[Binaural summation of the ipsilateral and contralateral acoustic stapedius reflex (author's transl)].

The binaural summation of the simultaneous contra- and ipsilateral acoustic stapedius reflexes was examined in 20 normal-hearing patients. In these patients, the effect of summation was most pronounced with subthreshold intensities. A contralateral reflex was increased by ipsilateral intensities 8 db below threshold. A reflex could also be elicited by binaural intensities which were contra- and ipsilaterally 4 db below threshold. As a result of our studies, we recommend a revision of the definition of a positive Metz-recruitment. Because of possible binaural summation effects, the absolute val

Binaural processing after corrected congenital unilateral conductive hearing loss.

Binaural processing was measured in a series of tests in patients before and after surgery to correct congenital unilateral conductive hearing losses. Data are presented from 19 patients between the ages of 6 and 33 years that had an abnormal external and/or middle ear on one side but normal hearing in the other ear. Surgery improved thresholds an average of 36 dB HL (from 56 to 20 dB HL). Patients were tested pre- and postoperatively for interaural temporal difference limens, alternate and simultaneous loudness balances, sound localization, binaural detection thresholds, and speech perception

Vestibular recruitment in Meniere's disease.

The aim of this study was to search for vestibular recruitment in a sample of patients with Meniere's disease. Recruitment was defined as an abnormal growth of response with increasing stimulus intensity. Twenty-nine patients were tested with sinusoidal rotation of three different magnitudes at four different frequencies. We also searched for auditory recruitment in each patient via tests of auditory brain stem responses, acoustic reflexes, and loudness balance and discomfort level. Analysis of vestibular responses indicated, on average, a linear relationship between stimulus magnitude and res

Comparison of measures of frequency resolution and recruitment in patients undergoing neuro-otological investigation.

Loudness recruitment and reduced frequency resolution both occur in cochlear types of hearing loss. One theory of loudness recruitment suggests that, as intensity is coded partly by spread of excitation across the nerve fibre array, recruitment is a direct consequence of the broad spread of excitation associated with poor frequency resolution. The present study investigated the relationship between these two quantities. The study involved a simple measure of frequency resolution (three-point psychoacoustical tuning curve, PTC) and conventional measures of recruitment obtained from patients und

Spectral analysis of auditory brain stem responses in perceptive hearing loss.

The Fourier analysis of auditory brain stem responses in 45 patients aged 19 to 56 treated because of perceptive hearing loss in Cracow ORL Clinic was carried out. The group consisted of: 15 patients with sudden deafness, 15 patients with Menier's disease and 15 cases with retrocochlear hearing impairment. In cases with recruiting cochlear hearing loss considerable and significantly extension of frequency range in brain stem responses was observed. Moreover, responses induced by stimulus intensity causing recruitment are identical for both ears as far as their shape and power spectrum is conce

Neurotological studies on Meniere's disease and sudden deafness.

During the period from January 1973 to June 1975, 60 cases with definite Meniere's disease and 30 cases with sudden deafness were listed according to such criteria for the diagnosis of both diseases as determined by the Research Committee of Japan. A neurootological assessment for both diseases were performed in our series. In patients with definite Meniere's disease, hearing impairment was characterized by deficiency in detecting low- and middle-pitched tones and fluctuation of auditory threshold in low frequency range. In sudden deafness, highly impaired hearing loss, i.e., total deafness, w

Critical bandwidth in Menière's disease.

The critical bandwidth in loudness summation was estimated in 20 patients with typical Menière's disease using noise bands centered around 1 kHz. A reduction of the normal loudness difference between broad-band noise and narrow-band noise was present at all except the highest levels. Judged individually, 7 of the 20 patients appeared to have a widened critical band, but in the pooled data the size of the critical band was normal. This was the case in patients with a hearing loss less than 50 dB HL as well as in patients with a hearing loss greater than or equal to 50 dB HL. Expressed in terms

Personal results, trial of classification and standardization in clinical impedancemetry.

The author analyzes the results obtained in clinical impedance testing of 2,799 ears by tympanometry and study of the acoustico-facial reflex. Impedance terminology is discussed and classified in an attempt to bring order to the results. Important findings include the high incidence of negative middle ear pressure in older, profoundly hard-of-hearing children.

Hearing loss due to aging (presbycusis).

Presbycusis is the progressive hearing loss caused by histologic changes due to aging. Schuknecht described four histopathologic types of presbycusis and correlated these findings to audiometric data. In a retrospective study, some strict criteria were set to distinguish the "pure" cases of presbycusis in a group of 1,181 subjects, 50 years of age and older, seen in our Audiology Clinic over one year; 91 subjects were found to meet these criteria. An attempt was made to fit the pure tone threshold curves into the types described by Schuknecht and correlate these curves to speech discrimination

[Hearing and equilibrium disorders in patients with neurosyphilis].

83 patients with neurosyphilis and 12 patients with latent syphilis underwent a cochleovestibular examination. Pathological changes were present in 54% of the patients with neurosyphilis. In the group of patients with latent syphilis functional changes could only be found in 25% of the patients. The greatest number of hearing disturbances occurred in persons suffering from general paresis, 65% were affected. Usually damage occurred in both ears. Specific topical diagnosis was often not possible although the results of the stapedial reflex indicated that peripheral cochlear lesions were present

Cochleo-vestibular correlations in Meniere's disease.

The cochlear and vestibular functions were investigated in a sample of 36 patients with unilateral Meniere's disease. Caloric reactions and hearing thresholds were compared separately at several frequencies. A topodiagnostic relationship between the cochlear and vestibular function was discovered. Using four qualitative categories, a significantly high correlation was obtained between the basal hearing loss (4000 Hz) and unilateral weakness, whereas no correlation was obtained when evaluating the more apical hearing loss (lower frequencies 250-1 000 Hz). A normal caloric reaction can be reason

Detecting the recruitment phenomenon in adults using 80-Hz auditory steady-state response.

OBJECTIVES: The ASSR is used widely as an objective measurement of hearing in clinical settings because of its high frequency specificity. The recruitment phenomenon is generally detected using subjective evaluations which require direct communication with the patient. If the recruitment phenomenon can be detected with ASSR, it would facilitate diagnosis in patients with developmental disorders and infants. SUBJECTS AND METHODS: We examined 2 groups of subjects: 10 subjects with unilateral hearing impairment in whom the recruitment phenomenon was detected by the alternate binaural loudness ba

Ozone therapy and pressure-pulse therapy in Ménière's disease.

The aim of this study was to evaluate the usefulness of ozone and pressure-pulse therapies in treating Ménière's disease. Using objective otoacoustic emissions and short-increment sensitivity index (SISI) tests together with subjective anamnesis, we tested 15 patients (8 men, 7 women) who had suffered from Ménière's disease for 1-3 years and had permanent sensorineural hypoacusis; we compared results before and after treatment. We performed ozone therapy and pressure-pulse treatments simultaneously for one 10-minute session each day for 10 consecutive days. After treatment, both otoacoustic-sp

[Rupture of the membrane of the round window in stapes fixation].

A case of sudden deafness is described. Examination showed a mixed deafness on the left side, suspected to be due to fixation of the stapes and a simultaneous perilymph fistula. These lesions were proved by an exploratory tympanotomy. The interesting aspects of the case are emphasized: firstly recruitment caused by a perilymph fistula, present for 5 months without causing complete deafness; secondly improvement of hearing after surgical treatment of both pathological conditions; and finally the complete absence of vestibular signs.

[Detection of loudness recruitment in patients with retrocochlear lesions using the "Würzburger Hörfeld" loudness scaling].

BACKGROUND: The pathogenesis of hearing loss caused by cerebellopontine angle tumors such as acoustic neuromas is unknown. The lack of loudness recruitment is thought to be one of the features of retrocochlear hearing impairment. In contrast to conventional suprathreshold tests, the categorial loudness scaling using the "Würzburger Hörfeld" is a valuable tool to describe the individual perception of sound. The aim of the present study was to analyze the loudness growth rate in patients with acoustic neuroma. PATIENTS AND METHOD: Pure tone and speech audiometry as well as auditory brainstem re

[Is there noise-induced hearing loss in the low and middle tone frequencies?].

Statistical material was collected on over 1000 patients with noise-induced hearing loss (expert opinions on these patients). All those patients were excluded who showed any other cause for their hearing loss i.e. head trauma, mumps, ototoxic medication, family history of hearing disease, or other causes. 200 patients showed loss of hearing at 250 Hz and 500 Hz of 20 dB or more and at 1000 Hz a hearing loss of at least 30 dB. Several matched-pair investigations with great variability of hearing loss are given. There is substantial evidence to prove that noise may damage the lower and medium fr

Evolution of recruitment at different frequencies during the development of endolymphatic hydrops in the guinea pig.

The study of human temporal bones has identified endolymphatic hydrops as a common feature of several diseases. In particular it is systematically found in those bones removed from patients with premortem Menière's disease. Menière's disease is known to induce sensorineural pathology with recruitment, which changes with the evolution of the hearing loss, and is suspected to induce a cochlear conductive loss by a possible increase in static pressure of endolymph. Amplitude/intensity functions of sensorineural responses can reflect recruitment and/or conduction loss. Experimentally induced hydro

Verification of the air-bone gap using cancellation and evoked responses.

Using the cancellation method as well as evoked responses, we investigated their application in the estimation of the air-bone gap in patients undergoing operation on middle ear structures. The cancellation method had a closer relationship with pure-tone audiometry than evoked responses in predicting the air-bone gap. This was caused by recruitment, by which the cancellation method was not affected. The results of brainstem-evoked responses to bone and air conduction signals showed therefore an underestimation in the prediction of the air-bone gap. Combining the cancellation method with evoked
