Does informal care reduce public care expenditure on elderly care? Estimates based on Finland's Age Study.

BACKGROUND: To formulate sustainable long-term care policies, it is critical first to understand the relationship between informal care and formal care expenditure. The aim of this paper is to examine to what extent informal care reduces public expenditure on elderly care. METHODS: Data from a geriatric rehabilitation program conducted in Finland (Age Study, n = 732) were used to estimate the annual public care expenditure on elderly care. We first constructed hierarchical multilevel regression models to determine the factors associated with elderly care expenditure. Second, we calculated the

Ten-Year Public Expenditure Savings Associated with TNF Inhibitor Biosimilars under Australia's Pharmaceutical Benefits Scheme.

Biologic therapies are a major and growing driver of pharmaceutical expenditure globally, with tumor necrosis factor (TNF) inhibitors among the most widely used and costly biologics. In Australia, these therapies are funded through the Pharmaceutical Benefits Scheme (PBS), where biosimilars offer lower cost alternatives; however, the long-term public expenditure impact of TNF inhibitor biosimilars under regulated pricing systems has not been comprehensively quantified. We conducted a retrospective, population-level expenditure analysis using publicly available PBS Item Reports data from 2015 t

Trends in antidiabetic drug use and expenditure in public hospitals in Northwest China, 2012-21: a case study of Gansu Province.

BACKGROUND: Since the twenty-first century, the prevalence of diabetes has risen globally year by year. In Gansu Province, an economically underdeveloped province in northwest China, the cost of drugs for diabetes patients accounted for one-third of their total drug costs. To fundamentally reduce national drug expenditures and the burden of medication on the population, the relevant departments of government have continued to reform and improve drug policies. This study aimed to analyse long-term trends in antidiabetic drug use and expenditure in Gansu Province from 2012 to 2021 and to explore

The cost of delay: foregone public expenditure savings from delayed adalimumab biosimilar listing in Australia's pharmaceutical benefits scheme.

BACKGROUND: Adalimumab is among the highest-expenditure biologics under Australia's Pharmaceutical Benefits Scheme (PBS). Although the Pharmaceutical Benefits Advisory Committee recommended listing adalimumab biosimilars in 2018, reimbursement did not begin until April 2021. The public savings forgone through this delay have not been quantified. This study aimed to estimate the PBS savings achievable had biosimilars been listed earlier. RESEARCH DESIGN AND METHODS: We conducted a retrospective policy evaluation using nationwide PBS dispensing data across all nine subsidized indications from J

Out-of-pocket payment and catastrophic health expenditure of tuberculosis patients in accessing care at public-private mix clinics in Myanmar, 2022.

BACKGROUND: The financial burden of tuberculosis (TB) can hinder patients and their families, creating obstacles throughout the care cascade, despite TB prevention and control being provided free of charge. In Myanmar, patients can visit private providers operating under public-private mix (PPM) schemes, where TB services (diagnosis and treatment) are typically offered at no cost. The study focused on quantifying the financial burden faced by TB patients seeking care from Myanmar's PPM providers. METHODS: This cross-sectional telephone survey included 695 adults seeking TB treatment [drug-sus

Italian Public Health Expenditure for Vitamin D is still high: New Outlook to Saving From a Consumption Analysis in the Liguria Region.

PURPOSE: Italian expenditure for vitamin D greatly increased in the last few years, reaching €314 million ($376.8 million) in 2019. In Italy, the main cause of the increase in public spending for vitamin D is the marketing of high-cost medicines. At national and regional levels, some interventions have been performed to reduce expenditure, but spending has continued to increase. The aim of this work is to propose a new saving strategy determined by an analysis of a significant sample of the market. METHODS: Data on the use of vitamin D formulations, including data for the different active sub

Understanding the financial burden of incomplete abortion: An analysis of the out-of-pocket expenditure on postabortion care in eight public-sector health care facilities in Dakar, Senegal.

Timely access to treatment is essential for women when they experience abortion complications. Out-of-pocket (OOP) expenditure is a known barrier to health care access. In 2018, we assessed the financial burden of accessing postabortion care (PAC) borne by women in Dakar, Senegal, where studies estimate that half of poor women with complications obtain PAC. We interviewed 729 women following discharge from PAC. Women reported expenditures on transportation, admission, treatment, family planning, hospitalisation, complementary tests, prescriptions, other medicines and materials. We compare wome

The Spatiotemporal Threshold Effects of Health Expenditure and Health Developmental Assistances on Health-Related Outcome in Sub-Saharan Africa Post Covid19: The Case of the Public-Private Partnerships Donors.

The COVID-19 pandemic has not only revealed pre-existing vulnerabilities in global health systems and imposed heavy costs on countries worldwide, but also exacerbated existing disparities. These institutional and financing limitations were part of the development strategy, which did not adequately accommodate the health risks accompanying the outbreak of the virus. This disruption has ongoing implications for access to healthcare and treatment, particularly in economically disadvantaged and marginalised communities. This study documents the spatial spillover and threshold effects of health dev

Expanding budget space to improve health outcomes in low- and middle-income countries: what role for tax expenditures?

Recent evidence indicates that budget space for health can be improved through increasing government revenues, expanding the budget's health share, and improving expenditure efficiency through enhancing public financial management (PFM), with government revenue mobilization being the most substantial. Government revenue mobilization can be achieved by broadening the tax base, a key component of which is the rationalization of tax expenditures. Tax expenditures are preferential tax treatments, relative to a baseline tax regime, intended to achieve specific objectives by providing financial supp

Analysis of the Impact of Disaggregates Government Expenditure on Economic Growth in Iraq.

BACKGROUND: Iraq heavily depends on oil revenues, which expose it to global oil price fluctuations as well as structural crises. Therefore, in the 2004-2023 crisis period, Iraq experienced economic shocks that followed the change in global prices and internal disturbances that undermined the stability and an enabling environment for growth. The government spending in the last five years has increased; however, its contribution to economic growth has been limited due to poor targeting on productive investment-driven expenditure and high reliance on current spending in the budget.
