Treatment of cryptococcal meningitis in resource limited settings.

PURPOSE OF REVIEW: Cryptococcal meningitis most commonly occurs in advanced HIV. Although diminishing in the developed world with antiretroviral therapy (ART), it remains a major problem in resource-limited settings. ART rollout will improve long-term HIV survival if opportunistic infections are effectively treated. Considering cryptococcal meningitis in that context, this review addresses excess morbidity and mortality in developing countries, treatment in areas of limited drug availability and challenges posed by combined anticryptococcal and HIV therapy. RECENT FINDINGS: From Early Fungici

Diagnostics in Ebola Virus Disease in Resource-Rich and Resource-Limited Settings.

The Ebola virus disease (EVD) outbreak in West Africa was unprecedented in scale and location. Limited access to both diagnostic and supportive pathology assays in both resource-rich and resource-limited settings had a detrimental effect on the identification and isolation of cases as well as individual patient management. Limited access to such assays in resource-rich settings resulted in delays in differentiating EVD from other illnesses in returning travellers, in turn utilising valuable resources until a diagnosis could be made. This had a much greater impact in West Africa, where it contr

Infection-associated acute kidney injury in resource-limited settings.

Acute kidney injury (AKI) is a frequent and life-threatening complication of infection, and a major contributor to preventable morbidity and mortality in resource-limited settings. In these contexts, AKI is commonly driven by a broad range of infections, including enteric, zoonotic and vector-borne diseases, that occur outside the hospital and in populations with limited access to diagnostics and supportive care. Infection-associated AKI results from diverse and overlapping mechanisms, including hypovolaemia, inflammation, endothelial and microcirculatory dysfunction, haemolysis and direct kid

Managing hypertension and cardiovascular risk in resource-limited settings.

PURPOSE OF REVIEW: Hypertension management is a gateway for cardiovascular risk reduction. The status of hypertension treatment and control is low in resource-limited lower income countries and similar settings in middle-income and high-income countries. Implementation of strategies for prevention and management of hypertension can lead to a substantial increase in its control and decline in associated cardiovascular mortality and disease burden. RECENT FINDINGS: Population-wide and clinical interventions that can be deployed in resource-limited settings globally to improve hypertension contr

Ataxia-Telangiectasia in Resource-Limited Settings: A Diagnostic Challenge.

Ataxia-telangiectasia (A-T) is a rare autosomal recessive disorder characterized by cerebellar ataxia and oculocutaneous telangiectasias, with increased malignancy risk from radiation-induced DNA double-strand breaks. Its multisystem involvement makes diagnosis challenging, and confirmation requires identifying biallelic pathogenic variants in the ATM gene. We report an eight-year-old boy with unsteady gait and bilateral ocular telangiectasia. He had no recurrent infections, and immunoglobulin (Ig) levels were normal, except for low IgA. Although serum alpha-fetoprotein was elevated and MRI de

Unraveling Histiocytic Disorders: Two Cases From Resource-Limited Settings.

Histiocytic disorders include a range of uncommon illnesses marked by the buildup of cells that have developed into macrophages, dendritic cells, or monocytes in diverse tissues and organs. Over 100 distinct subtypes have been documented, exhibiting a diverse array of clinical symptoms, presentations, and histologic features that can be confused with other clinical conditions leading to delayed diagnosis. They affect both children and adults, generating a variety of clinical symptoms that can be limited to one position, numerous areas within one system, or affect many systems in the body. We p

Bubble CPAP splitting: innovative strategy in resource-limited settings.

BACKGROUND: Non-invasive respiratory support for neonates using bubble continuous positive airway pressure (bCPAP) delivery systems is now widespread owing to its safety, cost effectiveness and easy applicability. Many innovative solutions have been suggested to deal with the possible shortage in desperate situations like disasters, pandemics and resource-limited settings. Although splitting of invasive ventilation has been reported previously, no attempts to split non-invasive respiratory support have been reported. OBJECTIVE: The primary objective was to test the feasibility of splitting th

Tools for Detection of Schistosomiasis in Resource Limited Settings.

Schistosomiasis is a debilitating disease affecting over 200 million people, with the highest burden of morbidity and mortality in African countries. Despite its huge impact on the health and socio-economic burden of the society, it remains a neglected tropical disease, with limited attention from governments and stakeholders in healthcare. One of the critical areas that is hugely under-developed is the development of accurate diagnostics for both intestinal and urogenital schistosomiasis. Diagnosis of schistosomiasis is important for the detection and treatment of disease in endemic and non-e

The burden of pulmonary hypertension in resource-limited settings.

Pulmonary vascular disease (PVD) is a significant global health problem and accounts for a substantial portion of cardiovascular disease in the developing world. Although there have been considerable advances in therapeutics for pulmonary arterial hypertension, over 97% of the disease burden lies within the developing world where there is limited access to health care and pharmaceuticals. The causes of pulmonary arterial hypertension differ between industrialized and developing nations. Infectious diseases-including schistosomiasis human immunodeficiency virus, and rheumatic fever-are common c

Application of Cancer Screening Guidelines in Resource Limited Settings.

Cancer screening is increasingly promoted as a public health intervention, yet its effective translation with demonstrable impact on disease burden remains limited in low resource settings due to the suboptimal implementation of guidelines. While evidence from high-income countries and settings have informed international guidelines, the direct application of these recommendations often overlooks the realities of under-resourced settings, where diagnostic infrastructure, referral pathways, and treatment capacity are fragmented and inadequate. Moreover, the societal and health belief systems in

Diagnostic applications for Lassa fever in limited-resource settings.

Lassa fever, caused by arenavirus Lassa virus (LASV), is an acute viral haemorrhagic disease that affects up to an estimated 300 000 individuals and causes up to 5000 deaths per year in West Africa. Currently available LASV diagnostic methods are difficult to operationalise in low-resource health centres and may be less sensitive to detecting all known or emerging LASV strains. To prioritise diagnostic development for LASV, we assessed the diagnostic applications for case detection, clinical management, surveillance, outbreak response, and therapeutic and vaccine development at various healthc

Recent advances in tuberculosis diagnostics in resource-limited settings.

Smear-negative and drug-resistant cases of tuberculosis (TB) disease necessitate the development of new diagnostic methods, especially in resource-limited settings. To improve the current TB situations, sensitive and specific TB point-of-care tests (POCTs) should be developed. This review addresses the current status of TB, novel diagnostic methodologies for TB, and the impact of those new diagnostics on TB control in such situations. Moreover, the perspective of TB management based on laboratory examinations is described. Smear microscopy with sputum samples is the only laboratory examination

HIV-associated tuberculosis and cryptococcosis in resource-limited settings.

Tuberculosis (TB) and cryptococcosis are common infectious complications in HIV in resource-limited settings and contribute substantial morbidity and mortality. The increasing access to highly active antiretroviral treatment (HAART) has invited numerous challenges such as timing of HAART, cotreatment (drug dosages and interaction), immune reconstitution syndromes, and withdrawal of chemoprophylaxis. Numerous small studies propose the feasibility of concomitant TB/HIV treatment that needs to be confirmed in large, randomized trials. Treatment of acute cryptococcocal meningo-encephalitis with am

Partial heart transplantation in resource-limited settings: challenges and feasibility.

INTRODUCTION: Partial heart transplantation provides living, growth-capable valve implants for paediatric recipients. The feasibility and challenges of implementing partial heart transplantation in resource-limited settings are evaluated, where the burden of valvular heart disease is greatest and paediatric valve replacement options remain severely limited. METHODS: A comprehensive search was conducted examining partial heart transplantation clinical outcomes, organ procurement logistics, immunosuppression requirements, and the infrastructure landscape for cardiac surgery in low-and middle-in

Establishing oncofertility care in limited resource settings: a narrative review.

PURPOSE: While the literature has addressed the implementation of oncofertility care at developed institutions, minimal advice exists for those seeking to build oncofertility programs in limited resource settings (LRS). Our research offers a promising conversation on establishing oncofertility care in such settings from the perspective of a practitioner working to establish care in Latin America. We propose practices that have the potential to significantly improve access to and quality of care in these challenging settings. METHODS: PubMed was used to identify articles on fertility preservat

Determinants of preterm birth among newborns in resource limited settings.

The World Health Organization defines preterm birth as any birth before 37 completed weeks of gestation, or fewer than 259 days since the first day of the woman's last menstrual period. Globally, 1 in 10 babies is born prematurely and more than a million babies are estimated to die annually. The determinants of preterm birth may vary from region to region within the same country due to variation in socioeconomic status and health care service coverage. Therefore, this study was designed to identify determinants of preterm birth among newborns born in selected public hospitals in west Guji, eas

Providing Neurocritical Care in Resource-Limited Settings: Challenges and Opportunities.

Acute neurologic illnesses (ANI) contribute significantly to the global burden of disease and cause disproportionate death and disability in low-income and middle-income countries (LMICs) where neurocritical care resources and expertise are limited. Shifting epidemiologic trends in recent decades have increased the worldwide burden of noncommunicable diseases, including cerebrovascular disease and traumatic brain injury, which coexist in many LMICs with a persistently high burden of central nervous system infections such as tuberculosis, neurocysticercosis, and HIV-related opportunistic infect
