Nerve Versus Tendon Transfer for Radial Nerve Paralysis Reconstruction.

PURPOSE: With radial nerve lesions, the results of nerve transfers and how they objectively compare with the outcomes of tendon transfers remain unstudied. We compared the results after nerve transfer in patients with less than 12 months since radial nerve injury with the results after tendon transfer in patients not eligible for nerve surgery because of longstanding paralysis (minimum of 15 months). METHODS: In 14 patients with radial nerve lesions incurred less than 12 months previously, we transferred the anterior interosseous nerve to the nerve of the extensor carpi radialis brevis (ECRB)

Spontaneous nerve torsion: unusual cause of radial nerve palsy.

Spontaneous nerve torsion is a rare cause of nerve palsy. We describe a case of nerve torsion affecting the radial nerve in order to inform radiologists of the existence of this condition and subtle features on cross-sectional imaging that can suggest the diagnosis preoperatively.

Superficial radial nerve-lateral antebrachial cutaneous nerve anatomic variation.

INTRODUCTION: This study focuses on an anatomic variation in which the lateral antebrachial cutaneous nerve (LACN) innervates the radial border of the dorsum of the hand and thumb in addition to, or replacing, the superficial radial nerve (RSN). Here, we propose a technique of nerve conduction that identifies this variation. METHODS: We studied nerve conduction in 200 upper limbs of two series of 50 volunteers. We sought evidence of the aforementioned variation on the dorsum of the hand and in the thumb. RESULTS: We found eight occurrences of this variation on the dorsum of the hand and 11 v

Results of interfascicular nerve grafting for radial nerve lesions.

During the 10 year interval 1979-1989, 20 patients underwent nerve grafting of a radial nerve lesion, 13 high radial and 7 posterior interosseous. Average follow-up was 38 months (range 12 months-10 years). Overall 72% of patients achieved a Highet Scale rating of M3 or better function and 44% M4 or better recovery. Age of the patient and length of the nerve graft did not seem to influence outcome. Time from initial injury to nerve grafting did affect outcome, with 85% of patients grafted within 6 months obtaining M3 or better recovery. No patient grafted 12 months after injury recovered any u

Low profile radial nerve palsy orthosis with radial and ulnar deviation.

Individuals who sustain damage to the radial nerve experience a significant loss in functional use of the hand. Traditional orthoses have been effective in providing assistance with wrist stabilization and finger/thumb MP extension. These authors adapted a low profile orthosis to provide the necessary support while allowing radial and ulnar deviation of the wrist, thus increasing functional use of the hand.--Victoria Priganc, PhD, OTR, CHT, CLT, Practice Forum Editor.

Current approach to radial nerve paralysis.

After studying this article, the participant should be able to: 1. Identify all potential points of radial nerve compression and other likely causes of radial nerve injury. 2. Accurately diagnose both surgical and nonsurgical causes of radial nerve paralysis. 3. Define a safe and effective approach to the surgical release and reconstruction of the radial nerve. Radial nerve paralysis, which can result from a complex humerus fracture, direct nerve trauma, compressive neuropathies, neuritis, or (rarely) from malignant tumor formation, has been reported throughout the literature, with some contro

[Radial nerve paralysis in congenital angioleiomyoma].

An isolated paralysis of the radial nerve in a newborn was reported. The displaced growth of the histologically proved angioleiomyoma caused a circumferential compression with subsequent atrophy of the nerve (Fig. 4). During operative treatment of the tumor, the damaged part of the nerve was resected, too. In a second stage the defect in the nerve was closed by suralis grafts. Possibly due to scar adhesions a good result was not obtained.

[Nerve compression syndromes of the median and radial nerves at the elbow].

Nerve compression syndromes of the median and radial nerves at the elbow are usually caused by a combination of traction and pressure and are mainly characterized by loss of strength and dull, localized elbow pain. Muscular functional impairments can occur, whereas sensory impairments are less common. The diagnostics including a detailed medical history, neurological examination with, for example, lateral comparison muscle testing and imaging procedures such as X‑ray, sonography or magnetic resonance imaging (MRI) are essential. In the early stages, conservative treatment with immobilization a

Nerve Transfer Versus Tendon Transfer for Isolated Radial Nerve Palsy.

BACKGROUND: Reconstructive options for high radial nerve injuries include tendon transfer (TT) and nerve transfer (NT), each with distinct advantages and limitations. This prospective, randomized clinical trial compared the outcomes of TT and NT in 21 patients with isolated radial nerve injuries. METHODS: Patients were randomly assigned to the NT group (11 patients) or the TT group (10 patients). Nerve transfer involved transferring the distal anterior interosseous nerve to the extensor carpi radialis brevis motor branch and the flexor carpi radialis motor branch to the posterior interosseous

Comparison of nerve versus tendon transfer for radial nerve palsy.

OBJECTIVE: This study aims to investigate the choice of intervention time and operation mode between nerve and tendon transfer for the treatment of radial nerve palsy (RNP). METHODS: 46 RNP patients underwent nerve transfer (n = 22) and tendon transfer (n = 24). The intraoperative blood loss, main incision length, operation duration, and length of hospital stay and follow-up period of patients in these two groups were recorded and compared. The range of motion (ROM) of the elbow, wrist, fingers, and thumb, the hand grip and pinch strength, the Disabilities of Arm, Shoulder, and Hand (DASH) an

Radial tunnel syndrome. A retrospective review of 30 decompressions of the radial nerve.

Radial tunnel syndrome results from compression of the radial nerve by the free edge of the supinator muscle or closely related structures in the vicinity of the elbow joint. Despite numerous reports on the surgical management of this disorder, it remains largely unrecognized and often neglected. The symptoms of radial tunnel syndrome can resemble those of tennis elbow, chronic wrist pain or tenosynovitis. Reliable objective criteria are not available to differentiate between these pathologies. These difficulties are discussed in relation to 29 patients who underwent 30 primary explorations an

Quantified relationships of the radial nerve with the radial groove and selected humeral landmarks.

Anatomical relationships between the radial nerve, the deltoid muscle insertions and several bony landmarks have been investigated to assess the feasibility of surgical transfer of the deltoid transfer during humeral osteotomy. Eleven embalmed human specimens were dissected. Each specimen included the whole thorax, both shoulders and upper limbs. Spatial position of the radial nerve along the radial groove, the deltoid muscle, and several anatomical landmarks was digitised using a three-dimensional (3D) digitiser. Sixteen distances and one angle characterizing the relationships between the pat

[Repair and reconstruction of radial nerve injury].

OBJECTIVE: To investigate the reparative method and effect of radial nerve injury. METHODS: From 1990 to 2000, 50 cases with radial nerve injuries were adopted in this study. Among them, there were 38 males and 12 females, aged from 5 to 65 years. For the 50 cases, there were 28 cases with complete nerve rupture, 3 cases with incomplete nerve rupture, 10 cases with compressive injury, 5 cases with contusion injury and 4 cases with defect and irreversible injury. All the patients were treated with radial nerve exploration in 1 hour to 6 months after injury. 31 cases were treated with nerve ana

[Humeral fractures with paralysis of the radial nerve].

Forty fractures of the humerus associated with radial nerve palsy were reviewed after an average follow-up of 11 years. Early exploration of the radial nerve and internal fixation of the fracture was performed in 20 patients, while the other 20 patients were treated by closed reduction and immobilization in a plaster cast. In four patients of the latter group the radial nerve did not recover and a delayed exploration was performed. In each treatment group functional recovery of the radial nerve was observed in 16 patients (80%). On the basis of these results the authors suggest treating fractu

[Problems in severe injuries of the radial nerve].

By the presentation of the clinical material a series of 14 cases of the radial nerve microsurgical reparation has been analyzed with the reference to the modern achievements in this field and the possibilities of its quicker and better rehabilitation have been quoted. In our material we used the intraneural microsurgical technique with the direct funiculoraphy (3), the interfascicular nerve transplantation (8) and the external and internal microneurolysis (3). Twelve reconstructed radial nerves were evaluated by these methods. The successful motoric function (M3 and more), with the performanc

Isolated radial nerve lesion in the newborn.

Two newborn infants had isolated radial nerve lesion documented on electromygraphy. Fibrillation potentials present in one child at age 6 days suggest the possibility of an in utero onset. Skin necrosis present above the triceps muscle and radial nerve favors an entrapment mechanism, possibly from the umbilical cord. Because both patients had complete resolution in 4 months, it is important to differentiate this lesion form the more common but more serious medial brachial plexus lesion.
