From lobotomy to connectomics: a critical review of contemporary psychosurgery.

Contemporary psychosurgery is increasingly regarded less as an invasive surgical discipline and more as a circuit-based medicine. This narrative critical review traces the evolution from the extensive frontal lesions of the early twentieth century to contemporary techniques, in which focal ablation and reversible neuromodulation form part of a shared therapeutic armamentarium. Building on the classical history-from trephinations and lobotomies to stereotaxy-we outline a functional neuroanatomical framework that integrates thelimbic system, basal ganglia, prefrontal cortex, and monoaminergic sy

Against the Current: Wilder Penfield, the Frontal Lobes and Psychosurgery.

Psychosurgery refers to the surgical interruption of the white matter fibres joining the frontal cortex to the remainder of the cortical mantle and to the thalamus, in an attempt to mitigate the manifestations of psychosis. It reached its heyday following World War Two and was abandoned with the introduction of major tranquilisers such as chlorpromazine. Wilder Penfield, unlike most of his contemporaries, had a jaundiced view of psychosurgery. This paper addresses Penfield's early experience with experimental, penetrating brain trauma and with the surgical resection of frontal, epileptogenic l

A cohort study of psychosurgery cases from a defined population.

All cases from an urban population treated by psychosurgery over a 20 year period were followed up; 44 out of 47 were available for study, and 33 of these were interviewed. Outcome was measured on a five-point scale, and follow-up was from 1 to 20 years, with a mean of 11; almost all patients previously had had severe, disabling and intractable illnesses. Operations were non-stereotactic (36), stereotactic (6), with double procedures in one case: outcome was better in the non-stereotactic group. On a five-point scale of outcome, 25 of the 33 interviewed patients were placed in the two best cat

Forgotten lobotomies in Rome: new evidence of psychosurgery in postwar Italy.

The history of psychosurgery, perhaps the most controversial venture in clinical neuroscience's past, has long been written without Rome. While leucotomies and lobotomies performed have been documented in northern and southern Italian centres, the Italian capital - despite being the birthplace of electroconvulsive therapy (ECT) and home to two major state psychiatric institutions - has remained largely absent from historical accounts. Drawing on previously unexamined archival materials from Italy and the United States, and despite substantial gaps in the surviving documentation-including the i

Functional psychosurgery in psychiatry: indications and preoperative evaluation. A narrative review.

Functional neurosurgery has evolved from a marginal and controversial intervention into a promising, technically feasible option for carefully selected patients with treatment-resistant psychiatric disorders. Among available techniques, deep brain stimulation has gained increasing clinical acceptance for selected patients with severe obsessive-compulsive disorder and Tourette syndrome, while other psychiatric indications, including treatment-resistant depression, substance use disorders, schizophrenia, anorexia nervosa and pathological aggression, remain largely investigational or restricted t

Evolution in the Treatment of Psychiatric Disorders: From Psychosurgery to Psychopharmacology to Neuromodulation.

The treatment of psychiatric patients presents significant challenges to the clinical community, and a multidisciplinary approach to diagnosis and management is essential to facilitate optimal care. In particular, the neurosurgical treatment of psychiatric disorders, or "psychosurgery," has held fascination throughout human history as a potential method of influencing behavior and consciousness. Early evidence of such procedures can be traced to prehistory, and interest flourished in the nineteenth and early twentieth century with greater insight into cerebral functional and anatomic localizat

[Shock therapy and psychosurgery in the early German Democratic Republic (GDR)].

Patient files, textbooks and published articles of the time show that the wide range of psychiatric therapies of the 1950s and 1960s was also used in the early German Democratic Republic (GDR). The use of insulin coma therapy, cardiazol and electroconvulsive therapies and especially of leucotomy in the GDR must not only be seen in the context of the international development and debate concerning these therapies up to the introduction of psychopharmaceutic therapy but also, in a similar way as in the Federal Republic of Germany, in relation to the locally sometimes different availability of in

Behavioral Improvements following Lesion Resection for Pediatric Epilepsy: Pediatric Psychosurgery?

INTRODUCTION: Resection of brain lesions associated with refractory epilepsy to achieve seizure control is well accepted. However, concurrent behavioral effects of these lesions such as changes in mood, personality, and cognition and the effects of surgery on behavior have not been well characterized. We describe 5 such children with epileptogenic lesions and significant behavioral abnormalities which improved after surgery. CASE DESCRIPTIONS: Five children (ages 3-14 years) with major behavioral abnormalities and lesional epilepsy were identified and treated at our center. Behavioral problem

The advent of psychosurgery in Australia-with particular attention to its introduction into Sydney.

OBJECTIVE: The objective of this study was to describe the advent of prefrontal lobotomy in Sydney and, less comprehensively, its introduction into Australia. METHOD: Reference to journal articles, books, reports and archival data held at the Royal Prince Alfred Hospital and via internet searches, interviews and personal memory. RESULT: This paper describes the arrival of psychosurgery in Sydney in the mid-1940s, and less comprehensively, its arrival in other Australian cities. CONCLUSIONS: In New South Wales, from 1945 or 1946, prefrontal lobotomies were conducted in private clinics and in

From psychosurgery to neuromodulation: deep brain stimulation for intractable Tourette syndrome.

Tourette syndrome is a neuropsychiatric disorder characterized by motor and vocal tics. It is often associated with depression, obsessive-compulsive symptoms, self-injurious behaviour and attention deficit-hyperactivity disorder (ADHD). In intractable patients, neuromodulation using deep brain stimulation (DBS) has widely replaced psychosurgery. Three different key structures are defined for DBS, the medial portion of the thalamus, the globus pallidus internus and the anterior limb of the internal capsule/nucleus accumbens. This is a comprehensive overview on the effect of DBS on motor and non

The Network Systems Underlying Emotions: The Rational Foundation of Deep Brain Stimulation Psychosurgery.

Science and philosophy have tried to understand the origin of emotions for centuries. However, only in the last 150 years have we started to try to understand them in a neuroscientific scope. Emotions include physiological changes involving different systems, such as the endocrine or the musculoskeletal, but they also cause a conscious experience of those changes that are embedded in memory. In addition to the cortico-striato-thalamo-cortical circuit, which is the most important of the basal ganglia, the limbic system and prefrontal circuit are primarily involved in the process of emotion perc

Psychosurgery and deep brain stimulation as ultima ratio treatment for refractory depression.

For decades, the most severe, protracted and therapy-resistant forms of major depression have compelled clinicians and researchers to look for last resort treatment. Early psychosurgical procedures were hazardous and often associated with severe and persistent side effects including avolition, apathy and change of personality. With the introduction of psychopharmacological treatments in the 1950s, the frequency of ablative procedures declined rapidly. The past decade, however, has witnessed the resurgence of surgical strategies as a result of refined techniques and advances such as high freque

Neuroplasticity and the brain connectome: what can Jean Talairach's reflections bring to modern psychosurgery?

Contrary to common psychosurgical practice in the 1950s, Dr. Jean Talairach had the intuition, based on clinical experience, that the brain connectome and neuroplasticity had a role to play in psychosurgery. Due to the remarkable progress of pharmacology at that time and to the technical limits of neurosurgery, these concepts were not put into practice. Currently, these concepts are being confirmed by modern techniques such as neuroimaging and computational neurosciences, and could pave the way for therapeutic innovation in psychiatry. Psychosurgery commonly uses a localizationist approach, ba

Past and Present Role of Neurosurgical Interventions in the Management of Psychiatric Disorders: A Literature Review on the Evolution of Psychosurgery.

Despite advancements in psychiatric treatments, many patients with treatment-resistant disorders are turning to neurosurgical interventions. These include neuromodulation-based surgeries such as deep brain stimulation (DBS) and ablative surgeries such as cingulotomy, offering relief for severe conditions such as post-traumatic stress disorder (PTSD), depression, schizophrenia, obsessive-compulsive disorder (OCD), anxiety, and substance use disorder. While "psychosurgery" has sparked debate due to concerns about patient well-being, recent studies indicate promising symptom improvement rates acr

History of psychosurgery at Sainte-Anne Hospital, Paris, France, through translational interactions between psychiatrists and neurosurgeons.

Sainte-Anne Hospital is the largest psychiatric hospital in Paris. Its long and fascinating history began in the 18th century. In 1952, it was at Sainte-Anne Hospital that Jean Delay and Pierre Deniker used the first neuroleptic, chlorpromazine, to cure psychiatric patients, putting an end to the expansion of psychosurgery. The Department of Neuro-psychosurgery was created in 1941. The works of successive heads of the Neurosurgery Department at Sainte-Anne Hospital summarized the history of psychosurgery in France. Pierre Puech defined psychosurgery as the necessary cooperation between neurosu

The early argument for prefrontal leucotomy: the collision of frontal lobe theory and psychosurgery at the 1935 International Neurological Congress in London.

The pathophysiology of mental illness and its relationship to the frontal lobe were subjects of immense interest in the latter half of the 19th century. Numerous studies emerged during this time on cortical localization and frontal lobe theory, drawing upon various ideas from neurology and psychiatry. Reflecting the intense interest in this region of the brain, the 1935 International Neurological Congress in London hosted a special session on the frontal lobe. Among other presentations, Yale physiologists John Fulton and Carlyle Jacobsen presented a study on frontal lobectomy in primates, and
