PURPOSE OF REVIEW: As life expectancy increases, the prevalence of pelvic organ prolapse in general, and rectoceles, in particular, will continue to grow. The objectives of this article are to review the basic anatomy and contributing factors associated with the development of rectoceles and to discuss the appropriate work-up and treatment options.
RECENT FINDINGS: The main themes in the current literature stress the importance of not only anatomic restoration, but also quality of life issues regarding visceral and sexual function when performing a rectocele repair. Many recent studies are al

Rectocele is a posterior colpocele inhabited by an expansion of the anterior wall of the rectum. Its physiopathology is similar to that of other vaginal prolapse and is essentially dependent on the opening of the vulva secondary to a damage of the pelvi-perineal musculature. Its specific symptomatology is poor except for dyschesis. A colpocystogram may be helpful when there is a need to differentiate between rectocele and elytrocele. The surgical treatment of rectocele requires, most of the time, a vaginal approach with posterior colpo-perineomyorraphy. More rarely, when the rectocele is isola

OBJECTIVES: Our primary objective was to determine the association between rectocele size on defecography and physical examination in symptomatic patients. Our secondary objective was to describe the associations between both defecography and physical examination findings with defecatory symptoms and progression to surgical repair of rectocele.
METHODS: We performed a retrospective review of all patients referred to a female pelvic medicine and reconstructive surgery clinic with a diagnosis of rectocele based on defecography and/or physical examination at a single institution from 2003 to 201

Using the descriptions of Uhlenhuth, and Milley and Nichols, we have been able to identify a sheet of strong connective-type tissue between the rectum and vagina. This layer has been named the rectovaginal septum, referred to, by some, as the rectovaginal fascia. As it is considered to be analogous to the rectovesical septum in men, first described by Denovilliers, some refer to it as Denonvilliers' fascia in the female. This layer is immediately beneath the vaginal mucosa and clearly is what many would consider part of the vaginal wall. Its principal attachments are peripheral--it merges supe

INTRODUCTION: Pelvic organ prolapse is a common condition among women with a prevalence of 11% and may affect the anterior, posterior, or apical compartment with a negative impact on sexual function.
AIM: Aim of the current study was to evaluate sexual function before and after surgical rectocele fascial repair in sexually active patients who suffer from symptomatic rectoceles.
MAIN OUTCOME MEASURES: Female Sexual Function Index (FSFI) and anatomical outcome after rectocele repair.
METHODS: Between December 2000 and December 2009, we asked sexually active female patients who were to undergo

Pelvic organ prolapse repair poses a unique challenge to the reconstructive pelvic surgeon. Traditional levator plication has been promoted for the past century and has recently been shown to create unsatisfactory functional results, among them dyspareunia. In an attempt to improve both functional and quality-of-life outcomes, new methods for posterior vaginal wall defect repair have been developed and are now undergoing critical evaluation. We present a review of posterior vaginal wall repair with a focus on new anatomic concepts and site-specific rectocele repair.

BACKGROUND: Transanal repair of rectocele involving the suprasphincteric portion of the rectovaginal septum has been shown to provide excellent results in up to 90 per cent of cases. Selection of patients suitable for repair is important. Rectocele with concomitant cystocele is best repaired transvaginally. An alternative approach is recommended for enterocele.
METHODS: With the patient in the prone position and using local anaesthesia, a mucomuscular endorectal flap is raised and the underlying tissues are plicated. The excessive flap is excised, and the cut edges are approximated. A retrosp

The two types of anterior rectocele, "distension" of Type 1 rectocele (T1R) and "displacement" or Type 2 rectocele (T2R), have different anatomical, clinical and therapeutic profiles. The aim of this study was to assess anorectal function in patients with distension or displacement rectocele. Three groups of female patients and one group of healthy female subjects were studied. Both the 10 Group 1 subjects, who had been diagnosed as having T1R, and 10 Group 2 women who had been diagnosed as having T2R, were symptomatic for digital evacuation of the rectum. The 10 Group 3 females had complained

Rectocele may cause colorectal symptoms particularly difficulty in evacuation. It is readily identified on clinical examination of the perineum but the pathophysiological abnormality can easily be defined by defaecography. Between 1984 and 1988 we have operated on 16 patients who presented with difficulty in evacuation associated with rectocele. We have used a simple endo-anal repair aimed to restore a firm recto-vaginal septum. Excellent functional results were obtained in 11 patients, 4 were considerably improved and one patient had a poor result.

OBJECTIVES: Large rectoceles (>2 cm) are believed to be associated with difficulty in evacuation, constipation, rectal pain, and rectal bleeding. The aim of our study was to determine whether rectocele size is related to patient's symptoms or defecatory parameters.
METHODS: We conducted a retrospective study on data collected on patients referred to our clinic for the evaluation of evacuation disorders. All patients were questioned for constipation, fecal incontinence, and irritable bowel syndrome and were assessed with dynamic perineal ultrasonography and conventional anorectal manometry.

We describe a new technique in the surgical treatment of rectocele using a dermal allograft to augment site-specific fascial defect repair of the rectovaginal fascia. The posterior vaginal wall is opened and discrete defects in the rectovaginal fascia are repaired in a site-specific fashion using delayed absorbable suture. A second layer of support is created using a rectangular dermal allograft placed over the site-specific repair and secured to the normal anatomic attachments of the rectovaginal fascia using permanent sutures. The vagina is then closed and routine perineorrhaphy performed as

BACKGROUND: Rectocele is a common finding in patients with intractable evacuatory disorders. Although much rectocele surgery is conducted by gynecologists en passant with other forms of vaginal surgery, many reports lack appreciation of the importance of coincident anorectal symptoms, and do not report functional and clinical outcome data. The pathogenesis of rectocele is still controversial, as is the embryological and anatomical importance of the rectovaginal septum as well as recognizable defects in its integrity and its relevance in formal repair when rectocele is operated upon as the prin

INTRODUCTION: Rectocele may be associated with both chronic constipation and anal incontinence. Several different surgical procedures have been advocated for rectocele repair. The aim of the present study was to evaluate anorectal function and clinical outcome in a consecutive series of patients who underwent selected endorectal or transperineal surgery for rectocele for whom operative treatment was determined by clinical and proctographic features. Attention was paid to the cohort of rectocele patients presenting with incontinence as a leading symptom.
METHODS: Sixty consecutive patients wit

Rectocele (RC) is common in women, especially in parous women. In this study the radiologic finding of RC in 239 patients was reported. Based on its depth, RC was divided into three grades: grade I (6-15 mm), Grade II (16-30 mm) and Grade III (greater than 31 mm). Seventy out of 239 cases were surgically treated. The results were excellent or good in Grade II and III lesions, but poor in those of Grade I, especially when the depth of RC less than or equal to 10 mm. The authors believe the above grading of RC is useful in selecting patients for surgery.

A retrospective review of 64 rectocele repairs done over a four-year period was performed. The most common indication for repair was constipation. Thirty-five patients were repaired transanally, and 29 were repaired transvaginally. The overall morbidity was 34 percent, and the overall mortality was 0 percent. The most common complication was urinary retention in 12.5 percent. There was no difference in complications between techniques. Of 46 patients contacted for follow-up, 25 (54 percent) still complained of constipation, 17 (34 percent) had partial incontinence, 8 (17 percent) noted persist

OBJECTIVES: Rectocele is common in female patients. To date, there is no literature comparing outcomes of rectocele repairs in combination with other perineal surgeries. We aim to analyze perioperative morbidity and mortality as well as long-term outcome of rectocele repair in combination with other perineal surgeries (RR combination) and compare this with solo rectocele repair (solo RR).
DESIGN: The type of study was case-control. Data of patients who received solo rectocele repair or rectocele repair in combination with other perineal surgeries between January 2011 and December 2015 were id
