Commercial surrogacy: how provisions of monetary remuneration and powers of international law can prevent exploitation of gestational surrogates.

Increasing globalisation and advances in artificial reproductive techniques have opened up a whole new range of possibilities for infertile couples across the globe. Inter-country gestational surrogacy with monetary remuneration is one of the products of medical tourism meeting in vitro fertilisation embryo transfer. Filled with potential, it has also been a hot topic of discussion in legal and bioethics spheres. Fears of exploitation and breach of autonomy have sprung from the current situation, where there is no international regulation of surrogacy agreements--only a web of conflicting nati

A spatial regression analysis of German community characteristics associated with voluntary non-remunerated blood donor rates.

BACKGROUND AND OBJECTIVES: Previous studies have shown substantial geographical variation in blood donation within developed countries. To understand this issue better, we identified community characteristics associated with blood donor rates in German municipalities in an ecological analysis. MATERIALS AND METHODS: We calculated an aggregated rate of voluntary blood donors from each of 1533 municipalities in south-west Germany in 2007 from a database of the German Red Cross Blood Service. A multiple linear regression model estimated the association between the municipality-specific donor rat

Enabling remuneration for patient authors of company-sponsored publications: approaches and case study examples aligned with Good Publication practice.

Patient authors have been increasingly included on company-sponsored research publications, contributing a unique and needed expertise. Patients are routinely and rightfully remunerated for their contributions to advisory boards and non-authorship publication activities, such as peer review. However, the remuneration of patients for authorship activities has remained inconsistent and even contentious. Here, a working group of Good Publication Practice (GPP) steering committee members, patients and patient involvement and engagement experts, and industry partners present six commonly described

Barriers and enablers to non-remunerated plasma donation: A meta-synthesis of the qualitative literature using the theoretical domains framework.

BACKGROUND AND OBJECTIVES: Plasma is an essential component of therapies used to treat immune deficiencies, coagulation disorders and other serious conditions. Despite increasing global demand, many countries, including Canada, remain dependent on imported plasma products. Understanding the psychological, social and structural factors influencing plasma donation is critical to designing strategies that strengthen domestic supply. This meta-synthesis aimed to identify barriers and enablers to plasma donation using the theoretical domains framework (TDF). MATERIALS AND METHODS: A systematic sea

Voluntary and non-remunerated blood donation by 2025? A comparative study of blood donor eligibility in 19 healthcare institutions in Lebanon.

Eligibility criteria in blood donation are essential to protect both donor and recipient health. This study examines variability in donor selection criteria and donor health questionnaires across 19 healthcare institutions in Lebanon amid its transition toward a voluntary and non-remunerated blood donation (VNRD) system. Notable discrepancies were identified across medical, behavioral, demographic, and geographic deferral categories. A comparative analysis against Lebanese Committee for Blood Transfusion guidelines and EU-based standards reveals a lack of centralization and inconsistent adhere

Five year follow -up of a university initiated national voluntary non remunerated blood donation programme in a developing country.

International bodies continue to recommend blood services based on voluntary non remunerated blood donation as an essential prerequisite for blood safety and adequacy. Trinidad and Tobago is a multi-ethnic, multi-religious, high income developing country with a long history of family replacement and remunerated blood donation. Delivery of blood services is fragmented across five autonomous Regional Health Authorities and policy is established by the National Blood Transfusion Service in the Ministry of Health. A voluntary non remunerated blood donor programme initiated by The University of the

The complex remuneration of human resources for health in low-income settings: policy implications and a research agenda for designing effective financial incentives.

BACKGROUND: Human resources for health represent an essential component of health systems and play a key role to accelerate progress towards universal health coverage. Many countries in sub-Saharan Africa face challenges regarding the availability, distribution and performance of health workers, which could be in part addressed by providing effective financial incentives. METHODS: Based on an overview of the existing literature, the paper highlights the gaps in the existing research in low-income countries exploring the different components of health workers' incomes. It then proposes a novel

The Royal College of Ophthalmologists' National Ophthalmology Database study of cataract surgery: report 16, influence of remuneration model on choice of intraocular lens in the UK.

BACKGROUND/OBJECTIVES: Cataract surgery with intraocular lens (IOL) implantation is one of the most commonly performed surgeries worldwide. Within the UK, publicly funded cataract surgery is remunerated by two models: (1) "block contract" (BC), which commissions organisations to deliver whole service pathways without considering specific activity items; or (2) "payment by results" (PbR), which pays a tariff price for each procedure. This study aimed to examine the association between remuneration model and the cost and types of IOL used. SUBJECTS/METHODS: Cataract operations recorded on the R

Seroprevalence and trends in transfusion transmissible infections among voluntary non-remunerated blood donors at the Malawi Blood Transfusion Service-a time trend study.

INTRODUCTION: Collecting blood from voluntary non-remunerated blood donors from low risk populations is a key strategy for blood safety. Identifying such populations involves analysis of population and blood donor data to identify risk factors for transfusion transmissible infections (TTIs). There are no recent seroprevalence statistics for blood donors in Malawi. This study fills this gap by describing characteristics of blood donors, trend of annual prevalence of HIV, HBV, HCV and syphilis and factors associated with each TTI. METHODS: Retrospective analysis of blood donors' records in the

Degree of blood safety of voluntary non-remunerated versus replacement blood donations: A multi-centre study of the large cohort of blood donors from two provinces of Pakistan.

BACKGROUND AND OBJECTIVES: Voluntary non-remunerated blood donors (VNRBDs) are recognized as being crucial for the safety and sustainability of national blood supplies. Systems based on replacement donors (RDs) pose high risks of transfusion transmissible infections (TTIs). Currently, only 10%-13% of blood donations are voluntary in Pakistan. No large-scale studies have been conducted to objectively evaluate the impact of the mode of donation on the frequency of TTIs, a gap this study aimed to fill. MATERIALS AND METHODS: The study was conducted at the Indus Hospital, Karachi. Data from a tot

Hospitalization cost of Pressurized Intraperitoneal Aerosol chemotherapy (PIPAC).

INTRODUCTION: Pressurized Intraperitoneal Aerosol chemotherapy (PIPAC) is a new surgical technique for the treatment of unresectable peritoneal carcinomatosis. Very little data is available on the costs of this treatment in France as there is currently no code for PIPAC in the French Common Classification of Medical Acts (CCAM). Our objective was to estimate the mean cost of hospitalization for PIPAC in two French public teaching hospitals. METHODS: The mean cost of hospitalization was estimated from the mean fixed-rate remuneration paid to the hospital and the mean additional costs of treatm
