An interactive treatment planning system for ophthalmic plaque radiotherapy.

Brachytherapy using removable episcleral plaques containing sealed radioisotope sources is being studied as an alternative to enucleation in the treatment of choroidal melanoma and other tumors of the eye. Encouraging early results have been reported, but late complications which lead to loss of vision continue to be a problem. A randomized national study, the Collaborative Ocular Melanoma Study (COMS) is currently in progress to evaluate the procedure. The COMS specified isotope is 125I. Precise dosimetric calculations near the plaque may correlate strongly with complications and could also b

Potential gains: Comparison of a mono-isocentric three-dimensional conformal radiotherapy (3D-CRT) planning technique to hybrid intensity-modulated radiotherapy (hIMRT) to the whole breast and supraclavicular fossa (SCF) region.

INTRODUCTION: Until late 2018, standard of practice at the Northern Sydney Cancer Centre (NSCC) for breast and nodal treatment was a conformal mono-isocentric technique. A planning study comparing an existing mono-isocentric three-dimensional conformal radiotherapy (3D-CRT) planning technique to a hybrid intensity-modulated radiotherapy (hIMRT) approach for the whole breast and supraclavicular fossa (SCF) region was undertaken with the aim to improve plan quality by improving dose conformity/homogeneity across target volumes and reducing hotspots outside the target. METHODS: A cohort of 17 pa

Applicability and usage of dose mapping/accumulation in radiotherapy.

Dose mapping/accumulation (DMA) is a topic in radiotherapy (RT) for years, but has not yet found its widespread way into clinical RT routine. During the ESTRO Physics workshop 2021 on "commissioning and quality assurance of deformable image registration (DIR) for current and future RT applications", we built a working group on DMA from which we present the results of our discussions in this article. Our aim in this manuscript is to shed light on the current situation of DMA in RT and to highlight the issues that hinder consciously integrating it into clinical RT routine. As a first outcome of

Digitally Reconstructed Radiograph Dosimetry: Introducing a New Application of Digitally Reconstructed Radiographs for Evaluation of Dose Distribution in Radiation Therapy.

PURPOSE: The digitally reconstructed radiograph (DRR) has traditionally been used for geometric radiation therapy (RT) treatment verification, with no reported dosimetric applications to date. This study aimed to introduce a methodology for the first time, allowing us to derive a dose distribution within an RT treatment plan using the DRR image. METHODS AND MATERIALS: Initially, the correlation between the pixel values of the DRR image and the depth of the water phantom was established. Subsequently, the relationship between depth and absorbed dose was derived. By combining these 2 equations,

The dose accumulation and the impact of deformable image registration on dose reporting parameters in a moving patient undergoing proton radiotherapy.

INTRODUCTION: Potential changes in patient anatomy during proton radiotherapy may lead to a deviation of the delivered dose. A dose estimate can be computed through a deformable image registration (DIR) driven dose accumulation. The present study evaluates the accumulated dose uncertainties in a patient subject to an inadvertent breathing associated motion. MATERIALS AND METHODS: A virtual lung tumour was inserted into a pair of single participant landmark annotated computed tomography images depicting opposite breathing phases, with the deep inspiration breath-hold the planning reference and

Online adaptive radiotherapy: Clinical utility, challenges and perspectives.

Online adaptive radiotherapy (oART) represents a major evolution in radiation oncology, enabling daily plan adaptation to account for anatomical variations during treatment. This review synthesizes current evidence on its clinical utility, implementation challenges, and future directions. A comprehensive PubMed search (2014-June 2025) identified studies reporting dosimetric and clinical outcomes for oART across tumor sites. Delivered via magnetic resonance- or computed tomography-guided systems, oART consistently improved target coverage while reducing exposure to organs at risk across multipl

Extracting the gradient component of the gamma index using the Lie derivative method.

Objective. The gamma index (γ) has been extensively investigated in the medical physics and applied in clinical practice. However,γhas a significant limitation when used to evaluate the dose-gradient region, leading to inconveniences, particularly in stereotactic radiotherapy (SRT). This study proposes a novel evaluation method combined withγto extract clinically problematic dose-gradient regions caused by irradiation including certain errors.Approach. A flow-vector field in the dose distribution is obtained when the dose is considered a scalar potential. Using the Lie derivative from differen

Effect of fluence smoothing on the quality of intensity-modulated radiation treatment plans.

A fluence-smoothing function applied for reducing the complexity of a treatment plan is an optional requirement in the inverse planning optimization algorithm of intensity-modulated radiation therapy (IMRT). In this study, we investigated the consequences of fluence smoothing on the quality of highly complex and inhomogeneous plans in a treatment-planning system, Eclipse™. The smoothing function was applied both in the direction of leaf travel (X) and perpendicular to leaf travel (Y). Twenty IMRT plans from patients with cancer of the nasopharynx and lung were selected and re-optimized with us

Dependence of planning target volume margins on degrees of freedom and correlations of uncertainties.

BACKGROUND: The aim of this paper is to describe and extend the analytical framework from the original van Herk margin recipe to deal with more complex situations, which are typical in the current era where accuracy in radiotherapy delivery has dramatically improved. This new framework deals with movement ranging from rigid motion to multiple independent CTVs, deformable CTVs, and delineation variation. METHODS: Margins for random errors could be described by dose blurring using the local uncertainty. Margins for systematic errors depend on the number of CTVs and their motion correlation, and

Isodose surface differences: A novel tool for the comparison of dose distributions.

BACKGROUND: Comparing dose distributions is a routine task in radiotherapy, mainly in patient-specific quality assurance (PSQA). Currently, the evaluation of the dose distributions is being performed mainly with statistical methods, which could underestimate the clinical importance of the spotted differences, as per the literature. PURPOSE: This paper aims to provide proof-of-concept for a novel dose distribution comparison method based on the difference of the isodose surfaces. The new method connects acceptance tolerance to QA limitations (equipment capabilities) and integrates a clinical a

Characterization of the output for helical delivery of intensity modulated slit beams.

The UW tomotherapy workbench utilizes a convolution/superposition based dose calculation and optimization program. It specifies the energy fluence that must be delivered from each leaf for each phantom projection angle. This requires that the spectrum of the radiation emitted from the one-dimensional MLC (multileaf collimator) attached to the linear accelerator be determined. The steps involved in that process are described. The spectrum along the central axis of the slit beam was determined, as well as the softening with off-axis position. Moreover, the magnitude of the energy-fluence output

Scanned particle-beam tracking with beam correction based on predictive volumetric imaging: A simulation study.

BACKGROUND: Tracking irradiation to moving targets in spot-scanning particle therapy, which corrects the spot position and energy in real-time, may decrease treatment time and increase accuracy. However, because of the temporal performance of the system, clinical translation remains challenging. Processing time, including image acquisition, volumetric image synthesis, correction assessment, and system response, is required to control the actual treatment system. These processing delays cause millimeter-order discrepancies due to tumor motion. Predicting future states may compensate for this la

The conversion of electroanatomic maps for compatibility with treatment planning systems in cardiac radioablation target volume definition.

BACKGROUND: Cardiac radioablation (CRA) is a new and promising treatment modality for patients with ventricular tachycardia refractory to standard-of-care treatment. Electroanatomic maps are used to define radiation target volumes; however, there is currently no native method to import electroanatomic maps into the treatment planning system (TPS). PURPOSE: To develop Edico, a semi-automated tool to enable electroanatomic map import into a TPS, by converting electroanatomic maps to a Digital Imaging and COmmunications in Medicine (DICOM) standard. The overall aim is to facilitate target volume

Optimal Target Volume Delineation for Pancreatic Cancer: Triangulating What Should be Targeted.

Optimal target volume delineation for pancreatic cancer remains an unanswered question across all stages of disease. To-date, target volume delineation has been highly variable across major clinical trials that have explored the role of radiation for this disease, which undoubtedly has contributed to the significant variation in outcomes that have been observed between such trials and the current controversy that has therefore been generated surrounding the role of radiation for pancreatic cancer. Importantly, emerging data has increasingly suggested that elective volume targeting is critical

Evaluation of the small-field output factor in eclipse modeling methods using representative beam and measured data with averaged ionization chamber and diode detector measurements.

Beam modeling for radiotherapy treatment planning systems (RTPS) can be performed using representative beam data (RBD) or direct measurements. However, RBD typically excludes output factor (OPF) measurements for fields smaller than 3 × 3 cm2. The Eclipse treatment planning system addresses this limitation by incorporating measured OPF data for fields as small as 1 × 1 cm2. Although existing studies have primarily examined the accuracy of small-field OPFs for plastic scintillator detectors, studies directly comparing the OPF values obtained through RBD modeling with and without OPF measurements

The integration of electroanatomic maps into cardiac radioablation treatment planning: A systematic review.

BACKGROUND: Ventricular tachycardia is a life-threatening cardiac arrhythmia for which radiation therapy is an emerging therapeutic option. Electroanatomic maps (EAMs) are used to define clinical target volumes (CTVs) in cardiac radioablation (CRA) treatment planning. Treatment planning systems are unable to integrate EAM data, thus many different workflows have been developed to guide clinicians in CTV creation. PURPOSE: To provide a review of existing CTV definition protocols involving EAM integration for CRA. METHODS: PubMed was searched on January 11, 2024, using appropriate search terms
