[Distal radius fractures in children].

Metaphyseal and physeal fractures of the distal radius are common in children. Most cases are best treated with closed reduction and cast immobilization. Long-term outcomes of these injuries are excellent when specific treatment principles of reduction and casting are followed. Surgical indications are limited and include open fractures, intra-articular fractures, non-reducible fractures, unstable fractures, and the presence of associated nerve injury. Closed reduction and percutaneous pin fixation is the most commonly used surgical option. The clinician should be aware of delayed complication

Fractures of the distal radius.

Distal radius fractures represent 1 of every 6 fractures treated in emergency departments (EDs) each year. This injury has a peak incidence within both the 5- to 24-year-old age group and the elderly female population aged 65 years and older. An understanding of the functional anatomy of the wrist fracture will greatly aid the practitioner in proper treatment and referral for these injuries. After obtaining wrist radiographs, a thorough examination must be performed. Once anesthesia is achieved (via hematoma block, sedation, or Bier block), the first line of treatment of distal radius fracture

Distal radius fractures: current concepts.

Despite the frequency of distal radius fractures, the optimal treatment remains without consensus opinion. A trend toward increased distal radius fracture open reduction and internal fixation has been identified, with biomechanical and clinical studies suggesting treatment advantages of certain fixation methods over others. Well-controlled patient trials are still missing to lend objective findings to management algorithms. This article reviews the literature over the past 5 years to guide our management regarding this common upper-extremity injury.

Complications of distal radius fractures.

Fractures occur at the distal end of the radius more frequently than at any other location. The reported complication rates of distal radius fractures in the literature vary from 6% to 80%. Complications may occur from the fracture or its treatment. This article reviews complications caused by distal radius fractures and their treatment. Complications are divided chronologically into immediate, early (less than 6 weeks), and late (greater than 6 weeks).

Distal radius fractures: current concepts.

Despite the frequency of distal radius fractures, studies in the existing literature have not been able to determine the optimal surgical strategies for various fracture patterns. Numerous clinical articles have been written, but most are level IV case series or expert opinion reviews. Good biomechanics studies have been published that suggest advantages of certain fixation methods over others. Transference of these expectations to clinical reality, however, requires well-controlled patient trials. In large part, this has not happened. This article reviews the theoretical pros and cons of diff

Nonunion of distal radius fractures.

Nonunion of a distal radius fracture is extremely uncommon. Healing problems in the distal radius seem to be related to unstable situations, such as concomitant fracture of the distal radius and ulna, and to an inadequate period of immobilization. Nonunion should be suspected if there is continuing pain after remobilization of the wrist in combination with a progressing deformity. The diagnosis may be confirmed by showing movement at the fracture site on lateral radiographs of the wrist in flexion and extension. Because of the rarity of distal radius fracture nonunion, it is not surprising tha

[Fracture of the distal radius].

Distal fracture of the radius is an injury to a complex joint that is comprised functionally of four partial joints and makes it possible for the hand to move in all directions. The injuries to bone and cartilage and to the stabilizing ligamentous structures and the surrounding soft tissue vary as functions of the impact responsible the mechanism of injury, and any previous illnesses. The objectives of treatment are restoration of pain-free, unrestricted and lasting function of the wrist and lower arm and avoidance of the typical complications. Stable fractures are treated by conservative mean

[Compression/flexion fractures of the radius base (Smith fractures)].

Out of 416 Colles-fractures we observed 18 of the Smith-type. The Smith fracture is a rare injury, it will generally occur in consequence of a fall on the supinated hand which is forced in pronation. The dislocation of the carpus and peripheral fragment is in the volar and proximal direction. It should be treated operatively, especially in intraarticular fractures. Surgical treatment of patients will result in better movability of he wrist and intact distal joint surfaces.

Distal radius instability and stiffness: common complications of distal radius fractures.

Emphasis in the literature over the past hundred years regarding distal radius fracture management has been on restoration of anatomic radio-carpal alignment. Until the two most recent decades, little emphasis has been placed on the great morbidity and compromise to upper limb function associated with distal radio-ulna joint (DRUJ) pathology occurring with fractures of the distal radius. This article emphasizes that attention to restoration of anatomy of the DRUJ should be considered at least as important as that given to the radio-carpal relationship. This article also points out how stiffnes

ESTES recommendations regarding distal radius fractures.

Distal radius fractures (DRFs) account for 15% of all fractures in adults. Optimal treatment of DRFs depend on patient and fracture characteristics. Non- or minimally displaced DRFs can be treated nonoperatively with a cast. Although surgical treatment is becoming more popular, the majority of patients with a DRF are still treated nonoperatively. In addition, these considerations may be different for elderly patients. Conflicting evidence exists on the different aspects of treatment of DRFs. These recommendations regarding distal radius fractures describe considerations regarding diagnostic mo

[Conservative management of distal radius fractures].

Distal radius fractures are the third most frequent fractures in Germany. The decision for conservative treatment and the weighing up of surgical treatment need an exact consideration of the indications based on instability criteria and the extent of possible articular involvement. Indications for an emergency operation must be excluded. In cases of stable fractures or multimorbid patients in a poor general condition conservative treatment is indicated. The principles for a successful treatment are the precise reduction and stable retention in a plaster splint. In the further course, fractures

Avoiding Complications in Distal Radius Fractures.

Managing fractures of the distal radius is a fundamental skill for orthopaedic surgeons. Given the prevalence of these fractures, complications following operative management are well described and frequently encountered. Surgeons should be observant in the acute phase for emergent conditions such as acute carpal tunnel syndrome. Careful radiographic examination intra-operatively can help prevent delayed complications by identifying surgical errors such as hardware malposition or malreduction. Many problems that arise during the treatment of distal radius fractures are the result of technical

Distal Radius Fractures in the Athlete.

Although the technical details of distal radius fracture fixation in athletes are largely similar to the general population, the issues surrounding the injury, desire to return to sport, and rehabilitation require specialized attention. Athletes are generally healthy, with a drive to recover and must balance the risk of long-term consequences of returning to play too early with the potential loss of scholarship, salary, or opportunities for advancement. Outcomes after nonoperative and operative treatment of distal radius fractures are generally excellent in athletes and return to the same leve

Perioperative management of distal radius fractures.

Fractures of the distal radius are the most common upper limb fracture and account for over a sixth of all fractures seen in emergency departments. Although most of these fractures are managed non-operatively, a significant number of complex injuries undergo operative management. This educational review of up to date guidelines discusses the perioperative management of distal radius fractures and provides readers with continuing professional development activities.
