Client laughter in psychodynamic psychotherapy: Not a laughing matter.

Источник пометил публикацию как отозванную

We studied 814 client laughter events nested within 330 sessions nested within 33 clients nested within 16 therapists at one community clinic in which doctoral student therapists provided psychodynamic psychotherapy to adult community clients. Each laughter event in Sessions 1 to 5 and 16 to 20 was rated for cheerfulness, politeness, reflectiveness, contemptuousness, and nervousness. Across all clients, there was an average of about one laughter even per session. The average laughter event lasted 3.5 seconds, and was characterized primarily by politeness and reflectiveness. Overall amount of c

Brief Psychodynamic Psychotherapy: A Review and Illustrative Case Vignette.

In this fictional case vignette, the authors illustrate the inclusive criteria and supportive-interruptive continuum techniques utilized in brief psychodynamic therapy (BPT), a therapeutic strategy designed to shorten psychotherapy from months to weeks in certain types of patients. The authors provide examples of a fictional dialogue between a young adult patient with generalized anxiety disorder (GAD) and ruminating thoughts and her therapist to demonstrate how the therapeutic techniques of BPT (interpretation, confrontation, clarification, encouragement to elaborate, empathic validation, adv

The role of cognitive biases in short-term psychodynamic psychotherapy.

UNLABELLED: The concept of biased thinking - or cognitive biases - is relevant to psychotherapy research and clinical conceptualization, beyond cognitive theories. The present naturalistic study aimed to examine the changes in biased thinking over the course of a short-term dynamic psychotherapy (STDP) and to discover potential links between these changes and symptomatic improvement. This study focuses on 32 self-referred patients consulting for Adjustment Disorder according to DSM-IV-TR. The therapists were experienced psychodynamically oriented psychiatrists and psychotherapists. Coding of c

Brief psychotherapy in late adolescence: a psychodynamic and developmental approach.

Four cases were discussed in which college students presented themselves in crisis for psychiatric treatment; each was dealing with a normal developmental crisis of late adolescence centering around separation-individuation issues. A type of brief psychotherapy was described which maintained the focus on the primary dynamic conflict and promoted ego development by interpreting the unconscious intrapsychic conflict. The incestuous, aggressive, and regressive themes were explored, and defenses were actively interpreted whenever they interfered with the awareness of autonomous strivings consisten

Evaluation of psychodynamic psychotherapy in a community mental health center.

This study describes an evaluation of the effectiveness of psychodynamic psychotherapy provided in an outpatient community mental health clinic. The study used a single group pretest-posttest design involving 78 clients. Clinical outcomes included overall psychosocial functioning and quality of life, level of subjective distress, interpersonal functioning and role functioning, measured by the Outcome Questionnaire (Lambert, Hansen, Umpress, Lunnen Okilshi, & Burlingame, 2000). Clients showed statistically significant improvement from pretest (first sessions) to completion of treatment in overa

Predictors of dropout in psychodynamic psychotherapy of borderline personality disorder.

This study aimed to identify patient factors that predict early dropout from psychodynamic psychotherapy for borderline personality disorder (BPD). Thirty-six BPD patients began an open-ended course of twice per week psychodynamic psychotherapy that was defined in a treatment manual and supervised. Dropout rates were 31% and 36% at 3 and 6 months of therapy, respectively. Survival analysis techniques demonstrated that age and hostility ratings predicted early dropout, with continuers more likely to be older and expressing lower levels of hostility than dropouts. Many variables hypothesized to

Psychoanalytic Contributions to Psychodynamic Psychiatry and Psychotherapy: Freud's Structural Hypothesis.

Psychoanalysis has long lost its historical influence on U.S. academic psychiatry. Psychoanalytic theory, however, provides us with a rich and remarkably comprehensive knowledge of human development, both normative and pathologic. This article describes a psychoanalytic concept that enriches our understanding of the mind and its disorders: Freud's structural hypothesis. This core concept provides a theoretical foundation for understanding the clinical features of both neurotic and personality disorders. It also informs a psychodynamic psychotherapy.

Childhood depression: a place for psychotherapy. An outcome study comparing individual psychodynamic psychotherapy and family therapy.

BACKGROUND: Although considered clinically effective, there is little systematic research confirming the use of Individual Psychodynamic Psychotherapy or Family Therapy as treatments for depression in children and young adolescents. AIMS: A clinical trial assessed the effectiveness of these two forms of psychotherapy in treating moderate and severe depression in this age group. METHODS: A randomised control trial was conducted with 72 patients aged 9-15 years allocated to one of two treatment groups. RESULTS: Significant reductions in disorder rates were seen for both Individual Therapy and

Therapeutic Relationship: A Factor for Substantial Change in Psychodynamic Psychotherapies and Psychoanalysis.

Significant change in psychodynamic psychotherapy resides in the uncovering of unconscious conflicts as they manifest themselves in the lives of patients. My intent in this article is to illustrate the special power of the therapeutic relationship as a separate element beyond interpretation as a vehicle for a change. This is illustrated by three patients who revealed special feelings of affection for me in the context of our work together and the progress that they had made. The substrate of these feelings is a product of a consistent acceptance and valuation of the patient in all that they re

Therapeutic alliance trajectories and associations with outcome in psychodynamic child psychotherapy.

OBJECTIVE: This study investigated therapeutic alliance (TA) trajectories, their demographic and symptomatic predictors, and associations with outcome in psychodynamic child psychotherapy. METHOD: The sample included 89 Turkish children (Mage = 6.87, SD = 2.11, 46% girls) with internalizing (37.11%), externalizing (21.14%), and comorbid (38.20%) problems; 12% of the children were in the nonclinical range. Independent raters coded 328 sessions from different phases of treatment using the Therapy Process Observational Coding System-Alliance Scale. Outcome measures were collected at intake and t

Varieties of Castration Experience: Relevance to Contemporary Psychoanalysis and Psychodynamic Psychotherapy.

Although Freud considered castration to be one of the two major anxieties of human life, the castration complex has been relatively neglected in contemporary psychoanalytic writing and is insufficiently discussed in presentations of clinical cases. This article discusses the relevance of the concept to contemporary psychoanalysis and psychodynamic psychotherapy, in particular the important contributing role of castration conflicts in the pathogenesis of a wide range of clinical symptoms. The author begins by briefly reviewing some classical and contemporary psychoanalytic ideas about castratio

The empirical status of psychodynamic psychotherapy - an update: Bambi's alive and kicking.

BACKGROUND: The Task Force on Promotion and Dissemination of Psychological Procedures proposed rigorous criteria to define empirically supported psychotherapies. According to these criteria, 2 randomized controlled trials (RCTs) showing efficacy are required for a treatment to be designated as 'efficacious' and 1 RCT for a designation as 'possibly efficacious'. Applying these criteria modified by Chambless and Hollon, this article presents an update on the evidence for psychodynamic therapy (PDT) in specific mental disorders. METHODS: A systematic search was performed using the criteria by Ch

Open trial of psychodynamic psychotherapy for panic disorder: a pilot study.

OBJECTIVE: This report contains preliminary data from an open trial of brief psychodynamic psychotherapy for panic disorder. METHOD: Fourteen patients with primary DSM-IV panic disorder completed a 24-session, twice-weekly course of psychodynamic psychotherapy. Other psychiatric treatment was not permitted throughout the 12-week treatment period and the 6-month follow-up. Symptoms were assessed at baseline, treatment termination, and 6-month posttermination follow-up (40 weeks). RESULTS: Statistically significant, clinically meaningful improvements appeared in panic, depression, anxiety, and

Transference interventions in psychodynamic psychotherapy for adolescent depression: a qualitative study.

INTRODUCTION: Psychodynamic therapy is based on the understanding that past experiences, relationships, and unconscious conflicts can influence a person's current behavior and mental state. Transference is a central construct in psychodynamic psychotherapy, which refers to the emotions and attitudes that the patient attributes to the therapist. These emotions shape the patient's perception of both the therapist and the therapeutic process. Emphasizing and exploring how transferential phenomena manifest within the therapeutic relationship is assumed to have a beneficial therapeutic effect. The

Videoconferencing psychotherapy from a psychodynamic point of view. A qualitative analysis.

There is a growing interest in delivering videoconferencing psychotherapy (VCP) due to the enormous impact of the COVID-19 pandemic on our lives since the beginning of severe restrictions worldwide in March 2020. Scientific literature has provided interesting results about the transition to remote sessions and its implications, considering different psychotherapy orientations. Less is known about whether and how VCP affects psychodynamic psychotherapeutic approaches and reports on remote work with severe and complex mental health problems such as severe personality disorders are still scarce.

[Effectiveness of psychodynamic psychotherapy in children and juveniles - results of a pilot study].

QUESTION: The effectiveness of psychodynamic psychotherapy for children and adolescents has yet to be studied sufficiently in Germany. Because psychodynamic psychotherapy aims to reduce symptoms in both the short and long term - besides achieving other long-term goals - in this paper we present results focusing on whether symptoms and problem behavior can be improved within 25 h of outpatient psychodynamic psychotherapy taking place in a practice. Moreover, it addresses whether a positive change of life quality can be achieved in the same timeframe. METHOD: Thirty children and juveniles with

Client experience of psychodynamic psychotherapy for bulimia nervosa: an interview study.

We examined 14 bulimic clients' experiences of individual psychodynamic psychotherapy through semistructured interviews, which were analyzed using qualitative methods. The results showed that the psychodynamic approach was a challenge to most of the clients. Yet, most clients profited from therapy both symptomatically and with regard to interpersonal relations and affect regulation. There were, however, marked differences in the clients' experiences. One subgroup rather quickly felt that the therapy met their needs, another initially felt challenged by the approach and the therapeutic attitude

[Efficacy of psychodynamic short-term psychotherapy for children and adolescents with depression].

Depression is highly prevalent in children and adolescents. Psychodynamic therapies are only insufficiently evaluated in this field although many children and adolescents suffering from depression are treated using this approach. Therefore, the aim of our study was to evaluate the efficacy of psychodynamic short-term psychotherapy (PSTP) for the treatment of depression in children and adolescents. In a waiting-list controlled study, 20 children and adolescents fulfilling diagnosis of major depression or dysthymia were included. The treatment group received 25 sessions of psychodynamic psychoth

A pilot open trial of brief psychodynamic psychotherapy for panic disorder.

This is a complete report of an open trial of manualized psychodynamic psychotherapy for treatment of panic disorder, Panic-Focused Psychodynamic Psychotherapy (PFPP). Twenty-one patients with PD were entered into a trial of twice-weekly, 24-session treatment. Sixteen of 21 experienced remission of panic and agoraphobia. Treatment completers with depression also experienced remission of depression. Improvements in symptoms and in quality of life were substantial and consistent across all measured areas. Symptomatic gains were maintained over 6 months. This report was prepared specifically to d

Suitability for brief dynamic psychotherapy: psychodynamic variables as predictors of outcome.

The success rate in predicting symptom change 2 years after brief dynamic psychotherapy increased 20% by adding quality of interpersonal relations from a psychodynamic evaluation interview to DSM-III diagnoses, Axis V and Axis II. The success rate in predicting dynamic change 4 years after therapy increased nearly 40% by adding the same variable. Selection criteria such as circumscribed focus, motivation and involvement in the evaluation interview were shown to be redundant as predictors.
