Pulsed-wave tissue Doppler echocardiography for the analysis of fetal cardiac arrhythmias.

OBJECTIVES: Rhythm analysis of the fetal heart is hampered by the inability to routinely obtain electrocardiographic recordings of the fetus. Doppler studies of fetal cardiac tissue movements, assessing cardiac movements both qualitatively and quantitatively, have recently been described. We used a conventional high-resolution ultrasound system to obtain rhythm data from pulsed-wave tissue Doppler signals of the fetal heart in normal cardiac rhythm and in a variety of fetal cardiac arrhythmias. METHODS: Fifty-five fetuses with normal (sinus) rhythm, 45 fetuses with rhythm disturbances and two

Pulse wave analysis as a tool for the evaluation of resuscitation therapy in septic shock.

Objective. Pulse wave analysis (PWA) can provide insights into cardiovascular biomechanical properties. The use of PWA in critically ill patients, such as septic shock patients, is still limited, but it can provide complementary information on the cardiovascular effects of treatment when compared to standard indices outlined in international guidelines. Previous works have highlighted how sepsis induces severe cardiovascular derangement with altered arterial blood pressure waveform morphology and how resuscitation according to standard haemodynamic targets is not able to restore the physiologi

Estimated pulse wave velocity predicts outcomes after acute ischemic stroke: a multinational cohort analysis.

BACKGROUND: Estimated pulse wave velocity (ePWV), an easy to calculate proxy of carotid to femoral PWV (cfPWV), can be derived from 24-hour blood pressure monitoring (24h-BPM). Its role in acute ischemic stroke (AIS) has not previously been investigated in large multinational datasets. METHODS: A pooled individual patient data analysis from 13 cohorts across seven countries was conducted, including 2933 AIS patients who underwent 24h-BPM during the hyperacute to subacute phase. ePWV was calculated using a validated equation. Associations with clinical characteristics, stroke severity, etiolog

Association between pulse wave velocity and cerebral microbleeds: a systematic review and meta-analysis.

Cerebral microbleeds are associated with events that are among the highest mortality and disability events combined worldwide, as well as with hypertensive vasculopathy. The aim of the present study was to investigate the relationship between a marker of hypertensive vasculopathy, arterial stiffness assessed by pulse wave velocity, and cerebral microbleeds. A systematic review and meta-analysis was performed using PubMed, Scopus, and Web of Science, according to the Meta-analysis of Observational Studies in Epidemiology (MOOSE) and Cochrane Collaboration Handbook statements. Penile flow index utilizing a Doppler pulse wave analysis to identify penile vascular insufficiency.

A Doppler ultrasound probe was used in 42 men to evaluate penile circulation. Twenty-six patients had confirmation of vascular anatomy by pelvic arteriography. The average penile acceleration (peak velocity over pulse rise time) relative to the radial artery acceleration was expressed as the penile flow index. Eleven potent men had a penile flow index of 3.4 plus or minus 0.4 standard error, while 13 impotent men with vascular disease on arteriography had a penile flow index of 21.7 plus or minus 5 standard error (p less than 0.01). Patients with intermediate penile flow index values had varia

The role of pulse wave analysis indexes for critically ill patients: a narrative review.

Objective.Arterial pulse wave analysis (PWA) is now established as a powerful tool to investigate the cardiovascular system, and several clinical studies have shown how PWA can provide valuable prognostic information over and beyond traditional cardiovascular risk factors. Typically these techniques are applied to chronic conditions, such as hypertension or aging, to monitor the slow structural changes of the vascular system which lead to important alterations of the arterial PW. However, their application to acute critical illness is not currently widespread, probably because of the high hemo

Deep Learning-Based Assessment of Sleepiness from Fingertip Pulse Wave Analysis During Objective Daytime Testing.

PURPOSE: Objective evaluation of daytime sleepiness is a key factor in the management of sleep disorders. However, objective testing is limited due to the labor-intensive nature of standard methods; the Multiple Sleep Latency Test (MSLT) and the Maintenance of Wakefulness Test (MWT). Deep learning analysis of a simple fingertip pulse wave (photoplethysmography, PPG) recording could be a viable option for the routine assessment of sleepiness in patients with suspected sleep disorders. METHODS: A deep learning-based sleep staging model was tested in MSLT (n = 143) and MWT (n = 127) datasets. Th

Quantitative comparison of the performance of acoustic, optical and pressure sensors for pulse wave analysis.

Arterial pulse wave measurement is beneficial in clinical health assessment and is important for effectively diagnosing different types of cardiovascular disease. Computational pulse signal analysis utilizes sensors and signal processing techniques to understand, classify, and predict disease pulse patterns. However, the choice of sensor types impacts the measurement results. This study presents the first comprehensive quantitative comparison of three sensor modalities (acoustic, optical, and pressure) for radial pulse measurement, employing a novel multi-parameter analysis framework that comb

Arterial pulse wave analysis can detect vascular alterations in response to therapy in septic shock.

The severe vascular decoupling induced by septic shock may dissociate the physiological relation between pressure and flow in the periphery and this may have important clinical implications. We hypothesize that arterial pulse wave analysis (PWA) can track these vascular alterations and we tested this hypothesis in a swine experiment of septic shock and resuscitation with different drugs that differently interact with the cardiovascular system. PWA was found to effectively detect the different arterial response to treatment, proving to be highly informative in the monitoring of therapy response

Identifying Coronary Artery Lesions by Feature Analysis of Radial Pulse Wave: A Case-Control Study.

BACKGROUND: Cardiovascular diseases have been always the most common cause of morbidity and mortality worldwide. Health monitoring of high-risk and suspected patients is essential. Currently, invasive coronary angiography is still the most direct and accurate method of determining the severity of coronary artery lesions, but it may not be the optimal clinical choice for suspected patients who had clinical symptoms of coronary heart disease (CHD) such as chest pain but no coronary artery lesion. Modern medical research indicates that radial pulse waves contain substantial pathophysiologic infor

Arterial stiffness and central blood pressure, as determined by pulse wave analysis, in rheumatoid arthritis.

BACKGROUND: Rheumatoid arthritis (RA) is associated with increased cardiovascular mortality for reasons which are insufficiently understood. Chronic inflammation may impair vascular function and lead to an increase of arterial stiffness, an important determinant of cardiovascular risk. OBJECTIVE: To investigate the augmentation index (AIx) as a measure of arterial stiffness in patients with RA, free of cardiovascular disease or risk factors, by means of a matched cohort pilot study. METHOD: Patients with a diagnosis of RA, aged 50 years or younger, were screened for the absence of clinical c

Association between pulse wave velocity and coronary artery calcification: a systematic review and meta-analysis.

Cardiovascular diseases are the leading cause of mortality and morbidity combined worldwide. Traditional risk factors may underestimate risk in individuals with subclinical atherosclerosis, prompting the development of novel markers. Coronary artery calcium assessment has been proposed as a promising marker of subclinical atherosclerosis, although it involves exposure to ionizing radiation. This systematic review and meta-analysis aimed to explore the association between a noninvasive marker, pulse wave velocity (PWV), and coronary calcification, as well as the factors influencing this associa

Association between pulse wave velocity and extracoronary arterial calcification: a systematic review and meta-analysis.

Cardiovascular diseases remain the leading cause of mortality worldwide. Their increasing prevalence has focused attention on new promising biomarkers such as vascular calcification, although its assessment involves radiation exposure. Thus, this systematic review and meta-analysis aimed to examine the association between arterial stiffness, assessed through pulse wave velocity (PWV) and extracoronary arterial calcification (ECC), in addition to identifying possible factors influencing this relationship. A systematic search was conducted in PubMed, Scopus, and Web of Science according to PRISM

Wave separation analysis-derived indexes obtained from radial and carotid tonometry in healthy pregnancy and pregnancy-associated hypertension: Comparison with pulse wave analysis-derived indexes.

BACKGROUND: Increased wave reflections assessed by pulse wave analysis (PWA) was proposed as one of the potential culprits of hypertension seen in women with pregnancy-associated hypertension (PAH). However, this statement has never been confirmed with "Wave Separation Analysis" (WSA), a more sophisticated mathematical approach that analyzes the amplitude and interaction between forward and backward aortic pressure waveform components. OBJECTIVE: To characterize potential changes in pressure wave components of PAH compared to healthy non-pregnant (NP) women and women with normal pregnancies (

Effects of trigeminal nerve stimulation on parameters derived from pulse wave analysis in young healthy subjects.

Studies on trigeminal nerve stimulation (TNS) by facial cooling (FC) could give an insight to physiological mechanisms of cardiovascular response to activation of both parasympathetic and sympathetic nervous systems. We hypothesised that trigeminal nerve stimulation would positively influence on the factors which increase left ventricle work. The aim of the present study was to investigate hemodynamic changes assessed by pulse wave analysis (PWA) in the very early phase of this reflex. In 15 young (aged 20.3±1.3 y/o; mean ±SD) healthy individuals (10 males) PWA was performed before and after 3

Overnight pulse wave analysis to assess autonomic changes during sleep in insomnia patients and healthy sleepers.

Insomnia has been associated with increased cardiovascular (CV) risk, which may be linked to sympathetic activation. Non-invasive overnight pulse wave analysis may be a useful tool to detect early signs of autonomic changes during sleep in insomniacs. Fifty-two participants (26 men, 37±13 years, BMI: 24±5 kg/m2, 26 insomniacs/ 26 controls) underwent overnight polysomnography with pulse oximetry and pulse wave analysis including pulse rate, vascular stiffness (pulse propagation time, PPT), and a composite cardiac risk index based on autonomic function and overnight hypoxia. The association between pulse wave velocity and pregnancy-associated diseases: A systematic review and meta-analysis.

BACKGROUND: Maintaining healthy vascular structure and function is important for a healthy pregnancy. Obesity is a well-known predictor for poor postoperative outcomes of vascular surgery. However, the association between pulse wave velocity (PWV), a well-recognized parameter for arterial stiffness assessment, and pregnancy-associated diseases is still unclear. Therefore, we conducted this systematic review, and a meta-analysis was performed to assess the relevant associations. METHODS: We systematically searched the Web of Science and PubMed databases to obtain articles on PWV and pregnancy-
