Radial head and neck fractures represent up to 14% of all pediatric elbow fractures and can be a difficult challenge in the pediatric patient. In up to 39% of proximal radius fractures, there is a concomitant fracture, which can easily be overlooked on the initial standard radiographs. The treatment options for proximal radius fractures in children range from non-surgical treatment, such as immobilization alone and closed reduction followed by immobilization, to more invasive options, including closed reduction with percutaneous pinning and open reduction with internal fixation. The choice of …

OBJECTIVES: After diagnosis of an isolated radial head or neck fracture and selection of nonoperative treatment, the value of subsequent radiographs is uncertain. This study tested the null hypothesis that there are no patient, surgeon, or injury factors associated with alteration in patient management based on subsequent radiographs. Secondarily, we tested the null hypothesis that the use of subsequent radiographs is not associated with patient, surgeon, and fracture characteristics.
METHODS: We identified 415 adult patients with nonoperative treatment for isolated Broberg and Morrey modifie

BACKGROUND: Minimally displaced radial head and neck fractures are common and the outcome with conservative treatment is generally excellent. A new protocol was introduced to manage patients with these suspected fractures at a major urban hospital. Simple, undisplaced fractures without other associated injuries or instability were discharged with structured advice but no further face-to-face review. Patients with more complex injuries were reviewed at a "virtual clinic." The aim of this study was to examine the outcome of this process in terms of patient flow, satisfaction, reattendance, and r…

Fracture-dislocations of the elbow can be difficult to treat, with unsatisfactory results in some cases. In general, it is preferable to preserve the fractured radial head when possible, but some patients present a unique treatment challenge because of extremely comminuted fractures and bone loss. In these cases, the only options available are radial head prosthesis or allograft. The authors present a case of a 45-year-old man with a fracture-dislocation of the left elbow that was treated with an allograft of the radial head and neck because of extreme comminution of the fracture. There have b

OBJECTIVE: To investigate the curative effects of open reduction and internal fixation with Kirschner wire for Mason type-II radial head or neck fracture in children.
METHODS: From September 2007 to June 2009, 17 cases of Mason type-II radial head or neck fracture were treated, including 11 males and 6 females with an average age of 8.5 years (4-11 years). The locations were left side in 5 cases and right side in 12 cases. All fractures were caused by falling and classified as Mason type-II fracture. Two cases complicated by radial nerve deep branch injury. The time from injury to operation w

A volar compartment syndrome of the forearm was identified following a minimally displaced or angulated fracture of the radial head or neck in three children. The fractures were due to a fall from a bed or from a standing height on an outstretched hand. All three patients had symptoms and signs that were consistent with elevated intracompartmental pressure in the forearm when they were first examined, twelve to twenty-four hours after the injury, and all were managed with an emergency fasciotomy of the forearm. The radial fracture was treated without reduction in the first patient, with manipu

The management of isolated radial neck and head fractures is controversial. Plate fixation and tripod fixation are two commonly employed tec … AVN (1.8%). No study reported any patients requiring revision surgery. Intramedullary fixation for radial head and neck fractures appears to provide a reliable alternative treatment option in terms of union, range of motion, and functional outcomes. Further robust trials direct comparing against open fixation techniques are required.

BACKGROUND: Monteggia equivalent lesion represents a series of combined elbow and forearm injuries that resemble typical Monteggia fracture either in presentation or mechanism. The term has gradually evolved since its introduction, as sporadic case reports continued to complement it. The aim of this study was to present a furthermore type of that lesion which no previous study had reported and arouse pediatric orthopedists' additional awareness of it.
CASE PRESENTATION: A 11-year-old girl, whose injury pattern initially appeared to be a mild radial neck fracture with undisplaced proximal ulna

The authors report four cases of Jeffery type 2 lesions in children. Despite the probable mechanism of such lesions, none of the cases reported here displayed a dislocation of the elbow. In the first observation the diagnosis was suspected only 15 days after injury. Two other cases were initially treated by closed external manipulations, one with percutaneous Kirschner wire leverage and the other with closed intramedullary pinning, but both cases displayed a 180 degree reversal of the radial head, suspected only secondarily. The first exhaustive review of the literature on the subject, shows t

BACKGROUND: Owing to the great variety of fracture patterns and limitations of the standard radiographic investigation, all the already available classification systems for radial head and neck fractures (RHNFs) are limited by a poor-to-moderate degree of intraobserver and interobserver reliability. Although computed tomography (CT) is being increasingly used to better understand the fracture characteristics, a CT-based classification system of RHNFs is still lacking. Therefore, in this agreement study, we aimed to propose a classification system based on two-dimensional and three-dimensional …

CASE: A 10-year-old girl sustained a radial neck fracture with a posterior elbow dislocation. She was treated with closed reduction of the elbow with subsequent intra-articular displacement of the radial head, which necessitated open reduction and pinning of the radial neck fracture.
CONCLUSION: Displacement of the radial neck fracture from impingement of the capitellum on the anterior radial head during closed reduction of the elbow dislocation is a rare injury pattern. It is important to examine the radial neck in high-energy posterior elbow dislocations before attempted reduction. We prese

PURPOSE: Isolated pediatric radial head and neck fractures are rare. In recent literature, their incidence is estimated to be around 1% of all fractures. High rates of complications are reported. Beside non-operative treatment, head fractures are treated with k-wires, mini-screws or polypins, whereas neck fractures are treated more and more with elastic stable intramedullary nailing (ESIN). Purpose of the study was to evaluate the operative management, complications and clinical outcomes of these injuries.
METHODS: Retrospective analysis between 2002 and 2014. 19 children with isolated radial…

BACKGROUND: Reviewing the current literature, very few reports are given on simultaneous bilateral radial head and neck fractures. There are no reports on this entity's incidence. Thus, the purpose of this study was to analyze the incidence and outcome of simultaneous bilateral radial head fractures.
METHODS: This study reviewed the clinical records and trauma database of this Level I Trauma Center and identified all adult patients with fractures of the radial head or neck who were admitted between 1992 and 2007. From a database of 2,296 adult trauma victims with radial head or neck fractures…

This is a review of 116 children who had a fracture of the proximal radial head or neck over a 15-year period. Of 33 teenagers with closed physes, 17 (52%) had intraarticular involvement. Of 83 younger children with an open proximal radial physis, six (7%) had an intraarticular fracture (Salter-Harris type III or IV). Of the 17 patients with closed physes and intraarticular fracture, 13 had adequate follow-up. There were eight excellent, three good, one fair, and one poor results. Of the six children with open physes and intraarticular fracture, there were one good and five poor results. This

Fractures involving multiple levels of the radius are rare and often associated with ulnar involvement or distal radius injury. We report a previously undocumented injury pattern: a comminuted ipsilateral fracture of the radial head, neck, and shaft, with an intact ulna, in a 28-year-old male following road traffic trauma. The classification system of the Arbeitsgemeinschaft für Osteosynthesefragen (AO), also known as the Working Group for Osteosynthesis Questions, does not include this type of fracture configuration. The patient underwent open reduction and internal fixation using a headless

BACKGROUND: This retrospective study aimed to compare the efficacy of internal fixation using headless compression screws (HCS) and radial head locking plate (RLP) for comminuted radial head and neck fractures with no more than 3 displaced fragments.
METHODS: This retrospective study included 84 patients with radial head and neck fractures treated at Peking University Third Hospital between January 2013 and December 2022, with 38 and 46 patients in the HCS and RLP groups, respectively. The main outcome was the comparison of complications between the two groups. Demographic data, pre-operative…

BACKGROUND: Nondisplaced and minimally displaced fractures of the radial head and neck are common injuries, yet the role of physical therapy (PT) in their treatment is unclear. The aim of this trial was to assess the need for formal PT following a simple fracture of the radial head or neck.
METHODS: Patients who had a nondisplaced or minimally displaced fracture of the radial head or neck and presented to 1 of 2 providers were enrolled prospectively between January 2014 and August 2016. Patients were randomized to receive outpatient PT or perform self-directed home exercise. The follow-up int
