Lumbosacral radiculopathy.

Lumbosacral radiculopathy is one of the most common disorders evaluated by neurologists and is a leading referral diagnosis for the performance of electromyography. Although precise epidemiologic data are difficult to establish, the prevalence of lumbosacral radiculopathy is approximately 3% to 5%, distributed equally in men and women. Degenerative spondyloarthropathies are the principal underlying cause of these clinical syndromes and are increasingly commonplace with age. Men are most likely to develop symptoms in their 40s, whereas women are affected most commonly between ages 50 and 60. Th

Thoracoabdominal radiculopathy.

Thoracic nerve root dysfunction (TNRD) manifested as abdominal pain is an infrequently reported condition. We present data on six patients who had chronic intermittent thoracoabdominal pain originating in the back. Diabetes and osteoarthritis of the spine were the chief causes of these symptoms. The electromyogram in all patients showed changes consistent with an acute radiculopathy. All patients responded to anti-inflammatory therapy in combination with phenytoin, carbamazepine, amitriptyline, or local nerve block. TNRD is a condition that may be diagnosed earlier if clinical suspicion is inc

Electrodiagnosis of radiculopathy.

While nerve conduction studies are an important part of the electrodiagnostic examination (EDX), the diagnosis of a radiculopathy rests mainly on the needle electrode examination (NEE) findings. Myotomal spontaneous activity and neurogenic motor unit potential (MUP) changes are the key abnormalities seen. To optimize the rate of identifying a radiculopathy, selection of appropriate muscles for the NEE is paramount. Myotomal charts derived from anatomic, radiographic, and EDX studies may help guide development of the NEE protocol for radiculopathy and increase the diagnostic yield. As recommend

Epidemiology of radiculopathies.

Many diseases affecting the peripheral nervous system or the spinal canal may be associated with pathology in the nerve roots, but the most common sources of radiculopathy are conditions that cause direct compression of nerve roots, including prolapsed intervertebral disks, disk degeneration, and osteoarthrosis of the apophyseal joints. The risk for prolapsed lumbar disk is highest in the group 20 to 39 years of age, and it is probably increased by the driving of motor vehicles, full-term pregnancies, and sedentary occupations. Cervical disk prolapses tend to occur in a slightly older age grou

Approach to Radiculopathy.

Low back pain and neck pain, often with associated radiculopathy, are two of the most common reasons for referral to the outpatient neurology clinic. A thorough clinical evaluation remains paramount in establishing an accurate diagnosis and subsequently an appropriate treatment plan. In this article, we review anatomic considerations for spondylotic radiculopathy; outline the clinical approach for the evaluation of these patients, including discussion of electrodiagnostic and imaging modalities; and address treatment options based on a stratified treatment approach.

Radiculopathy and Plexopathy

Disorders of the nerve roots and neural plexi present with pain, numbness, or weakness in the neck, back, or extremities. Although the history and physical examination provide essential diagnostic information, imaging and electrodiagnostic studies may further aid in localizing and characterizing the underlying lesion. Causes of radiculopathy include intervertebral disc herniation, spondylosis, spinal synovial cysts, infection, metastatic disease, hematoma, or infiltrative disease. The brachial and lumbosacral plexi are susceptible to trauma, structural anomalies, neoplastic infiltration, and i

Imaging in radiculopathy.

All patients presenting with signs and symptoms of lumbar radiculopathy must undergo a thorough history taking and physical examination. Often, however, the diagnosis remains unclear; it is accurate anatomically, but the underlying cause of the lesion is not confirmed, or the symptoms are so severe that more information on the anatomy is required. Therefore, the next step in the diagnostic process is imaging studies. This article discusses imaging modalities, including plain radiographs, magnetic resonance imaging, computed tomography (CT), CT myelogram, selective nerve root block, and bone sc

Electrophysiology of radiculopathies.

The anatomy, pathophysiology, and clinical evaluation of radiculopathies are discussed. Defining whether root injury is present and which roots are involved can be difficult but critical for patient management. In conjunction with clinical and radiological information, studies that establish physiological abnormalities of roots should be helpful and important. Clinical neurophysiological studies for radiculopathies are performed frequently but have yet to achieve a universally accepted role in the evaluation of these patients. Electrophysiological techniques for the evaluation of radiculopathi

Herpes zoster radiculopathy.

Herpes zoster-related radiculopathy usually can be easily diagnosed in the presence of cutaneous lesions. Before development of the skin rash, the diagnosis may be in doubt, particularly if motor symptoms and signs are a major clinical feature. We report a patient with herpes zoster-related radiculopathy whose clinical features mimicked other spinal disorders.

[Lumbosacral neoplastic radiculopathy].

Lumbar-disc protrusions (LDP) constitute well-defined syndromes on clinical and anatomical grounds, and neurosurgeons are prone to rely upon the clinical signs to identify the level of disc protrusion when a "typical" case is found. Sometimes, non-contrasted computerized tomographic (CT) scans centered on the L5-S1, L4-L5 and L3-L4 interspaces and spine roentgenograms are the only special ancillary means in presurgical evaluation. We report three patients from our series, in which neoplastic spinal disease presented as classic LDP (one patient with a cauda equina schwannoma, and two with metas

Acute Lumbar Radiculopathy

Chapter 19 presents a case of acute lumbar radiculopathy, and discusses that acute lumbosacral radicular pain is usually due to disc rupture and herniation or exacerbation of spinal stenosis, but neoplasm, fracture, infection, and compressive cauda equine syndrome must be excluded, that the most common lumbosacral roots to be symptomatic are S1 and L5, although L4 can also be involved, how pain can usually be controlled medically, and that manual and surgical approaches to acute radicular pain seem to be equivalent in terms of long-term outcome, although surgical intervention can provide some

Radiculopathy index: a proposition of uniformed clinical examination in cervical radiculopathy.

Background. Radiculopathy is a common cause of upper limb dysfunction. Diagnosis of the disorder is based upon clinical examination. A new diagnostic tool - Radiculopathy Index - is described and experimentaly applied in patients with spondylosis related chronic cervical radiculopathy.
Material and methods. We examined 82 patients (62 women and 20 men) aged 25-77 (mean 54,2) with radicular symptoms lasting from 0,5 to 30 years (mean 5,77). Unilateral radiculopathy was diagnosed in 60 patients and bilateral syndrome was revealed in 22 patients. 104 affected limbs were analysed.
Resu

Low back pain, radiculopathy.

Low back pain is a pervasive problem in the adult population. Most patients with low back pain will not require imaging as spontaneous recovery within 12 weeks is the rule. However, a small percentage of patients with low back pain will have serious underlying pathology requiring more intensive investigation. This chapter delineates the signs and symptoms related to potential serious underlying causes and discusses appropriate imaging modalities that should be utilized in patients with low back pain.

Electrodiagnosis of cervical radiculopathy.

Cervical radiculopathy is a common diagnosis with a peak onset in the fifth decade. The most commonly affected nerve root is C7, C6, and C8. The etiology is often compressive, but may arise from noncompressive sources. Patients commonly complain of pain, weakness, numbness, and/or tingling. Examination may reveal sensory or motor disturbance in a dermatomal/myotomal distribution. Neural compression and tension signs may be positive. Diagnostic tests include imaging and electrodiagnostic study. Electrodiagnostic study serves as an extension of the neurologic examination. Electrodiagnostic findi

Cervical radiculopathy: a review.

BACKGROUND: Cervical radiculopathy is defined as a syndrome of pain and/or sensorimotor deficits due to compression of a cervical nerve root. Understanding of this disease is vital for rapid diagnosis and treatment of patients with this condition, facilitating their recovery and return to regular activity. PURPOSE: This review is designed to clarify (1) the pathophysiology that leads to nerve root compression; (2) the diagnosis of the disease guided by history, physical exam, imaging, and electrophysiology; and (3) operative and non-operative options for treatment and how these should be appl

Natural history of radiculopathy.

Radicular pain is a frequent complaint of patients presenting to outpatient primary care and musculoskeletal clinics. Most cases of radiculopathy are self-limiting, and symptoms resolve over the course of weeks to months. There is spontaneous resolution of disc herniations, and clinical improvement correlates with morphologic resolution. Knowledge of the natural history of radiculopathy is crucial for the health care provider to appropriately counsel and treat patients with this disorder. Although each patient should be managed individually, the favorable prognosis of radiculopathy based on th
