The allocation of scarce medical resources across medical conditions.

This study aimed to determine if the criteria participants use to make decisions concerning scarce medical resources differ across medical conditions - from life-saving to life-enhancing surgery (heart transplant, in vitro fertilization treatment and cosmetic surgery). Participants completed three questionnaires requiring them to rank order 16 hypothetical patients in order of priority for each medical condition. Demographic information about the hypothetical patients varied on four dimensions: age, annual income, smoking behaviour and whether or not the patient had children. There were signif

A dual-task analysis of resource allocation in dysthymia and anhedonia.

Research has shown dysthymic individuals to be hyporesponsive at various stages of information processing, yet it is not clear whether dysthymics are deficient in the amount of available attentional resources for information processing or, instead, in the allocation of those resources. To distinguish between these possibilities, the authors compared dysthymics to anhedonic and normal control Ss during the performance of memory tasks, under conditions of varying task priority and difficulty. Although there were no performance differences, dysthymics and anhedonics exhibited a consistently small

An Efficient Dynamic Resource Allocation Framework for Evolutionary Bilevel Optimization.

Bilevel optimization problems (BLOPs) are characterized by an interactive hierarchical structure, where the upper level seeks to optimize its strategy while simultaneously considering the response of the lower level. Evolutionary algorithms are commonly used to solve complex bilevel problems in practical scenarios, but they face significant resource consumption challenges due to the nested structure imposed by the implicit lower-level optimality condition. This challenge becomes even more pronounced as problem dimensions increase. Although recent methods have enhanced bilevel convergence throu

Time to Confirmed Neuroinfectious Diagnoses: Diagnostic Testing and Resource Allocation.

BACKGROUND AND OBJECTIVES: In a retrospective study evaluating the diagnostic approach of definitive neurological infections at a tertiary referral center, we assessed the time to diagnosis from presentation, number of diagnostic tests ordered, and modality of etiologic diagnosis. METHODS: A total of 111 confirmed clinical cases of neurological infections from 2010-2018 were reviewed. Definitive neuroinfectious diagnoses were defined by positive cerebrospinal (CSF) polymerase chain reaction (PCR)/antigen, CSF culture, CSF antibody, serology, or pathology tests. RESULTS: An etiologic diagnosi

Resource Allocation for Cognitive LEO Satellite Systems: Facilitating IoT Communications.

Due to the characteristics of global coverage, on-demand access, and large capacity, the low earth orbit (LEO) satellite communication (SatCom) has become one promising technology to support the Internet-of-Things (IoT). However, due to the scarcity of satellite spectrum and the high cost of designing satellites, it is difficult to launch a dedicated satellite for IoT communications. To facilitate IoT communications over LEO SatCom, in this paper, we propose the cognitive LEO satellite system, where the IoT users act as the secondary user to access the legacy LEO satellites and cognitively use

Sources and resources: importance of nutrients, resource allocation, and ecology in microalgal cultivation for lipid accumulation.

Regardless of current market conditions and availability of conventional petroleum sources, alternatives are needed to circumvent future economic and environmental impacts from continued exploration and harvesting of conventional hydrocarbons. Diatoms and green algae (microalgae) are eukaryotic photoautotrophs that can utilize inorganic carbon (e.g., CO2) as a carbon source and sunlight as an energy source, and many microalgae can store carbon and energy in the form of neutral lipids. In addition to accumulating useful precursors for biofuels and chemical feed stocks, the use of autotrophic mi

Attentional resource allocation in the early stages of motor skill learning.

The relationship between attentional resource allocation and early motor skill learning remains unclear. This study examined how proficiency in drone operation influences attention and which factors contribute to individual differences. Twenty-three participants with no prior drone experience performed a simulator task consisting of 12 three-minute trials, divided into three phases of four trials each. We estimated attentional resource allocation using auditory-evoked potentials (AEPs) elicited by task-irrelevant auditory probes. Performance improved significantly across phases (ps < 0.001), a

Resource Allocation: Stable Patients Remain Stable 12-24 h Post-tPA.

BACKGROUND AND PURPOSE: Stroke patients are currently monitored for neurological deterioration for 24 h following treatment with intravenous tissue plasminogen activator (IV tPA) or mechanical thrombectomy. This requires low nursing ratios and an intensive-care-like setting. As the half-life of IV tPA is short, many patients may not require such prolonged intensive monitoring and could be downgraded much earlier. We evaluate the frequency of neurological deterioration in the 0-12 and 12-24 h post-treatment windows. METHODS: Patients presenting with acute ischemic stroke treated with IV tPA an

Autophagy controls resource allocation and protein storage accumulation in Arabidopsis seeds.

Autophagy is essential for nutrient recycling and plays a fundamental role in seed production and grain filling in plants. Autophagy participates in nitrogen remobilization at the whole-plant level, and the seeds of autophagy mutants present abnormal C and N contents relative to wild-type (WT) plants. It is well known that autophagy (ATG) genes are induced in leaves during senescence; however, expression of such genes in seeds has not yet been reported. In this study we show that most of the ATG genes are induced during seed maturation in Arabidopsis siliques. Promoter-ATG8f::UIDA and promoter

Recommendations for increasing the use of HIV/AIDS resource allocation models.

BACKGROUND: Resource allocation models have not had a substantial impact on HIV/AIDS resource allocation decisions in spite of the important, additional insights they may provide. In this paper, we highlight six difficulties often encountered in attempts to implement such models in policy settings; these are: model complexity, data requirements, multiple stakeholders, funding issues, and political and ethical considerations. We then make recommendations as to how each of these difficulties may be overcome. RESULTS: To ensure that models can inform the actual decision, modellers should underst

Cognitive decline limits compensatory resource allocation within the aged swallowing network.

Cognitive decline has been postulated to predispose to presbyphagia but the neurophysiological basis of this interaction is unclear. To investigate the role of cognition for compensatory resource allocation within the swallowing network and behavioral swallowing performance in dual-task cognitive and motor interference in ageing, volunteers ≥ 70 years of age without preexisting diseases causing dysphagia were investigated using Flexible Endoscopic Evaluation of Swallowing (FEES) including a cognitive and motor dual-task paradigm and a Montreal Cognitive Assessment. The neural correlates of swa

US federal resource allocations are inconsistent with concentrations of energy poverty.

Recent data from the US Energy Information Administration reveals that nearly one in three households in the United States report experiencing energy poverty, and this number is only expected to rise. Federal assistance programs exist, but allocations across states have been nearly static since 1984, while the distribution of energy poverty is dynamic in location and time. We implement a LASSO-based machine learning approach using sociodemographic and geographical information to estimate energy burden in each US census tract for 2015 and 2020. We then compare the allocation to states from the

Evaluation of the use of modelling in resource allocation decisions for HIV and TB.

INTRODUCTION: Globally, resources for health spending, including HIV and tuberculosis (TB), are constrained, and a substantial gap exists between spending and estimated needs. Optima is an allocative efficiency modelling tool that has been used since 2010 in over 50 settings to generate evidence for country-level HIV and TB resource allocation decisions. This evaluation assessed the utilisation of modelling to inform financing priorities from the perspective of country stakeholders and their international partners. METHODS: In October to December 2021, the World Bank and Burnet Institute led

The medicalisation of health inequalities and the English NHS: the role of resource allocation.

Tackling health inequalities (HI) has become a key policy objective in England in recent years. Yet, despite the wide-ranging policy response of the 1997-2010 Labour Government, socio-economic variations in health continued to widen. In this paper, we seek to explore why. We propose that a meta-narrative has emerged in which the health problems facing England's most deprived areas, and the solution to those problems, have increasingly come to be linked to levels of National Health Service (NHS) funding. This has been, in part, a response to key shortcomings in previous rounds of resource alloc

Allocation of resources--to treat or not to treat, and who shall beg the question?

Two separate clusters of events in recent months have spurred public discussions over the questions of how and to what degree the public should be involved in resource allocation decisions. In England, a group of physicians unilaterally decided to deny bypass surgery to heavy smokers, and in the United States, uninsured parents of conjoined twins elected to seek separation of the twins despite heavy odds against their survival, raising questions of whether health care reform will stymie such decisions in the future.

Aging increases the cortical resources allocated to static balance maintenance.

Maintaining balance requires a complex interplay between sensory and motor processes, and this ability deteriorates with age, impairing daily life activities and contributes to increased fall risks. Importantly, while cognitive-motor interference paradigms suggest an aging-related increase in the cortical involvement in balance regulation, direct evidence remains lacking. To clarify this issue, we assessed the effect of aging on sway-based corticokinematic coherence (CKC), which is a measure of the coupling between cortical electrophysiological signals and postural sway. To that end, we record

An optimisation framework for resource allocation in palliative and end-of-life care.

End-of-life care for frail and elderly patients is frequently characterised by high healthcare utilisation, fragmented service delivery, and limited coordination, resulting in variable quality and excess cost. This study presents a proof-of-concept framework, tested using synthetic data to illustrate potential applications in strategic planning. Few planning approaches integrate patient-level pathways into operational models that balance efficiency with patient-centred outcomes. Optimisation models were developed to support strategic resource planning for frail, elderly, and palliative patient

Influenza vaccine allocation in tropical settings under constrained resources.

Influenza virus seasonality, synchronicity, and vaccine supply differ substantially between temperate and tropical settings, and optimal vaccination strategy may differ on this basis. Many national vaccine recommendations focus on high-risk groups, elderly populations, and healthcare workers despite previous analyses demonstrating broad benefits to vaccinating younger high-contact age groups. In this study, we parameterized an age-structured nonseasonal asynchronous epidemiological model of influenza virus transmission for a tropical low-income setting. We evaluated timing and age allocation o

Allocation of resources to collaborators and free-riders in 3-year-olds.

Recent studies have shown that in situations where resources have been acquired collaboratively, children at around 3 years of age share mostly equally. We investigated 3-year-olds' sharing behavior with a collaborating partner and a free-riding partner who explicitly expressed her preference not to collaborate. Children shared more equally with the collaborating partner than with the free rider. These results suggest that young children are sensitive to the contributions made by others to a collaborative effort (and possibly their reasons for not collaborating) and distribute resources accord
