[Anorexia nervosa and music therapy].

Supporting people suffering from anorexia nervosa remains a challenge. While traditional treatment focuses more on speech and its emergence for the patient and family, non-medicinal and non-verbal therapies are used more systematically, sometimes as a last resort. Yet, as a non-verbal mediated therapy, music therapy can find a legitimate place and complement all the psychiatric, psychotherapeutic, somatic, dietetic and nursing areas where speech already circulates.

The feasibility of eye movement desensitization and reprocessing (EMDR) for older adults with posttraumatic stress disorder (PTSD) and comorbid psychiatric and somatic disorders.

OBJECTIVE: The aim of this study is to investigate the feasibility of eye movement desensitization and reprocessing (EMDR) therapy in older adults with posttraumatic stress disorder (PTSD), and to explicitly include information about presence of the comorbid psychiatric and somatic disorders as well as a history of traumatic events at treatment start. METHOD: A nonrandomized feasibility study in a multicenter design was conducted with 25 older PTSD patients (60-84 years). Treatment consisted of weekly 1-hour EMDR sessions for PTSD during 3, 6, or maximum 9 months. PTSD diagnosis was assessed

Improvement of Somatic Delusions with Altered Regional Cerebral Blood Flow Following Electroconvulsive Therapy in a Patient with Schizoaffective Disorder.

BACKGROUND Somatic delusions are false and fixed beliefs about health and organ function, which are observed in various psychiatric disorders. Psychotropic drugs such as antipsychotics and antidepressants are effective for some patients, while the efficacy of electroconvulsive therapy (ECT) for pharmacotherapy-resistant cases has been reported. Previous reports suggest that somatic delusions in delusional disorder somatic type are associated with reduced regional cerebral blood flow (rCBF), but it remains unclear whether this association is also observed in other psychiatric disorders. We repo

'Often there is a good deal to be done, but socially rather than medically': the psychiatric social worker as social therapist, 1945-70.

Seeking to align psychiatric practice with general medicine following the inauguration of the National Health Service, psychiatric hospitals in post-war Britain deployed new treatments designed to induce somatic change, such as ECT, leucotomy and sedatives. Advocates of these treatments, often grouped together under the term 'physical therapies', expressed relief that the social problems encountered by patients could now be interpreted as symptomatic of underlying biological malfunction rather than as a cause of disorder that required treatment. Drawing on the British Journal of Psychiatric So

Psychiatric and somatic co-morbidities in patients with Parkinson's disease: A STROBE-compliant national multi-center, cross-sectional, observational study COSMOS in Slovakia.

OBJECTIVE: Co-morbidities in any disorder can complicate its diagnostic process, they require more complex clinical management and lead to worse health outcomes and increased healthcare costs. There are regional differences in the prevalence of specific co-morbidities in Parkinson's disease (PD), and data from middle Europe are lacking. The project COSMOS aimed to disclose the prevalence of co-morbidities among patients with PD in Slovakia. METHODS: In a national, multi-center, cross-sectional, observational study, neurologists gathered relevant demographical and clinical data aimed at all ps

Brain stimulation for the treatment of psychiatric disorders.

PURPOSE OF REVIEW: There has been a resurgence of interest in brain stimulation techniques as therapies for psychiatric disorders. Various names are used for this class of treatments: neuromodulation, somatic therapies, brain stimulation techniques. The methods in this class range from non-invasive (transcranial magnetic stimulation) to invasive brain surgery (deep brain stimulation). RECENT FINDINGS: Within the past year, the results of several large multicenter trials have been published, clearing the way for US Food and Drug Administration approval of vagus nerve stimulation for recurrent

Interdisciplinary assessment and follow-up of patients with erectile dysfunction--psychiatric aspects.

The objectives of this investigation were to assess the psychiatric comorbidity and patients' satisfaction with treatment in a sample of 73 men with erectile dysfunction (ED) who were consecutively admitted to an andrologic outpatient centre. After comprehensive interdisciplinary assessment, 49% of these patients were assigned an organic etiology, 33% had psychogenic impotence, and in 18% somatic and psychogenic factors seemed to be relevant. 63% of all patients received a psychiatric diagnosis (according to ICD-9). Depressive disorders were found in 25%. Organic psychosyndromes (nonpsychotic)

Analysis of drug-drug interactions in spontaneous adverse drug reaction reports from EudraVigilance focusing on psychiatric drugs and somatic medication.

BACKGROUND: Psychiatric diseases are often treated with several drugs. In addition, the risk of developing somatic comorbidities which may require drug therapy is higher in patients with than in patients without psychiatric diseases. Further on, the risk of drug-drug interactions (DDI) increases with the number of drugs taken. The aim of this study was to analyze whether already known DDI between psychiatric drugs and somatic medications still occur in everyday clinical practice. METHODS: We analyzed 9,276 spontaneous adverse drug reaction (ADR) reports from Germany contained in the European A

[Consultative psychiatry examinations of patients--review of twelve months retrospective follow up].

INTRODUCTION: Psychiatric svymptoms in hospitalized somatic patients are not rare in practice, and therefore consultative psychiatric examinations are an important part of the treatment of patients in non-psychiatric departments. The term consultative psychiatry refers to psychiatric evaluation, advising and treatment of numerous and various psychiatric disorders in physically ill patients in non-psychiatric departments. The aim of the research was to determine the wards at which psychiatric consultations were the most frequent as well as the prevalence of psychiatric disorders. MATERIAL AND

[New therapeutic approaches in anorexia nervosa].

New therapeutic approaches in anorexia nervosa. Anorexia nervosa is characterized by nutritional, somatic and psychiatric symptoms that require multidisplinary approaches. New therapeutic approaches have the increase the quality of the alliance between the patient and the therapist, and therefore facilitate the commitment process (such as with to motivational therapy); these therapies also aim at facilitating mental flexibility, increase central coherence, elaborate new strategies to resolve problems (such as with cognitive remediation and mindfulness based cognitive therapy), and reinforcemen

Generalized anxiety disorder. Epidemiology, impact of comorbidity, and natural history.

According to epidemiologic data collected over the last 5 years, GAD is a prevalent psychiatric disorder that is more chronic than most other psychiatric conditions. Excessive worry, which is the definitive diagnostic criterion, is accompanied by secondary psychic and somatic symptoms. GAD often coexists with other psychiatric conditions, which adds to the serious functional impairment caused when it occurs alone. However, even GAD results in substantial psychosocial impairment that is comparable to that caused by panic disorder, major depression, and other psychiatric disorders. Recognition o

Patients presenting with somatic complaints: epidemiology, psychiatric comorbidity and management.

Somatic symptoms are the leading cause of outpatient medical visits and also the predominant reason why patients with common mental disorders such as depression and anxiety initially present in primary care. At least 33% of somatic symptoms are medically unexplained, and these symptoms are chronic or recurrent in 20% to 25% of patients. Unexplained or multiple somatic symptoms are strongly associated with coexisting depressive and anxiety disorders. Other predictors of psychiatric co-morbidity include recent stress, lower self-rated health and higher somatic symptom severity, as well as high h

Current trends in the assessment and somatic treatment of resistant/refractory major depression: an overview.

A significant proportion of depressed patients eventually present with treatment-resistant/refractory major depression (TRD), a debilitating condition that imposes significant health, social, and economic burdens. Recently, a growing level of consensus has been reached on the general meaning of TRD, according to which, depression is considered resistant when at least two trials with antidepressants from different pharmacologic classes (adequate in terms of dose, duration, outcome, and compliance) failed to achieve clinical remission. Regarding the management of TRD, a two-step approach is sugg

Cardiovascular and metabolic comorbidities in patients with Alzheimer's disease and vascular dementia compared to a psychiatric control cohort.

BACKGROUND: Multimorbidity in dementia is associated with an increased risk of complications and a higher need for care. Having knowledge of cardiovascular and metabolic comorbidities is crucial when making decisions about diagnostic procedures and therapies. We compared the prevalence of comorbidities in hospitalized patients with Alzheimer's disease (AD), vascular dementia, and psychiatric diseases other than dementia. Additionally, we compared clinically relevant health-care indicators (length of hospital stay, rate of re-hospitalization) between these groups. METHODS: We used information

Intractable Belching as a Psychiatric Consequence: A Case Report.

BACKGROUND: Belching is a common physiological occurrence, but it becomes bothersome when it occurs excessively and disrupts one's quality of life. It is hypothesized that psychiatric diagnoses may be an etiology and exacerbation for intractable belching.  Case presentation: We present a case report of a female in her 70s with a psychiatric history of major depressive disorder (MDD) and generalized anxiety disorder (GAD) with symptoms of intractable belching. This patient was diagnosed with supragastric belching (SGB), which is likely behaviorally induced as a result of her psychiatric comorbi

[Sleep disorders and child and adolescent psychiatric illnesses].

As a symptom of many psychiatric disorders of childhood and adolescence, sleep disturbances often complicate the course and treatment of the underlying disorder. A somatic aetiology, e.g., as in Kleine-Levin syndrome or narcolepsy, may lead to diagnostic misinterpretations. It is not clear whether specific alterations of sleep architecture already exist in this age group and are thus trait markers for psychiatric disorders. Although it is well-known that sleep problems in adults, especially insomnia, are important in the later development of depressive syndromes, it is not clear whether persis

Somatization in dermatology.

Medically unexplained dermatologic symptoms, such as pruritus, numbness and burning are known as somatization. These cutaneous symptoms can be very difficult to treat because of an absence of an objective explanation and they may not fit neatly into any known dermatological or psychiatric condition. These disorders are more commonly encountered in primary care and in dermatology, rather than in psychiatry. Certain skin disorders, for example, pruritus, could be a manifestation of somatization and others may predispose to somatic symptoms, for example, atopic dermatitis and psoriasis. Although

Resonant breathing in hospitalised psychiatric patients with persistent somatic symptoms: a randomised controlled trial.

BACKGROUND: Persistent somatic symptoms are a common feature in many psychiatric disorders. Moderate-to-severe cases often necessitate treatment in specialised inpatient psychiatric settings. AIMS: This study evaluated the efficacy, safety and acceptability of resonant breathing (personalised slow breathing to maximise heart rate variability (HRV)) as adjunctive treatment for hospitalised patients with psychiatric disorders presenting with persistent somatic symptoms on admission, when patients typically present with high symptom severity. METHODS: This multicentre, randomised, placebo-contr
