Radiological Loco-Regional Treatments for Cholangiocarcinoma: A Systematic Review of the Literature.

Cholangiocarcinoma (CCA) is the second most common primary liver cancer and is being diagnosed with increasing frequency worldwide. We performed a systematic review on radiofrequency (RFA), microwave (MWA), irreversible electroporation (IRE), trans-arterial chemoembolization (TACE) and selective internal radiation therapy (SIRT) in CCA. Thirty-one ablation studies (1526 patients), 21 TACE studies (1594 patients), and 38 SIRT studies (1954 patients) were included; most cohorts involved intrahepatic CCA (iCCA) and originated from Asia. For RFA/MWA, overall survival (OS) typically exceeded 25 mon

Predictors and outcomes of hepatic decompensation following transarterial locoregional therapy in hepatocellular carcinoma.

Transarterial radioembolization (TARE) and transarterial chemoembolization (TACE) are recognized locoregional therapies (LRTs) for patients with unresectable hepatocellular carcinoma (uHCC). We assessed the predictors of hepatic decompensation (HD) and its impact on overall survival (OS) in patients with uHCC undergoing TACE and TARE.We analyzed patients treated with TARE (n=135) or TACE (n=177) at four tertiary centers. HD was defined as ascites, variceal bleeding or portosystemic encephalopathy at baseline or its occurrence ≤6 months post LRT. Median OS and objective response (per mRECIST) w

Hepatocellular carcinoma: new therapies on the horizon.

INTRODUCTION: Although immune checkpoint inhibitor (ICI)-based combinations have established the frontline standard for advanced hepatocellular carcinoma (HCC), substantial unmet needs persist-limited response rates, primary and acquired resistance, absent validated predictive biomarkers, and uncertainty regarding optimal sequencing. Hepatic decompensation may rival tumor progression as a determinant of survival in selected patients. AREAS COVERED: Informed by a focused PubMed and Embase search prioritizing literature (January 2020-April 2026), this review examines near-horizon HCC therapies

Liver-Directed Therapy for Colorectal Cancer: Where Are We Now?

Approximately 25% of patients with colorectal cancer present with metastatic disease to the liver at diagnosis, and about 50% develop liver metastases at some point during the course of the disease. Surgical resection of hepatic metastases remains the gold standard treatment for liver metastases. However, many patients with colorectal liver metastases (CRLMs) are not eligible for up-front resection. Despite advances in systemic chemotherapy, long-term survival continues to be strongly influenced by liver-directed treatments. A multidisciplinary approach is recommended for all patients, with a

Selective internal radiotherapy vs transarterial chemoembolization for unresectable hepatocellular carcinoma: a systematic review.

BACKGROUND: Hepatocellular carcinoma (HCC) is the most common primary liver tumor, with transarterial chemoembolization (TACE) and selective internal radiotherapy (SIRT) as main therapeutic options for patients with intermediate-stage HCC. This review compares TACE and SIRT outcomes in unresectable HCC. METHODS: A literature search identified studies on HCC patients treated with TACE or SIRT. Quality was assessed using the Cochrane risk-of-bias 2 tool. Primary and secondary outcomes included overall survival (OS), time-to-progression (TTP), and progression-free survival (PFS). RESULTS: Ten s

Y-90 Radioembolization Through Aberrant Arterial Supply: Four-Case Experience.

Variations in hepatic vascular anatomy are commonly observed. There is limited evidence regarding Y-90 radioembolization (RE) via nonhepatic arterial feeders. In tumors with aberrant vascularization, RE is sometimes replaced by TACE or abandoned due to potential complications. Liver lesions may also receive blood supply from extrahepatic arteries depending on their location, and failure to recognize these vessels may result in suboptimal therapy. This case series shows that in selected patients, and with careful prevention of extrahepatic leakage, RE can be safely and effectively performed usi

A comprehensive review on the current applications and future perspectives of radioembolization in endovascular neurosurgery.

In recent years, there has been advancement in the management of primary and secondary brain tumors within a multidisciplinary framework. However, substantial challenges persist in optimizing treatment strategies, enhancing survival rates, and improving patient prognosis and quality of life. These challenges stem from the complex interplay of biological processes that drive tumor pathogenesis and encompass factors such as tumor heterogeneity and patient variability together with surgical access and resectability in certain brain areas. Current therapeutic modalities - surgical resection, radio

Convergence of Local and Systemic Therapies: A Comprehensive Society of Interventional Oncology Framework for 90Y Radioembolization with Immunotherapy in HCC.

Combination approaches using systemic immunotherapy agents are now standard of care for patients with advanced-stage hepatocellular carcinoma (HCC), resulting in improved overall survival. However, even with optimal systemic regimens, fewer than 40% of cases respond to treatment, presumably due to resistance mechanisms, including antidrug antibodies and acquired resistance related to alterations in the tumor immune microenvironment (TIME). As a result, new strategies are needed to improve immunotherapeutic efficacy in this setting. Early investigations into the local and systemic effects of yt

Dosimetric assessment of Gadolinium-159 for hepatic radioembolization: Tomographic images and Monte Carlo simulation.

A common therapeutic radionuclide used in hepatic radioembolization is yttrium-90 (90Y). However, the absence of gamma emissions makes it difficult to verify the post-treatment distribution of 90Y microspheres. Gadolinium-159 (159Gd) has physical properties that are suitable for therapy and post-treatment imaging in hepatic radioembolization procedures. The current study is innovative for conducting a dosimetric investigation of the use of 159Gd in hepatic radioembolization by simulating tomographic images using the Geant4 application for tomographic emission (GATE) Monte Carlo (MC) simulation

A PEI-Based Surface Strategy for Robust 177Lu Incorporation into Biodegradable Chitosan Microspheres: A Potential Platform for Hepatocellular Carcinoma Radioembolization.

While lutetium-177 (177Lu) holds immense promise as a theranostic radionuclide for image-guided radioembolization of hepatocellular carcinoma, its translation into effective biodegradable microsphere platforms has been critically hindered by the lack of stable and efficient carriers capable of robustly incorporating the radionuclide while maintaining biodegradability. To bridge this gap, we herein develop biodegradable chitosan microspheres engineered with an aminated surface via sequential grafting of poly(glycidyl methacrylate) (PGMA) and polyethylenimine (PEI) to establish a robust Lu(III)

Where does histotripsy fit into hepatocellular carcinoma care?

Histotripsy is a novel ultrasound-guided ablation technique that has been approved by the US Food and Drug Administration as a medical device for the noninvasive destruction of liver tumors. The noninvasive, nonthermal, nonionizing mechanical mechanism by which it destroys tumor tissues makes it an appealing local therapy option in the treatment of patients with early-stage hepatocellular carcinoma (HCC). In this article, the authors review the current evidence base for histotripsy in HCC and explore its potential role in routine care based on technical, safety, and clinical considerations. Th

Portal vein embolization versus radiation lobectomy as pre-treatment for major liver resection for colorectal liver metastases: functional assessment of the future liver remnant.

INTRODUCTION: Insufficiency of the future liver remnant (FLR) precludes surgery for liver tumors as it is associated with post-hepatectomy liver failure (PHLF). A common strategy to induce pre-operative FLR hypertrophy is portal vein embolization of the tumor bearing liver lobe. More recently, transarterial radio-embolization (radiation lobectomy, RL) has been employed. Direct functional assessment using 99mTc-mebrofenin-hepatobiliary-scintigraphy (HBS) predicts FLR sufficiency more accurately than conventional volume assessment. However, studies describing dynamic functional assessment of the

Yttrium-90 Transarterial Radioembolization for Intrahepatic Cholangiocarcinoma: A Comprehensive Review.

Intrahepatic cholangiocarcinoma (iCCA) represents an aggressive primary hepatic malignancy with limited non-surgical therapeutic options. This review examines the evolution of transarterial radioembolization (TARE) with Yttrium-90 microspheres from palliative intervention to curative-intent therapy, driven by advances over the past few decades. Partition-model dosimetry demonstrates superior survival compared to empiric approaches (14.9 versus 5.5 months, P < 0.001), establishing the critical importance of individualized treatment planning. In first-line settings, multimodal strategies combini

177Lu-Loaded Composite Microspheres with Micro/Nano-Structure for Radioembolization Therapy of Hepatocellular Carcinoma.

Transarterial radioembolization (TARE) is a well-established therapy for hepatocellular carcinoma. However, existing radioembolic microspheres face limitations, such as single size selection, poor elasticity, lack of theranostic integration, and insufficient stability, which limit their clinical application in TARE. In this study, polydopamine@chitosan (PDA@CS) microspheres featuring a micro-nano composite structure are fabricated by a facile microfluidic method. These microspheres stably chelate Lu3+ (forming 177Lu-PDA@CS microspheres) through catechol-metal coordination via the phenolic hydr
