Several converging lines of contemporary evidence suggest that weakness presents a more serious compromise to movement function in poststroke hemiplegia than spasticity. This review examines the clinical and functional phenomena of weakness in poststroke hemiplegia, currently available evidence identifying physiologic substrates contributing to weakness, and reports of early investigations involving high-resistance training targeted at improving strength and the transfer of strength to improvements in functional capacity. Based on this information, we describe some unsolved problems and indica

Understanding flexibility in the neural control of movement requires identifying the distribution of common inputs to the motor units. In this study, we identified large samples of motor units from two lower limb muscles: the vastus lateralis (VL; up to 60 motor units per participant) and the gastrocnemius medialis (GM; up to 67 motor units per participant). First, we applied a linear dimensionality reduction method to assess the dimensionality of the manifolds underlying the motor unit activity. We subsequently investigated the flexibility in motor unit control under two conditions: sinusoida

Voluntary contraction anomalies of poststroke survivors progress from flaccid paralysis to recovery of upper extremity motor function in the subacute phase. However, muscle weakness often persists, and it is unclear what changes or aberrations persist in neuromuscular function, particularly in motor unit behavior. Our objective was to characterize motor unit discharge behavior in patients with hemiplegic stroke in the subacute phase. We tested seven patients with subacute stroke at two timepoints (timepoints 1 and 2), a minimum of 2 wk apart during the subacute phase. We used wireless surface

BACKGROUND: Lower activation of higher threshold (type-II) motor units (MUs) has been suggested in children compared with adults. We examined child-adult differences in discrete MU activation of the flexor carpi radialis (FCR).
METHODS: Fifteen boys (10.2 ± 1.4 years), and 17 men (25.0 ± 2.7 years) completed 2 laboratory sessions. Following a habituation session, maximal voluntary isometric wrist flexion torque (MVIC) was determined before completing trapezoidal isometric contractions at 70%MVIC. Surface electromyography was captured by Delsys Trigno Galileo sensors and decomposed into indivi

KEY POINTS: The mechanisms affecting recruitment patterns of postganglionic sympathetic nerves remain unclear. The divergent and convergent preganglionic innervation patterns of postganglionic neurons and the presence of differently sized postganglionic nerves suggest that the ganglia may participate in modifying the discharge patterns of single sympathetic postganglionic neurons innervating the skeletal muscle circulation. Whether the ganglia affect the ordered behaviour of varying sized postganglionic sympathetic neurons in humans has not been studied. Trimethaphan infusion produced an order

Feedback from sensory elements as well as projection from higher Central Nervous System structures modify the level and pattern of motor outflow to the respiratory muscles and hence ventilation. In this review we describe the different methods to evaluate the degree to which higher centers determine the level and pattern of ventilation and coordinate use of the respiratory muscles in healthy humans and in patients with a number of respiratory disorders.

OBJECTIVES: To examine the anticipatory effect on lower extremity muscle activation during single leg drop landing and jumping.
METHODS: Seventeen healthy males performed single leg drop landing and jumping, under conditions manipulating the subsequent movement after landing. Participants performed the anticipated condition (subsequent movement-land/jump-was preplanned), followed by the unanticipated condition (subsequent movement was given via visual signals). Electromyography of the gluteus medius (GM), rectus femoris (RF), vastus medialis (VM), semitendinosus (ST), biceps femoris (BF), tib

The purpose of our study was to determine the influence of load compliance on the discharge characteristics of the same motor units in the first dorsal interosseus (FDI) muscle during submaximal isometric contractions. Motor unit activity in FDI was recorded using high-density, surface electromyography while participants performed isometric contractions either by pushing against a rigid force transducer (force task) or supporting an equivalent inertial load (position task). The coefficient of variation for force decreased as the target force increased from 10% to 30% of maximal FDI force (inde

Acute pain alters motor unit discharge properties in muscles that are painful or influence loading of painful structures. Less is known about the changes in discharge when pain is induced in distant tissues that are unable or have limited capacity to modify the load of the contracting muscle. We aimed to determine whether acute experimental pain alters quadriceps motor unit discharge when pain is induced in; (i) a muscle that is unlikely to be mechanically influenced by modified quadriceps activity (tibialis anterior: TA), or (ii) the antagonist muscle (biceps femoris: BF). This review is a comprehensive description of all muscles that assist lung inflation or deflation in any way. The developmental origin, anatomical orientation, mechanical action, innervation, and pattern of activation are described for each respiratory muscle fulfilling this broad definition. In addition, the circumstances in which each muscle is called upon to assist ventilation are discussed. The number of "respiratory" muscles is large, and the coordination of respiratory muscles with "nonrespiratory" muscles and in nonrespiratory activities is complex-commensurate with the diversity of act

AIMS: The external anal sphincter (EAS) is essential for maintaining fecal continence. Neurological disorders or traumatic injuries to muscle and nervous systems could lead to EAS denervation. Currently, there are no techniques available to document global innervation changes in the EAS in vivo. The aim of this study was to develop a novel approach to non-invasively estimate the number of functioning motor units (MUs) in the EAS and validate with immunofluorescent techniques in rats.
METHODS: Intra-rectal surface electromyography (EMG) signals of the EAS, induced by a series of intra-vaginall

Our objective was to establish the usefulness of the noninvasive method of the motor unit number index (MUNIX) in a large muscle and to study how macro electromyography (EMG) and MUNIX complement each other in describing the motor units (MUs) in prior polio. MUNIX and macro EMG were performed in 48 tibialis anterior muscles in 33 prior polio patients. In addition, the reproducibility of MUNIX was investigated. It is shown that MUNIX can be used to characterize MUs with high reproducibility, even in a large muscle. As judged by MUNIX values, the patients had a 25% reduction of motor neurons, wh

This chapter summarizes experiments which were designed to reveal how repetitive electrical stimulation of the mesencephalic locomotor region (MLR) recruits nearby hindbrain neurons into activity, such that locomotion can ensue in the tiger salamander, A. tigrinum. The MLR stimulus strength was subthreshold or near-threshold for locomotor movements to ensue. Such relatively weak stimulation of the MLR produced locomotor movements after a relatively long delay, which featured neuronal interactions in the hindbrain. MLR-evoked spike responses of single hindbrain neurons were recorded before loco

Persons with spinal cord injury (SCI) can benefit significantly from functional neuromuscular stimulation (FNS) systems for standing if manual tasks can be performed while upright. Using FNS to sufficiently activate the knee extensors to rise from a sitting position often results in inadvertent activation of the rectus femoris and/or sartorius, which flex the hip. In this study, intramuscular electrodes implanted in the vastus lateralis and medialis of four subjects with SCI were used to activate these muscles individually and simultaneously to measure knee extension moment. Support forces app

Spindles are a thalamocortical rhythm entirely dependent upon a thalamic mechanism. Generators of cortical spindle waves are the pyramidal neurons of the cortex. A positive statistical relationship exists between postsynaptic excitatory events of these neurons and spindle waves, but there is no significant association with inhibitory postsynaptic events. The excitatory synaptic coupling between thalamocortical fibers and cortical neurons implicated in the genesis of spindles is weak, but increases considerably after i.m. injection of penicillin which induces a transformation of spindles into g

BACKGROUND: High-density surface electromyography (HD-sEMG) has enabled non-invasive analysis of motor unit (MU) activity and recruitment, but its application to swallowing-related muscles is limited.
OBJECTIVE: We aimed to investigate the utility of HD-sEMG for quantitatively evaluating the MU recruitment characteristics of the suprahyoid muscles during tongue elevation.
METHODS: We measured the sEMG activity of the suprahyoid muscles of healthy participants during tongue elevation using HD-sEMG. Maximum voluntary contraction (MVC) was measured, followed by data collection during sustained

At high forces, the discharge rates of lower- and higher-threshold motor units (MU) are influenced in a different way by muscle pain. These differential effects may be particularly important for performing contractions at different speeds since the proportion of lower- and higher-threshold MUs recruited varies with contraction velocity. We investigated whether MU discharge and recruitment strategies are differentially affected by pain depending on their recruitment threshold (RT), across a range of contraction speeds. Participants performed ankle dorsiflexion sinusoidal-isometric contractions

The forces generated by the cat diaphragm (DIA) during different ventilatory and nonventilatory behaviors were determined by measuring transdiaphragmatic pressures (Pdi). The Pdi generated during eupnea was only approximately 12% of the maximum Pdi (Pdimax) generated by bilateral phrenic nerve stimulation. When the animals breathed a gas mixture of 10% O2 and 5% CO2, the Pdi increased to approximately 28% of Pdimax. During total airway occlusion, the Pdi generated by the diaphragm increased to approximately 49% of Pdimax. Only during the gag reflex and sneezing did Pdi reach maximal levels. A
