Lessons of the month: Pyroglutamic acidosis: long-term paracetamol and a high anion gap.

An 84-year-old woman presented in extremis with confusion and Kussmaul respiration. She had a history of urosepsis, renal impairment and osteoarthrosis. The venous blood gas showed a marked metabolic acidosis with a high anion gap. Lactate and ketones were normal. Her medications included regular paracetamol via a dosette box. Lactic acidosis and ketoacidosis being excluded, it emerged that the most likely cause of a high anion-gap acidosis in the presence of chronic paracetamol therapy is pyroglutamic acidosis, caused by the build-up of an acidic intermediate in the gamma-glutamyl cycle, the

Synthesis of α-Ketoglutaramic acid.

α-Ketoglutaramic acid (KGM, α-ketoglutaramate), also known as 2-oxoglutaramic acid (OGM, 2-oxoglutaramate), is a substrate of ω-amidase, also known as Nitrilase 2 (NIT2), and is essential for studying the canonical role of ω-amidase, as well as its role in multiple diseases. Until now, KGM used for biological studies has been prepared most often by the enzymatic oxidation of l-glutamine using snake venom l-amino acid oxidase, which provides KGM as an aqueous solution, containing by-products including 5-oxoproline and α-ketoglutarate. The enzymatic method for KGM preparation, therefore, cannot

An endogenous colon mitosis inhibitor and dietary calcium inhibit the increased colonic cell proliferation induced by cholic acid.

We have previously reported that a single intraperitoneal injection of the tripeptide pGlu-His-GlyOH (colon mitosis inhibitor (CMI] is followed by a transient reduction of cell proliferation in the colonic epithelium in mice. The effect of the tripeptide on accelerated cell proliferation in the colonic epithelium was tested during the acute response of the epithelial cells to a diet supplemented with cholic acid. The kinetics of this response was first examined with different amounts of calcium in the feed. We found an increased labeling index (LI) and mitotic rate 2 days after changing the di

Cuts by caspase-14 control the proteolysis of filaggrin.

Although mutations in the filaggrin gene (FLG) have been shown to be associated with ichthyosis vulgaris and atopic dermatitis, the function and regulation of filaggrin remain incompletely understood. In this issue, Hoste et al. report that filaggrin is directly cleaved by caspase-14. Acting in concert with other proteases, caspase-14 controls the breakdown of filaggrin to free amino acids and amino acid derivatives that contribute to the hydration and UVB absorption capacity of the stratum corneum. These findings identify a new layer of complexity in the regulation of epidermal barrier functi

Profound metabolic acidosis from pyroglutamic acidemia: an underappreciated cause of high anion gap metabolic acidosis.

The workup of the emergency patient with a raised anion gap metabolic acidosis includes assessment of the components of “MUDPILES” (methanol; uremia; diabetic ketoacidosis; paraldehyde; isoniazid, iron or inborn errors of metabolism; lactic acid; ethylene glycol; salicylates). This approach is usually sufficient for the majority of cases in the emergency department; however, there are many other etiologies not addressed in this mnemonic. Organic acids including 5-oxoproline (pyroglutamic acid) are rare but important causes of anion gap metabolic acidosis. We present the case of a patient with

The first identified neuropeptide in the insect order Megaloptera: a novel member of the adipokinetic hormone family in the alderfly Sialis lutaria.

This is the first report on the structural identity of a neuropeptide of the insect order Megaloptera. A peptide was isolated and sequenced from the retrocerebral corpora cardiaca glands of the alderfly, Sialis lutaria. The sequence of the peptide was deduced from the multiple MS(N) electrospray mass data as that of an octapeptide: pGlu-Ile/Leu-Thr-Phe-Thr-Pro-Ser-Trp amide. The ambiguity about the amino acid at position 2, Leu or Ile, was solved by comparing retention time on reversed-phase HPLC and establishing co-elution with the synthetic Leu(2)-form which also had exactly the same MS(2) m

Biochemical heterogeneity in glutathione synthetase deficiency.

Two different clinical syndromes are associated with glutathione synthetase deficiency, one presenting with hemolytic anemia and 5-oxoprolinuria, the other with isolated hemolysis. We have differentiated these disorders on an enzymatic basis. In 5-oxoprolinuria, all cell types examined have grossly deficient enzyme activity and glutathione content. In contrast, in the nonoxoprolinuric variant, erythrocytes have decreased enzyme activity and glutathione content, whereas nucleated cells maintain substantial levels of both. The enzyme in this disorder is unstable in vitro and has shortened surviv

Effects of analogs of (pyro)Glu-His-Gly-OH on food consumption and gastric acid secretion in rats.

The effects of administration of (pyro)Glu-His-Gly-OH and its analogs on food consumption and 2-Deoxy-D-Glucose (2-DG)-induced gastric acid secretion were examined in rats. (Pyro)Glu-His-Gly-OH and (pyro)Glu-His-EA significantly reduced food consumption, but not gastric acid secretion as compared with controls. The tripeptides: (pyro)Glu-3Me-His-Gly-OH, (pyro)Glu-His-D-Ala-OH, and (pyro)Glu-Phe-Gly-OH were found to significantly inhibit 2-DG-induced gastric acid secretion. Our results indicate that some analogs of (pyro)Glu-His-Gly-OH cause a greater inhibition of food consumption or gastric a

Zoomlab™-guided co-crystal engineering of nilotinib for improved dissolution.

Nilotinib (NH), a second-generation tyrosine kinase inhibitor for chronic myelogenous leukemia (CML), exhibits poor aqueous solubility and low intestinal permeability, classifying it as a Biopharmaceutics Classification System (BCS) Class IV drug. This study aimed to enhance NH solubility and dissolution through co-crystallization, guided by computational and experimental approaches. BASF's ZoomLab™ platform was utilized for rational coformer selection using the solubility parameter difference (Δδv) method. Validation with paracetamol and posaconazole datasets established 5 MPa0.5 as the optim

The Intrinsic Nutrient Sensing Adipokinetic Hormone Producing Cells Function in Modulation of Metabolism, Activity, and Stress.

All organisms confront the challenges of maintaining metabolic homeostasis in light of both variabilities in nutrient supplies and energetic costs of different physiologies and behaviors. While all cells are nutrient sensitive, only relative few cells within Metazoans are nutrient sensing cells. Nutrient sensing cells organize systemic behavioral and physiological responses to changing metabolic states. One group of cells present in the arthropods, is the adipokinetic hormone producing cells (APCs). APCs possess intrinsic nutrient sensors and receive contextual information regarding metabolic

Isolation of an anorexigenic protein from membranes.

Satiety means an internal state that leads to termination of eating. We have isolated an anorexigenic glycoprotein (M.W. 50,000 dalton) from human and rat erythrocyte membrane and from rat liver plasma membrane. The substance isolated from all these membrane sources has almost same onset and offset anorectic effect in rats deprived of food for 96 h as well as in normally fed rats without any rebound. Similar properties of membrane anorectic substance and plasma satietin indicated that it has membrane origin. The results also suggest that the loss of appetite in chronic diseases involving damag

Refractory high anion gap metabolic acidosis due to chronic paracetamol use: a case report.

INTRODUCTION: Metabolic acidosis is a frequent finding in clinical practice, particularly among critically ill patients. While common causes of high anion gap metabolic acidosis, such as diabetic ketoacidosis, renal failure, lactic acidosis, and toxins are easy to diagnose, less typical causes require heightened clinical suspicion. We report an unusual cause of high anion gap metabolic acidosis, which required specific therapeutic measures. CASE PRESENTATION: A 45-year-old Sinhalese female with diabetes presented with severe metabolic acidosis and Kussmaul breathing. Initial treatment for sep

Increased anion gap metabolic acidosis as a result of 5-oxoproline (pyroglutamic acid): a role for acetaminophen.

The endogenous organic acid metabolic acidoses that occur commonly in adults include lactic acidosis; ketoacidosis; acidosis that results from the ingestion of toxic substances such as methanol, ethylene glycol, or paraldehyde; and a component of the acidosis of kidney failure. Another rare but underdiagnosed cause of severe, high anion gap metabolic acidosis in adults is that due to accumulation of 5-oxoproline (pyroglutamic acid). Reported are four patients with this syndrome, and reviewed are 18 adult patients who were reported previously in the literature. Twenty-one patients had major exp

Inhibition of mouse-killing behaviour in magnesium-deficient rats: effect of pharmacological doses of magnesium pidolate, magnesium aspartate, magnesium lactate, magnesium gluconate and magnesium chloride.

Magnesium deprivation induced interspecific aggressive behaviour (muricidal behaviour) in rats undoubtedly attributable to magnesium deficiency since magnesium chloride, by correcting magnesium deficiency, suppressed it. Inhibition of magnesium deficiency-induced behaviour by various magnesium salts should enable the classification of the therapeutic effects of these salts. Consequently we compared the effects of various magnesium salts used therapeutically on the inhibition of the acute muricidal behaviour induced by magnesium deficiency. All the magnesium salts used (chloride, pidolate, aspa

Self-toxicity and tolerance mechanisms of honeybee venom in honeybees.

Honeybee venom (apitoxin) is a potent mixture of biologically active peptides and enzymes, primarily evolved as a defence against insect predators - including other bees. Recent evidence suggests that honeybees also employ components of apitoxin for both external and internal defence against parasites and microbial infections. Consequently, they are predicted to exhibit a remarkable resistance to their own venom, which they frequently encounter within colonies. To investigate this phenomenon, we examined the physiological responses of honeybee workers and drones to envenomation. Individuals we

Elimination of certain honeybee venom activities by adipokinetic hormone.

The primary aim of this study was to analyse the influence of honeybee venom on various aspects of Drosophila melanogaster physiology and to assess the efficacy of adipokinetic hormone (AKH) in mitigating venom toxicity. We examined the harmful effects of venom on the thoracic muscles and central nervous system of Drosophila, as well as the potential use of AKH to counteract these effects. The results demonstrated that envenomation altered AKH levels in the Drosophila CNS, promoted cell metabolism, as evidenced by an increase in citrate synthase activity in muscles, and improved relative cell
