (rDNA) gene were analyzed for a set of 57 strains, as well as partial sequences of the elongation factor 1-alpha (EF 1-alpha). Incongruence among 3 locus lineages was detected by partition homogeneity test (PHT). The maximum par

Subcutaneous infection with Pseudallescheria boydii in an immunocompromised patient.

With the broad employment of immunosuppressive therapy, the incidence of Pseudallescheria boydii infections is rising. We report a first case of the localized subcutaneous P. boydii infection in a patient with microscopic polyangiitis. Favorable outcome related to the treatment with voriconazole adds to the growing body of evidence supporting the use of this particular agent in P. boydii infections.

Disseminated Pseudallescheria boydii infection in a nonimmunocompromised host.

We present a highly unusual case of pulmonary Pseudallescheria boydii infection in a nonimmunocompromised host with a cavitating mass lesion. The diagnosis was confirmed by open lung biopsy. The patient was treated at another institution with course of amphotericin B, considered an ineffective therapy for this infection, and presented to us with direct extension and invasion of the left atrial appendage and the pulmonary artery, followed by massive pulmonary embolization and hematogenous dissemination to the liver, spleen, kidney, pancreas, and brain.

Pseudallescheria boydii infection of the central nervous system.

Pseudallescheria boydii is a rare cause of central nervous system infection characteristically presenting as a neutrophilic meningitis or multiple brain abscesses. Factors predisposing to central nervous system infection with this fungus include immunosuppression and near drowning. The organism is infrequently cultured from fluid obtained by lumbar puncture, delaying clinical recognition and appropriate antifungal therapy. All untreated patients with P boydii infection of the central nervous system died. We describe a patient who developed a persistent neutrophilic meningitis with focal neurol

Pseudallescheria boydii (Petriellidium boydii) infection of the orbit.

A 10-year-old male developed a chronic orbital infection following penetrating trauma and retention of an orbital foreign body. Diagnosis of infection with Pseudallescheria boydii (Petriellidium boydii) was made by fungal smear and culture. Sensitivity tests indicated the organism was resistant to amphotericin B. Surgical debridement of the inferior orbit and removal of the foreign body was followed by a six-week course of intravenous miconazole. The patient has shown no signs of recurrence in a two-year follow-up period. To our knowledge, this is the first reported case of orbital infection w

TLR4 recognizes Pseudallescheria boydii conidia and purified rhamnomannans.

Pseudallescheria boydii (Scedosporium apiospermum) is a saprophytic fungus widespread in the environment, and has recently emerged as an agent of localized as well as disseminated infections, particularly mycetoma, in immunocompromised and immunocompetent hosts. We have previously shown that highly purified α-glucan from P. boydii activates macrophages through Toll-like receptor TLR2, however, the mechanism of P. boydii recognition by macrophage is largely unknown. In this work, we investigated the role of innate immune receptors in the recognition of P. boydii. Macrophages responded to P. boy

Fungal arthritis due to Pseudallescheria boydii (Scedosporium apiospermum).

Musculoskeletal infections by fungal agents are uncommon conditions. Pseudallescheria boydii is an ubiquitous saprophytic fungus frequently involved in maduromycosis but rarely in septic arthritis. We describe a case of Pseudallescheria boydii destructive suppurative arthritis and osteomyelitis of the knee in an adult Guadeloupean man. Osteoarticular infections due to Pseudallescheria boydii are reviewed.

Molecular variability of Pseudallescheria boydii, a neurotropic opportunist.

The sequences of the internal transcribed spacer (ITS) ribosomal DNA (rDNA) domain data obtained by restriction fragment length polymorphism analysis with 18S rDNA and fingerprinting (M13) for clinical and environmental strains of Pseudallescheria boydii (anamorph, Scedosporium apiospermum) were compared to those for related species of Pseudallescheria, Petriella, and Scedosporium. The infraspecific variability of P. boydii was considerable. There were five different lengths in the 18S rDNAs within P. boydii due to the occurrence of introns. In several cases, strains isolated from a single pon

Pseudallescheria boydii infection of the nasal cavity of a horse.

An 18-year-old 454-kg (1,000-lb) American Quarter Horse gelding was evaluated because of chronic intermittent malodorous right-sided nasal discharge. Endoscopy revealed a mycotic plaque in the nasal cavity adjacent to the nasomaxillary opening of the right caudal maxillary sinus. The nasomaxillary opening appeared to be larger than normal. Fungal culture of specimens of the mycotic plaque yielded Pseudallescheria boydii. The horse was treated with 2% miconazole intranasally, sodium iodide i.v., and potassium iodide p.o. Thirty and 60 days after treatment was initiated, the nasal cavity was fou

Pseudallescheria boydii infection in the acquired immunodeficiency syndrome.

Pulmonary and renal infection developed in a 44-year-old Hispanic man due to the fungus Pseudallescheria boydii. Pseudallescheria boydii has been associated with cutaneous infection known as mycetoma but occurs very infrequently in extracutaneous sites. To our knowledge, this is the second reported case of P boydii in a patient with the acquired immunodeficiency syndrome and the first case with pulmonary and/or renal involvement.

Eumycotic mycetoma caused by Pseudallescheria boydii in a dog.

Abdominal eumycotic mycetoma caused by Pseudallescheria boydii was diagnosed in a 3-year-old male Siberian Husky. The dog was examined because of weight loss and signs of depression. Initially, pyrexia was the only clinical finding. Antibiotic and corticosteroid treatment was ineffective. Two weeks later, the dog's appetite had decreased, it had vomited a few times, and the caudal portion of the abdomen was sensitive to palpation. Hematologic and serum biochemical abnormalities consisted of anemia, leukocytosis, hypoglycemia, hypoalbuminemia, hyperglobulinemia, and high alkaline phosphatase ac

Pseudallescheria boydii in a patient with acute lymphoblastic leukemia.

Pseudallescheria boydii, a common soil organism, is best known as the causative agent of mycetoma. Isolation of the organism from sites other than the extremities is relatively uncommon. However, the use of corticosteroids and cancer chemotherapy has been associated with an increasing frequency of localized superinfection as well as disseminated disease. Recently, an increased number of disseminated P boydii infections, primarily associated with immunosuppressed patients, has been reported in the literature. We diagnosed an additional case of P boydii in a 16-year-old boy who had acute lymphob
