BACKGROUND: Acute flaccid paralysis is an important clinical problem in neurological critical care. After implementing life-supporting measures, it is imperative to identify the correct diagnosis to provide timely appropriate care. Osteochondromas of the vertebral column are rare tumors and constitute about 3-4% of all primary vertebral column tumors. We report a case of osteochondroma arising from C3 lamina presenting with acute quadriplegia following a trivial fall. This paper describes a case of a 52-year-old male patient with spinal cord injury caused by doing push-up exercises in the prone position. To our knowledge, no previous report has been published describing a case of spinal cord injury caused by push-up exercise. These days, sport is recommended not only to the young but also to older persons. However, to the older, who inevitably have some degenerative disease, moderation should be recommended.

Fifty two patients with traumatic quadriplegia admitted to a spinal cord injury program within 6 months of injury were studied retrospectively. Seventy five per cent had shoulder pain documented in their medical records during initial rehabilitation, and 60% had shoulder pain for 2 weeks or more. The etiologic profile and possible predictors of etiology in children with spastic quadriplegia were assessed in a consecutive cohort of children with this motor impairment. Medical records from a single pediatric neurology practice over a 14-year interval were retrospectively and systematically reviewed. Rheumatoid arthritis often involves the axial skeleton. Rheumatoid nodule, often a manifestation of rheumatoid arthritis may be present in 20-30% of patients of rheumatoid arthritis. A 69-year-old male was presented with weakness of all the four limbs. He had history of rheumatoid arthritis for last 10 years. On examination, a soft cystic non-compressible lump was found over sacrum. Histopathological examination of the lump showed some changes which were consistent with the changes occur in rheumatoid nodule. The patient was treated conservatively and keeping well after 3 months of follow-up.

The purpose of this report is to describe our experience with the posterior interspinous fusion in the treatment of spinal injury with quadriplegia. The charts and roentgenograms of 22 patients treated with this operation by the senior author (RW) from July 1978 to June 1981 were reviewed retrospectively. Follow-up averaged 19.5 months. We present a case of post-operative iatrogenic quadriplegia, which occurred after subtotal parathyroidectomy. This patient was on long-term hemodialysis for 7 years. The need of prolonged neck extension for this procedure was probably the main risk factor for the spinal cord injury. Quadriplegics are able to compensate for alterations of operational length of the diaphragm by reflexly increasing neural drive to the diaphragm. This increase in neural drive is adequate to maintain required tidal volume and minute ventilation during quiet breathing in these patients with limited inspiratory muscle function.
