Proton beam re-irradiation for gastrointestinal malignancies: a systematic review.

BACKGROUND: Radiotherapy (RT) is part of the standard of care management of most gastrointestinal (GI) cancers. Even with advanced RT, systemic therapy, and surgical techniques, locoregional recurrences or second primary cancers can still occur within previously irradiated fields, which can present challenges in delivering effective and safe treatment. Options for reirradiation are often limited, but given the favorable dosimetric aspects of proton-beam RT, it may provide an effective and safe re-irradiation option for patients with recurrent or second primary GI cancers. METHODS: We conducte

Salvage re-irradiation in non-melanoma skin cancers: A multicenter analysis.

BACKGROUND AND PURPOSE: We conducted a multicentre real-world study to assess the outcomes of radical salvage re-irradiation for non-melanoma skin cancer (nMSC) recurrences following definitive or postoperative radiotherapy. MATERIALS AND METHODS: Data on patients treated between 2006 and 2022 with re-irradiation for nMSCs were retrospectively collected from five high-volume brachytherapy centers. The primary endpoint was local control (LC). Secondary endpoints included overall survival, progression-free survival, and adverse events (AEs). The Kaplan-Meier estimator and Cox Proportional-Hazar

Re-irradiation of Vertebral Body Metastases: Treatment in the Radiosurgery Era.

Vertebral bodies remain one of the most common sites of metastases. In cases where surgical intervention is not indicated or appropriate, conventional external radiation therapy (cEBRT) has been the standard treatment modality. Unfortunately, cEBRT is typically limited, with low complete response and poor local control rates. Disappointing results with re-irradiation using cEBRT highlight the need for innovative salvage therapeutic strategies, such as stereotactic body radiotherapy. A detailed description of this complex treatment strategy is outlined, as is a systematic review of current lite

Re-irradiation of metastatic disease in the neck from xeroderma pigmentosum.

BACKGROUND: Xeroderma pigmentosum, an autosomal recessive disease that occurs with a frequency of 1:250,000, is caused by a genetic defect in nucleotide excision repair enzymes. Mutation of these enzymes leads to the development of multiple basal cell and squamous cell carcinomas. OBJECTIVES: We present a case of xeroderma pigmentosum in a patient with cervical and intraparotid metastatic disease from recurrent cutaneous squamous cell carcinomas of the face and scalp, treated with neck dissection and re-irradiation. With the illustrative case report, we include a literature review of diagnosi

High dose 3-dimensional re-irradiation for locally recurrent nasopharyngeal cancer.

This is to report the results of 3-dimensional (3D) high dose re-irradiation (re-RT) for patients with locally recurrent nasopharyngeal cancer. Between May 1995 and Dec. 2000, 21 patients with locally recurrent cancer of the nasopharynx received high dose 3D re-RT at Samsung Medical Center. The median 55 (45 - 70) Gy was applied by daily fractions of 2.5 Gy or 3.0 Gy. The median survival period, the rates of local control, overall survival and disease-free survival at 5 years, of all patients, were 21 months, 71.8%, 32.3%, and 21.2% respectively. The number of patients who experienced treatmen

Clinical approach to re-irradiation for recurrent diffuse intrinsic pontine glioma.

BACKGROUND: We present our institutional approach for re-irradiation in diffuse intrinsic pontine glioma and their outcomes. METHODS: Consecutive patients of recurrent diffuse intrinsic pontine glioma treated with re-irradiation (January 2015-September 2019) were reviewed retrospectively to describe the clinical-response-based approach followed for the dose and volume decision. Outcomes were defined with clinical and steroid response criteria and survival endpoints included progression-free survival and overall survival as cumulative(c) overall survival and re-irradiation overall survival (re

Re-Irradiation of Hepatocellular Carcinoma: Clinical Applicability of Deformable Image Registration.

PURPOSE: This study aimed to evaluate whether the deformable image registration (DIR) method is clinically applicable to the safe delivery of re-irradiation in hepatocellular carcinoma (HCC). MATERIALS AND METHODS: Between August 2010 and March 2012, 12 eligible HCC patients received re-irradiation using helical tomotherapy. The median total prescribed radiation doses at first irradiation and re-irradiation were 50 Gy (range, 36-60 Gy) and 50 Gy (range, 36-58.42 Gy), respectively. Most re-irradiation therapies (11 of 12) were administered to previously irradiated or marginal areas. Dose summa

Re-irradiation for recurrent high-grade glioma: current evidence and practical considerations.

Re-irradiation is an increasingly important salvage option for patients with recurrent high-grade glioma and glioblastoma, but its use is complicated by limited prospective evidence, heterogeneous treatment approaches, and concern for cumulative toxicity to the brain and critical neurologic structures. Available studies suggest that carefully selected patients may achieve meaningful local control, symptom relief, and preservation of neurologic function, although a consistent overall survival benefit has not been demonstrated. Treatment decisions therefore require individualized assessment of p

Fractionated stereotactic re-irradiation after plaque radiotherapy (brachytherapy) failure in uveal melanoma.

PURPOSE: Local recurrence after ruthenium-106 (106Ru) interventional radiotherapy (IRT) for uveal melanoma remains a therapeutic challenge. This study aimed to evaluate the efficacy and safety of fractionated stereotactic radiotherapy (fSRT) used as a salvage, globe-preserving option after failed IRT. MATERIAL AND METHODS: Patients with locally recurrent uveal melanoma previously treated with 106Ru brachytherapy were retrospectively analyzed. All patients underwent transpupillary thermotherapy as first-line salvage treatment, followed by fSRT delivered with CyberKnife. Follow-up examinations

Spatially fractionated radiotherapy for re-irradiation: feasibility, safety, treatment planning, and outcomes.

Spatially fractionated radiotherapy (SFRT) is emerging as an option to deliver high dose radiation to bulky disease sites with an improved therapeutic ratio compared to established radiotherapy techniques. SFRT has not been previously studied for re-irradiation. We describe three patients with previously irradiated bulky oligometastatic disease to different sites (lung, thigh, and adrenal gland) who were re-treated with SFRT, with intention to relieve or prevent worsening of symptoms. Dose prescription was 66.7 Gy in 5 fractions to uniformly spaced lattice vertices in the gross tumour volume (

Re-irradiation practice and ESTRO/EORTC consensus recommendations: 2023 ASTRO education panel.

Indications for re-irradiation are increasing both for palliation and potentially curative attempts to achieve durable local control. This has been in part driven by the technological advances in the last decade including image-guided brachytherapy, volumetric-modulated arc therapy and stereotactic body radiotherapy. These enable high dose focal irradiation to be delivered to a limited target volume with minimal normal tissue re-irradiation. The European Society for Radiotherapy and Oncology (ESTRO) and the European Organisation for Research and Treatment of Cancer (EORTC) have collaboratively

Re-irradiation for painful heel spur syndrome. Retrospective analysis of 101 heels.

PURPOSE: Painful heel spur syndrome is a common disease with a lifetime prevalence of approximately 10 %. One of the most effective treatment options is radiotherapy. Many authors recommend a second or third series of radiation for recurrent pain and partial or no response to the initial treatment. As the results of re-irradiation have not been systematically analyzed the aim of this study was to document the results of repeated radiation treatment and to identify patients who could benefit from this treatment. MATERIAL AND METHODS: The analysis was performed on patients from 2 German radioth

Association Between Previous Radiotherapy Response and Pain Response After Palliative Re-irradiation.

PURPOSE: Pain response to palliative radiotherapy is often temporary; some require re-irradiation. Whether previous radiotherapy response is associated with response to re-irradiation within the same lesions remains unclear. This study compared pain response rates to palliative re-irradiation between patients with previous response and recurrent pain in the same lesion ("previous responders") and those with persistent or progressive pain after non-response to previous radiotherapy ("previous non-responders"). METHODS AND MATERIALS: This retrospective lesion-level analysis was conducted using

Unravelling Salvage Re-irradiation for Locally Recurrent Prostate Cancer: Challenges and Complexities.

The management of locally radiorecurrent prostate cancer after radical treatment with external beam radiotherapy (EBRT) presents challenges due to limited comparative data on salvage modalities and concerns regarding genitourinary and gastrointestinal toxicity. Patient selection for salvage therapy requires careful consideration of long-term toxicity, the metastatic disease development, and life expectancy. While androgen deprivation treatment (ADT) is commonly used, it can negatively impact patients' quality of life. Local salvage treatments such as salvage prostatectomy, reirradiation with b

Re-Irradiation in Pediatric Diffuse Midline Glioma: A Multi-Institutional Retrospective Study.

BACKGROUND: Children with recurrent diffuse midline gliomas (DMGs) have limited therapeutic options at recurrence. Re-irradiation (RT2) may be used at progression, but with uncertainty about the benefit. METHODS: We conducted a multi-institutional retrospective study of children aged < 18 with DMG treated at three centers (Toronto, Canada; Birmingham, UK; Nottingham, UK). Patients with radiologic progression after the first radiation (RT1) were included. Overall survival (OS) was calculated from the date of progression after RT1. Survival was analyzed using the Kaplan-Meier method. Multivaria

Re-irradiation in oligorecurrence and oligometastatic cervical cancer in modern radiotherapy era.

BACKGROUND: Recurrent cervical cancer remains a therapeutic challenge despite advances in primary treatment. The emerging paradigm of oligorecurrence and oligometastasis has opened avenues for curative-intent local therapies, including re-irradiation. Modern radiotherapy techniques have enabled high-dose delivery with acceptable toxicity. This study aims to assess clinical outcomes and treatment-related toxicities in patients with oligorecurrent or oligometastatic cervical cancer treated with modern re-irradiation techniques. METHODS: This retrospective study included 20 cervical cancer patie
