Treatment of a lacrimal pneumocele with punctal plug insertion.

A 37-year-old man presented with symptoms of air and mucus reflux from the right medial canthus on blowing his nose. Examination showed crepitus in the region overlying the right lacrimal sac, and he was able to produce bubbles in his conjunctival lake on instillation of topical 2% fluorescein. Dacryoscintigraphy showed symmetrical slow passage of tracer from the lacrimal sac in the nasolacrimal duct indicating delay at the postsac level. Dacryocystography showed that the right lacrimal system contained round filling defects, suggesting the presence of soft tissue tumors. However, a CT scan sh

Plug or Punctoplasty (POPPY) Study: Perforated Punctal Plug Versus Three-Snip Punctoplasty in the Treatment of Acquired Punctal Stenosis.

PURPOSE: Acquired punctal stenosis (APS) represents a common cause of epiphora and a reason for attendance at oculoplastic clinics. The three-snip punctoplasty is the mainstay of APS treatment, although perforated punctal plugs (PPPs) represent an alternative, offering reversibility and less burden on minor-operation lists. This study aimed to identify if PPPs are an acceptable alternative to the three-snip punctoplasty through comparison of functional, anatomical, and quality of life outcomes. METHODS: Participants with APS were randomized to receive either a three-snip punctoplasty or PPP i

Failure of collagen plugs to predict epiphora after permanent punctal occlusion.

In two patients with keratoconjunctivitis sicca, epiphora did not occur after temporary canalicular occlusion with collagen implants, but did occur after subsequent permanent punctal occlusion. This suggests that collagen implants may not always produce total occlusion. Patients should be warned of this possibility.

Canalicular Cyst After Punctal Plug Insertion.

This study describes the case of a patient with a canalicular cyst after punctal plug insertion. A 73-year-old male patient presented with a cystic mass on the right lower eyelid. He had undergone bilateral punctal occlusion with an absorbable punctal plug for dry eye syndrome 8 years ago. The lesion was palpable on the medial side of the right lower eyelid, accompanied by ectropion but without any inflammatory signs. Incision and drainage were performed, and a reduction in the size of the cystic mass was observed. Dacryocystography revealed cystic dilation of the right lower canaliculus, and

Inferior punctal occlusion with removable silicone punctal plugs in the treatment of dry-eye related contact lens discomfort.

Thirty subjects complaining of a decrease in soft contact lens wearing time secondary to dry eye discomfort were selected for our study. All subjects were between the ages of 25 and 40 and selected on the basis of subjective dry eye complaints alone. Five of these subjects had punctum that were too small to facilitate plug placement and were therefore eliminated from further study. Removable silicone punctal plugs were monocularly placed in the lower puncta of the remaining 25 subjects. In an attempt to minimize the placebo effect, both puncta were manipulated equally prior to plug insertion a
