BACKGROUND: Temporomandibular disorders (TMDs) are among the most common causes of orofacial pain. Hyperactivity of the lateral pterygoid muscle (LPM) is associated with the pathological mechanisms associated with TMD. Botulinum toxin-A (BTX) injections into the LPM can be used as a treatment for TMD. However, a lack of clinical standardisation for this procedure can lead to adverse outcomes, especially when using anatomical landmark-based approaches.
MATERIALS AND METHODS: To better understand the precision of extraoral landmark- based LPM BTX injections, a maxillofacial surgeon injected 1.5

PURPOSE: Lateral pterygoid muscle (LPM) plays an important role in jaw movement and has been implicated in Temporomandibular disorders (TMDs). Migraine has been described as a common symptom in patients with TMDs and may be related to muscle hyperactivity. This study aimed to compare LPM volume in individuals with and without migraine, using segmentation of the LPM in magnetic resonance (MR) imaging of the TMJ.
MATERIALS AND METHODS: Twenty patients with migraine and 20 volunteers without migraine underwent a clinical examination of the TMJ, according to the Research Diagnostic Criteria for T

INTRODUCTION: Myositis ossificans (MO) is characterized as heterotopic bone formation within muscle. MO rarely occurs in the head and neck region. Excision of the heterotopic bone is the standard treatment. This report summarizes a case of a 12-year old female with MO involving the lateral pterygoid muscle. The heterotopic bone was excised using an intraoral incision. Despite intensive physical therapy, the operation failed as evidenced by new bone formation in the area within three weeks of the operation.
PRESENTATION OF CASE: A twelve years old female patient presenting with mouth opening o

The postoperative skeletal stability following surgical advancement of the mandible can be influenced by several factors. The effect of the medial pterygoid muscle and the stylomandibular ligament on the stability of results following surgical advancement has possibly been underestimated. In this retrospective study, the long-term postoperative skeletal stability following surgical advancement of the mandible in two groups of patients was studied and compared. In one group the medial pterygoid muscle and the stylomandibular ligament were stripped from the medial side of the angle of the mandib

The lateral pterygoid (LP) has been implicated in temporomandibular joint (TMJ) pathology. Few studies have examined muscle architecture of the superior (SLP) and inferior (ILP) heads of LP; moreover, the pattern of intramuscular innervation is poorly defined. The purpose of this study was to determine patterns of intramuscular innervation of LP using 3D modeling. The superior and lateral aspects of LP were exposed in 10 embalmed cadaveric specimens. Nerves entering the muscle, all branches of the mandibular nerve (V(3) ), were followed intramuscularly in short segments and sequentially digiti

OBJECTIVE: The theory of internal derangement of the temporomandibular joint involves the anterior displacement of the meniscus, which is thought to be the result of hyperactivity of the lateral pterygoid muscle. There is, however, a great deal of confusion with respect to the attachment of this muscle to the meniscus. This anatomic study reexamines the meniscal attachment of the lateral pterygoid muscle, in particular, the upper head of this muscle.
STUDY DESIGN: The anatomic relationship between the lateral pterygoid muscle and the meniscus was examined in the postmortem specimens of 25 mal

Numerous experimental studies and clinical anecdotal evidence demonstrate a close interaction between masticatory muscle function and skeletal form. The purpose of this study was to determine whether a relationship existed between histochemical characteristics of human masseter and medial pterygoid muscles and cephalometric measurements of the 11 subjects from whom specimens were collected. Muscular specimens were obtained at the time of corrective surgery through incisions made to expose the mandibular ramus. Histochemical analysis was used to determine the size and distribution of fiber type

BACKGROUND: Temporomandibular disorders (TMDs) are a frequent cause of orofacial pain, causing functional disability and a negative impact on quality of life. Botulinum toxin (BTX-A) injection in lateral pterygoid muscle (LPM) is one of the treatment modalities proposed, but the blind puncture guided by EMG carries a risk of vascular puncture or diffusion of the toxin to nearby muscles. We describe an ultrasound-guided approach and evaluate the spread of the injection in a fresh human cadaver.
METHODS: A fresh human cadaver was injected. An out-of-plane approach was performed using a convex p

During a routine dissection of the infratemporal fossa and lateral face, a branch of the left lingual nerve was observed entering the medial pterygoid muscle. Normally, the nerve to the medial pterygoid is a direct branch from the mandibular nerve, with no communications with the lingual nerve. There are many reports involving variations of the mandibular nerve; however, few reports describe lingual nerve variations involving the medial pterygoid muscle. Reconstructive surgeries for cosmesis and trauma, tumor excision, and impacted third molar removal may all damage the lingual nerve and might

OBJECTIVE: To describe an unusual case of bilateral masseter and pterygoid muscle hypertrophy.
CLINICAL PRESENTATION AND INTERVENTION: A 23-year-old male patient presented with a bilateral, painless swelling of 1 year duration at the parotid areas without improvement after using antibiotics/systemic corticosteroids/non steroidal anti-inflammatory agents. His medical history was not significant. The initial differential diagnosis included salivary gland/jaw bone/masseter pathology, but the MRI revealed only an increase in the size of the masseter and pterygoid muscles. The patient was informed

In this report, we present a case of myositis ossificans traumatica (MOT) of the medial pterygoid muscle that had developed after mandibular block anesthesia administered for endodontic treatment of the lower right second molar, demonstrating typical features of this condition. MOT should be considered as a differential diagnosis when there is severe limitation of jaw opening and an associated trauma. Panoramic radiographs and axial and coronal computed tomography (CT) scans can effectively delineate the calcified mass. Other imaging studies that may be helpful include magnetic resonance imagi

Many patients with temporomandibular joint dysfunction and/or myofascial pain syndrome have numerous symptoms. In cases of multiple complaints, an attempt should be made to resolve symptoms on a step-by-step basis. In this case report, a patient was treated for only one of her numerous complaints, that is, a burning sensation on her palate when she touched the right tuberosity area with her tongue or finger.

BACKGROUND: The lateral pterygoid muscle (LPM) plays a pivotal role in maintaining the stability of the temporomandibular joint (TMJ), especially the articular disc. A considerable number of patients afflicted with TMJ dysfunction may also experience LPM spasms, which can be challenging to discern through direct clinical examination. The objective of this study was to evaluate LPM length by measuring the distance between the origin and insertion points of the LPM with CBCT slices, a novel approach in the literature.
MATERIALS AND METHODS: The study encompassed a total of 498 TMJs and LPMs fro

BACKGROUND: Botulinum toxin has proven effective in treating persistent myogenous temporomandibular disorders (M-TMDs) unresponsive to conservative therapies. While the usual injection sites are the masseter and temporalis muscles, the deeper lateral pterygoid muscle (LPM) is often overlooked due to its difficulty of access and the risk of local complications. This study aims to evaluate the effectiveness of botulinum toxin-A injections (BTX-A) in the LPM with MR-guided navigation of patients with persistent M-TMDs.
METHODS: This retrospective study enrolled 34 patients suffering from M-TMDs

Complications related to lateral pterygoid muscle (LPM) botulinum toxin A (BtA) injection for recurrent temporomandibular joint dislocation are uncommon. No cases of velopharyngeal dysfunction (VPD) following LPM BtA injection have been reported to date. This report details the perioperative and follow-up findings for a patient developing VPD following LPM BtA injection.
