Human respiratory muscles: fibre morphology and capillary supply.

In man the diaphragm (DIA) and abdominal muscles comprise approximately 50% slow-twitch (ST) fibres, whereas a higher proportion (60%) is found in intercostal muscles and the scalenes. All respiratory muscles show an equal distribution of fast-twitch (FTa and b) fibres with the exception of the expiratory intercostal muscles which have few FTb fibres. The inspiratory muscles have a uniformly small fibre size, in contrast to the expiratory intercostal muscle fibres which are large. The fibre size of the inspiratory muscles is maintained with ageing, whereas that of the expiratory intercostal mu

Evaluation of the respiratory muscles efficiency during an incremental flow respiratory test.

The aim of this study was to evaluate the respiratory muscles efficiency during a progressive incremental flow (IF) respiratory test in healthy and Chronic Obstructive Pulmonary Disease (COPD) subjects. To achieve this, the relationship between mouth Inspiratory Pressure (IP) increment, which is a measure of the force produced by respiratory muscles, and respiratory muscular activity increment, evaluated by means of Mechanomyografic (MMG) signals of the diaphragm muscle, was analyzed. Moreover, the correlation between the respiratory efficiency measure and the obstruction severity of the subje

[Status asthmaticus with rhabdomyolysis of the respiratory muscles].

A 40-year-old roofer called the medical emergency team because of sudden dyspnea and chest constriction. He was suffering from bronchial asthma that exacerbated probably due to the inhalation of a solvent while at work. Because of status asthmaticus he was intubated immediately after arrival in the hospital and breathing had to be assisted artificially for ten hours. After extubation he complained of chest pain. In view of an elevated creatinkinase of 4368 U/1 this was interpreted as rhabdomyolysis of the respiratory muscles due to the status asthmaticus. As a cause of chest pain and elevation

Electromyography of respiratory muscles in amyotrophic lateral sclerosis.

We reviewed the records of 52 amyotrophic lateral sclerosis (ALS) patients examined between 1995 and 2000 who had needle electromyography (EMG) of their respiratory muscles, including the diaphragm, at or near the time of their diagnosis. With respiratory function testing, patients with abnormal diaphragmatic EMG at diagnosis (Group 1, n=23) had significantly lower forced vital capacity (FVC), lower daytime arterial PO(2) and higher PCO(2) measurements (p<0.05) than patients with normal diaphragmatic EMG (Group 2, n=29). Twenty-eight percent of the patients without symptoms or signs of respira

Respiratory activities in relation to external glossal muscles.

In this study, the activities of the two external glossal muscles, Genioglossus (Gg) and Styloglossus (Sg), related to respiration were examined through the electromyography (EMG) in dog. During quiet breathing, no phasic respiratory activity were observed in either muscle. With hypercapnic condition induced by closed rebreathing respiratory system, both Sg and Gg showed phasic respiratory activities in respiration period. At first inspiratory EMG activity was observed from Sg (PaCO2 > 50 mmHg), then after a while from Gg (PaCO2 > 55 mmHg). NaCN injection bilaterally to the carotid body enhanc

Aminophylline and the respiratory muscles: an alternative view.

The respiratory muscles constitute a pump as vital to life as the heart. In patients with severe lung disease, the work of breathing is increased greatly, whereas the capacity of the respiratory muscles to generate tension and ventilation is impaired. In these circumstances, respiratory muscle fatigue may occur. When the cardiac pump fails, it often is possible to improve its performance by drug therapy with inotropic agents. In recent years, attention has been focused on the possibility of enhancing respiratory muscle function with drugs. The agents most studied have been the methylxanthines,

Function of the respiratory muscles in acute hemiplegia.

Electromyograms of the parasternal intercostal muscles and of the diaphragm were obtained with surface electrodes in 20 patients with early flaccid hemiplegia due to cerebrovascular accident. In most patients, a striking reduction in activity was observed during voluntary inspirations in both the intercostal muscles and the diaphragm on the side of the paresis. These findings suggest that in most persons the respiratory muscles are innervated primarily by decussated fibers. Dysfunction of the respiratory muscles might be implicated in the respiratory tract infections of patients with early hem

Postural and respiratory functions of the pelvic floor muscles.

AIMS: Due to their contribution to modulation of intra-abdominal pressure (IAP) and stiffness of the sacroiliac joints, the pelvic floor muscles (PFM) have been argued to provide a contribution to control of the lumbar spine and pelvis. Furthermore, as IAP is modulated during respiration this is likely to be accompanied by changes in PFM activity. METHODS: In order to evaluate the postural and respiratory function of the PFM, recordings of anal and vaginal electromyographic activity (EMG) were made with surface electrodes during single and repetitive arm movements that challenge the stability

Respiratory rhythmically regulated electrical stimulation of paralyzed laryngeal muscles.

Electrical stimulation of the posterior cricoarytenoid muscle, synchronized with inspiration, was achieved in dogs, utilizing a radio frequency stimulus triggered by a chest wall expansion transducer. This system brings about the abduction of the paralyzed vocal cord for the entire duration of inspiration, which allows a normal flow of air through the larynx. The stimulation system could be tested successfully in actual experiments in dogs with artificial paralysis of the recurrent laryngeal nerve. Its effectiveness could be proved through observation of the vocal cord movements (photographic

Fulminant myocarditis with myositis of ocular and respiratory muscles.

A 46-year-old Japanese woman visited a nearby hospital because of diplopia after flu-like symptoms. One month later, she presented with blepharoptosis and external ophthalmoplegia. Laboratory tests showed a high creatine kinase concentration (3146 U/L). She underwent intravenous immunoglobulin therapy; however, her symptoms did not improve, prompting transfer to our institute. On admission, transthoracic echocardiography revealed 30% of left ventricular ejection fraction and edema of the left ventricular wall. Coronary angiography showed no significant coronary stenosis. Imaging of respiratory muscles in neuromuscular disease: A review.

Respiratory muscle weakness frequently occurs in patients with neuromuscular disease. Measuring respiratory function with standard pulmonary function tests provides information about the contribution of all respiratory muscles, the lungs and airways. Imaging potentially enables the study of different respiratory muscles, including the diaphragm, separately. In this review, we provide an overview of imaging techniques used to study respiratory muscles in neuromuscular disease. We identified 26 studies which included a total of 573 patients with neuromuscular disease. Imaging of respiratory musc

Respiratory muscle dysfunction in COPD: from muscles to cell.

Respiratory muscle dysfunction is a cardinal feature of acute and chronic respiratory failure in COPD. Diaphragm and accessory inspiratory muscles face increased load due to increased lung resistance and elastance, as well as increased ventilatory demands. Concomitantly, the capacity of the inspiratory muscles to generate pressure is decreased due to mechanical disadvantage imposed by hyperinflation. Additionally, inflammation and oxidative stress impair muscle fiber specific force generation and increase diaphragm susceptibility to sarcomer disruption during acute inspiratory loading. In resp

Thixotropy of rib cage respiratory muscles in normal subjects.

In this study, we searched for signs of thixotropic behavior in human rib cage respiratory muscles. If rib cage respiratory muscles possess thixotropic properties similar to those seen in other skeletal muscles in animals and humans, we expect resting rib cage circumference would be temporarily changed after deep rib cage inflations or deflations and that these aftereffects would be particularly pronounced in trials that combine conditioning deep inflations or deflations with forceful isometric contractions of the respiratory muscles. We used induction plethysmography to obtain a continuous re

Functional magnetic stimulation of the respiratory muscles in dogs.

This study assessed the ability of functional magnetic stimulation (FMS) to activate the respiratory muscles in dogs. With the animal supine, FMS of the phrenic nerves using a high-speed magnetic stimulator was performed by placing a round magnetic coil (MC) at the carotid triangle. Following hyperventilation-induced apnea, changes in volume (deltaV) and airway pressure (deltaP) against an occluded airway were determined. FMS of the phrenic nerves produced substantial inspired function (deltaV = 373 +/- 20.5 mL and deltaP = -20 +/- 2.0 cm H2O). After bilateral phrenectomies, maximal inspired d

Activation of human respiratory muscles during different voluntary manoeuvres.

1. This study used three techniques (bilateral phrenic nerve stimulation, motor cortex stimulation and quantitative electromyography) to assess the degree of activation of the diaphragm, intercostal-accessory muscles and abdominal muscles during postural tasks and respiratory manoeuvres. They included maximal inspiratory and expulsive efforts. 2. Bilateral phrenic nerve stimuli at supramaximal levels produced an average change in transdiaphragmatic pressure (Pdi) of 28 cmH2O during relaxation. During maximal inspiratory or expulsive efforts, all subjects were able to activate the diaphragm ful

Differential activation of respiratory muscles during wakefulness and sleep.

Evidence exists that strongly suggests that an imbalance in both the magnitude and timing of electrical activity between the upper airway and chest wall inspiratory muscles can exist during sleep. Too little or delayed activity of upper airway inspiratory muscles is associated with upper airway narrowing or collapse in patients with obstructive apnea. Preferential activation of upper airway muscles is associated with a decrease in upper airway resistance or in reopening of the airway following an apnea. These interrelationships between the upper and lower respiratory muscles may be exacerbated
