Clinical and Radiographic Outcomes of Metronidazole Monotherapy in Regenerative Endodontics: A Case Series.

The evolution from apexification to regenerative endodontic procedures reflects a paradigm centred on preservation of the apical stem cell niche as a prerequisite for restoration of the pulp-dentine complex. This case series explores the biological premise in 6 immature mandibular second premolars diagnosed with pulp necrosis and chronic apical pathology secondary to fractured Leong's tubercles. Management employed a tissue-preserving regenerative protocol characterised by minimal mechanical instrumentation and metronidazole monotherapy as the sole intracanal antimicrobial agent. Following can

Clinical cell-based versus cell-free regenerative endodontics: clarification of concept and term.

There is no consensus on the true meaning of clinical regenerative endodontics, and there is confusion over the concept and the term. Commonly used terms include revitalization and revascularization. The clinical methods for endodontic revitalization procedures and the tissue engineering concept differ depending on whether there is exogenous delivery of cells - called cell therapy, or not. Here, in this review, the difference is clarified by emphasizing the correct terminology: cell-free versus cell-based regenerative endodontic therapy (CF-RET versus CB-RET). The revitalization procedures pra

Epigenetic regulation of dental pulp stem cells and its potential in regenerative endodontics.

Regenerative endodontics (RE) therapy means physiologically replacing damaged pulp tissue and regaining functional dentin-pulp complex. Current clinical RE procedures recruit endogenous stem cells from the apical papilla, periodontal tissue, bone marrow and peripheral blood, with or without application of scaffolds and growth factors in the root canal space, resulting in cementum-like and bone-like tissue formation. Without the involvement of dental pulp stem cells (DPSCs), it is unlikely that functional pulp regeneration can be achieved, even though acceptable repair can be acquired. DPSCs, d

Regenerative endodontic procedure using Emdogain: a case series.

BACKGROUND: Emdogain (Straumann® Emdogain®, Basel, Switzerland) is an enamel matrix derivative and has an important role in odontogenesis, by potentializing pulp tissue repair and regeneration, as showed in biological studies. Nevertheless, the benefits of using Emdogain as a scaffold in clinical regenerative endodontic procedures has not been demonstrated and is not yet fully understood. The aim of this report was to show three clinical cases that had different preoperative conditions and underwent pulp regenerative procedures under an identical clinical protocol. This report also aimed to sh

Exosomes as Promising Therapeutic Tools for Regenerative Endodontic Therapy.

Pulpitis is a common and frequent disease in dental clinics. Although vital pulp therapy and root canal treatment can stop the progression of inflammation, they do not allow for genuine structural regeneration and functional reconstruction of the pulp-dentin complex. In recent years, with the development of tissue engineering and regenerative medicine, research on stem cell-based regenerative endodontic therapy (RET) has achieved satisfactory preliminary results, significantly enhancing its clinical translational prospects. As one of the crucial paracrine effectors, the roles and functions of

Progress and perspectives on cell-free regenerative endodontic therapy.

Although teeth with pulpitis/apical periodontitis are saved after successful endodontic therapy, they are devitalized and susceptible to reinfections and fractures. The development of biology-based approaches for dental pulp regeneration or repair is possible today because of recent advances in tissue engineering and biomaterials. Cell-free regenerative endodontic therapy offers a promising strategy for the treatment of necrotic immature permanent teeth in children and adolescents. However, studies are underway to determine whether this procedure can be applied to mature teeth.

[Biological basis and clinical exploration of regenerative endodontic therapy].

Regenerative endodontic therapy is a tissue engineering based approach of treatment for endodontic disease. Its purpose is to achieve the regeneration of the pulp-dentin complex, thus to promote root development of the immature permanent tooth with necrotic pulp. Like other treatments based on tissue engineering techniques, the success of regenerative pulp therapy depends on such three elements as seed cells, scaffold materials and growth factors. Since its inception 20 years ago, there have been various terminologies in the literature, with similarities and differences in connotation. The pre

The Effects of EDTA on Blood Clot in Regenerative Endodontic Procedures.

INTRODUCTION: In regenerative endodontic procedures (REPs), a blood clot acts as a natural scaffold for regenerating dental pulp tissue. In current protocols, 17% EDTA is recommended for liberating growth factors from root dentin. Although EDTA affects clot formation in periodontal studies, the anticoagulant effect of EDTA has not been shown in REPs. Therefore, this study aimed to evaluate the effects of 17% EDTA on the characteristics and fiber density of blood clots using in vitro dentin blocks. METHODS: The roots of 35 human mandibular premolars were prepared to simulate open apices and ir

Regenerative Endodontics in the Treatment of Periapical Lesions of Endodontic Origin: A Systematic Review of Randomized Controlled Trials.

AIM: This systematic review aims to assess the effectiveness of regenerative approaches in promoting periapical healing and restoring pulp vitality. BACKGROUND: Periapical lesions of endodontic origin (LIPOE) are a common clinical issue that can compromise the prognosis of the affected tooth. While conventional treatment focuses on infection control and periapical healing, regenerative endodontics has emerged as a promising alternative. RESULTS: The review protocol followed preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines and was registered on PROSPERO (

Next-generation polymeric nanoparticles for regenerative endodontics: engineering the pulp microenvironment beyond antimicrobial therapy.

Regenerative endodontics aims not only to control infection but also to restore a functional pulp-dentin complex. Achieving this predictably remains challenging because disinfection, inflammation, blood vessel formation, matrix remodeling, and stem cell behavior are closely connected. Polymeric nanoparticles are increasingly being explored as a way to influence this complex environment because they can deliver therapeutic agents in a controlled and, in some cases, responsive manner. This review examines how the pulp microenvironment affects regenerative outcomes and discusses nanoscale strateg

A Review on Triple Antibiotic Paste as a Suitable Material Used in Regenerative Endodontics.

As the root canal system shows different and complicated anatomies, mechanical instrumentation alone has not the ability to provide a bacteria-free environment in root canals. On the other aspect, necrotic tissue remaining can decrease the effects of root canal irrigants and medicaments and also interfere with the adaptation of root canal fillings to dentin. As a result, certain disinfection and irrigation procedures are required to remove the remaining tissues from the root canal area thoroughly and also be able to eliminate the microorganisms. Triple antibiotic paste (TAP) containing metroni
