Dual-Energy CT: Basic Principles, Technical Approaches, and Applications in Musculoskeletal Imaging (Part 2).

In recent years, technological advances have allowed manufacturers to implement dual-energy computed tomography (DECT) on clinical scanners. With its unique ability to differentiate basis materials by their atomic number, DECT has opened new perspectives in imaging. DECT has been successfully used in musculoskeletal imaging with applications ranging from detection, characterization, and quantification of crystal and iron deposits, to simulation of noncalcium (improving the visualization of bone marrow lesions) or noniodine images. Furthermore, the data acquired with DECT can be postprocessed t

Technical Note: Using virtual noncontrast images from dual-energy CT to eliminate the need of precontrast CT for x-ray radiation treatment planning of abdominal tumors†.

PURPOSE: Radiation therapy (RT) planning frequently utilizes contrast-enhanced CT. However, dose calculations should not be performed on a contrast-enhanced CT because the patient will not receive bolus during treatment. It is typical to acquire CT twice during RT simulation: once before injection of bolus and once after. The registration between these datasets introduces errors. In this work, we investigate the use of virtual noncontrast images (VNC) derived from dual-energy CT (DECT) to eliminate the precontrast CT and the registration error. METHODS: CT datasets, including conventional 120

Virtual non-enhanced dual-energy computed tomography reconstruction: a candidate to replace true non-enhanced computed tomography scans in the setting of suspected liver alveolar echinococcosis.

PURPOSE: When a suspected hepatic alveolar echinococcosis (AE) lesion is detected on a contrast enhanced computed tomography (CT) scan, an additional triphasic or non-enhanced CT scan is required to determine the presence of calcification and enhancement. As a result, imaging costs and exposure to ionizing radiation will increase. We can create a non-enhanced series from routine contrast-enhanced images using dual-energy CT (DECT) and virtual non-enhanced (VNE) images. This study's objective is to assess virtual non-enhanced DECT reconstruction as a potential diagnostic tool for hepatic AE. M

Optimizing twin-beam dual-energy CT reconstruction: Quantitative consistency and stability assessment in reference to 120 kV: An observational study.

The split filter CT can filter X-ray beam. Theoretically, the split filter CT not only provides a good low-energy beam, but also provides a more robust CT value. The aim of this study was to compare conventional single-energy computed tomography (SECT) and twin-beam dual-energy (TBDE) CT regarding the quantitative consistency and stabilities of HU measurements at different abdominal organs. Forty-four patients were prospectively enrolled to randomly receive SECT and TBDE protocols at either body part of a thorax-abdominal examination. Their overlapping scan coverage was subjected to further im

Evaluating renal lesions using deep-learning based extension of dual-energy FoV in dual-source CT-A retrospective pilot study.

PURPOSE: Dual-source (DS) CT, dual-energy (DE) field of view (FoV) is limited to the size of the smaller detector array. The purpose was to establish a deep learning-based approach to DE extrapolation by estimating missing image data using data from both tubes to evaluate renal lesions. METHOD: A DE extrapolation deep-learning (DEEDL) algorithm had been trained on DECT data of 50 patients using a DSCT with DE-FoV = 33 cm (Somatom Flash). Data from 128 patients with known renal lesions falling within DE-FoV was retrospectively collected (100/140 kVp; reference dataset 1). A smaller DE-FoV = 20

Dual-energy and photon-counting computed tomography in critical care.

Dual-energy computed tomography (DECT) acquires images at two X-ray energy levels, enabling material differentiation beyond conventional single-energy CT. DECT generates iodine maps, virtual non-contrast images, and other reconstructions that enhance diagnostic performance while reducing contrast dose and radiation exposure. These advantages are particularly relevant for critically ill patients, where accurate and rapid imaging with reduced contrast exposure can be organ or life saving. This review summarizes current evidence on DECT in critical care. In pulmonary embolism, DECT provides anato

Bone Marrow Edema on Dual-energy Computed Tomography: Spectrum of Causes and Pitfalls.

Bone marrow edema (BME) is a frequent but nonspecific imaging finding encountered in a wide range of musculoskeletal disorders, including fractures, infection, inflammatory arthritis, and neoplasia. While MR imaging is considered the reference standard for detection of BME, dual-energy computed tomography (DECT) offers a valuable alternative, particularly in emergent settings or when MR imaging is contraindicated. This article outlines the principles and acquisition strategies of DECT, including dual-source, rapid kilovoltage peak switching, and dual-layer spectral detector techniques. Integra

Virtual noncontrast images reveal gouty tophi in contrast-enhanced dual-energy CT: a phantom study.

BACKGROUND: Dual-energy computed tomography (DECT) is useful for detecting gouty tophi. While iodinated contrast media (ICM) might enhance the detection of monosodium urate crystals (MSU), higher iodine concentrations hamper their detection. Calculating virtual noncontrast (VNC) images might improve the detection of enhancing tophi. The aim of this study was to evaluate MSU detection with VNC images from DECT acquisitions in phantoms, compared against the results with standard DECT reconstructions. METHODS: A grid-like and a biophantom with 25 suspensions containing different concentrations o

A Review of the Applications of Dual-Energy CT in Acute Neuroimaging.

Dual-energy computed tomography (CT) is a promising tool with increasing availability and multiple emerging and established clinical applications in neuroradiology. With its ability to allow characterization of materials based on their differential attenuation when imaged at two different energy levels, dual-energy CT can help identify the composition of brain, neck, and spinal components. Virtual monoenergetic imaging allows a range of simulated single energy-level reconstructions to be created with postprocessing. Low-energy reconstructions can aid identification of edema, ischemia, and subt
