High-Flow Nasal Cannula Oxygen Therapy in the Management of Respiratory Failure: A Review.

High-flow nasal cannula (HFNC) oxygen therapy is gaining traction globally as a treatment for respiratory failure. There are several physiological benefits, and there is a growing body of evidence showing improved quality of life and patient comfort with HFNC, both in acute and home settings. Due to the increased burden of long-term respiratory conditions such as chronic obstructive pulmonary disease (COPD) on healthcare systems worldwide, the role of ward-based and post-discharge interventions in the prevention of hospital readmissions is an area of increasing interest. In this narrative revi

Risk factors and biomarkers for post-tuberculosis lung damage in a Chinese cohort of male smokers and non-smokers: protocol for a prospective observational study.

INTRODUCTION: Post-tuberculosis lung damage (PTLD) refers to the residual pulmonary impairment following the completion of antituberculosis (TB) therapy, characterised by persistent respiratory symptoms and abnormal pulmonary function. The risk factors and biomarkers for PTLD have been scarcely investigated. More importantly, whether and to what extent cigarette smoking is involved in PTLD remain to be known. METHODS AND ANALYSIS: This prospective observational study will enrol 400 male smoking or non-smoking patients aged 25-65 years, with newly confirmed active TB between 2022 and 2024, fro

Efficacy of high-flow oxygen by nasal cannula with active humidification in a patient with acute respiratory failure of neuromuscular origin.

The treatment of choice for patients with respiratory failure of neuromuscular origin, especially in patients with hypercapnic respiratory acidosis, is noninvasive ventilation (NIV). Endotracheal intubation and invasive ventilation are indicated for patients with severe respiratory compromise or failure of NIV. In recent years, high-flow oxygen therapy and active humidification devices have been introduced, and emerging evidence suggests that high-flow oxygen may be effective in various clinical settings, such as acute respiratory failure, after cardiac surgery, during sedation and analgesia,

A pilot study of nurse-led, home monitoring for patients with chronic respiratory failure and with mechanical ventilation assistance.

We assessed the feasibility of telemedicine for home monitoring of 45 patients with chronic respiratory failure (CRF) discharged from hospital. The patients transmitted pulsed arterial saturation (pSat) data via a telephone modem to a receiving station where a nurse was available for a teleconsultation. A respiratory physician was also available. Scheduled and ad hoc appointments were conducted. Thirty-five patients were on home mechanical ventilation, 13 with invasive and 22 with non-invasive devices. The main diagnosis was chronic obstructive pulmonary disease (COPD). The follow-up period wa

Comparing effectiveness of intelligent volume-assured pressure support (iVAPS) vs bi-level positive airway pressure spontaneous/timed (BPAP S/T) for hypercapnic respiratory failure in chronic obstructive pulmonary disease.

BACKGROUND AND AIM: Intelligent volume-assured pressure support (iVAPS) is a relatively new hybrid mode of non-invasive ventilation (NIV). There is still limited evidence for iVAPS. The aim of this study was to compare the effectiveness of iVAPS to that of bi-level positive airway pressure spontaneous/timed (BPAP S/T) in patients with acute hypercapnic respiratory failure or acute-on-chronic hypercapnic respiratory failure caused by acute exacerbation of chronic obstructive pulmonary disease (AECOPD) in the emergency department. MATERIAL AND METHODS: This was an observational, retrospective s

Respiratory compromise in children presenting to an urban emergency department of a tertiary hospital in Tanzania: a descriptive cohort study.

BACKGROUND: Respiratory compromise is the leading cause of cardiac arrest and death among paediatric patients. Emergency medicine is a new field in low-income countries (LICs); the presentation, treatment and outcomes of paediatric patients with respiratory compromise is not well studied. We describe the clinical epidemiology, management and outcomes of paediatric patients with respiratory compromise presenting to the first full-capacity Emergency Department in Tanzania. METHODS: This was a prospective cohort study of paediatric patients (< 18 years) with respiratory compromise (respiratory d

Recognizing and treating methemoglobinemia: a rare but dangerous complication of topical anesthetic or nitrate overdose.

Methemoglobinemia is a rare but dangerous complication of topical anesthetic overdose administration or exposure to nitrate-containing substances in the hospital or community. Paradoxically, although the patient may present with cyanosis with low oxygen saturation levels, they will not respond to oxygen therapy. Treatment is slow intravenous administration of a titrated dose of methylene blue. Adult and pediatric critical care nurses and respiratory therapists, operating room and procedure/diagnostic center nurses, short-stay or outpatient surgery center staff, and adult and pediatric emergenc

Approach to respiratory failure in emergency department.

OBJECTIVES AND BACKGROUND: The goal of this review is to provide update recommendations that can be used by emergency physicians who provide primary cares to patients with Acute Respiratory Failure (ARF), from the admission to an emergency department through the first 24 to 48 hours of hospitalization. This work wants to address the diagnosis and emergency medical care of ARF and the management of medical complications. STATE OF THE ART: A lot of statement has been developed for the early management and treatment of ARF; moreover, over the last fifteen years, we have assisted to the rise of a

A Preventative Respiratory Protocol to Identify Trauma Subjects at Risk for Respiratory Compromise on a General In-Patient Ward.

BACKGROUND: Patients are at risk for respiratory complications after sustaining blunt chest trauma, yet contradictory evidence exists about the utility of prophylactic respiratory therapy to reduce respiratory complications in this population. This study assessed the effectiveness of a proactive respiratory protocol on an in-patient ward to identify trauma patients at risk for pulmonary complications, administer appropriate therapies, and prevent deterioration requiring transfer to the ICU. METHODS: Trauma patients received a respiratory therapy evaluation at the time of admission to a genera

Verification of Budesonide/Glycopyrronium/Formoterol (BGF) Triple Therapy in the Department of Cardiology: A Pilot Study.

Background Chronic obstructive pulmonary disease (COPD) frequently coexists with heart failure (HF), worsening patient prognosis. While single-inhaler triple therapy (BGF: budesonide/glycopyrronium/formoterol) is effective, the clinical impact of its initiation by cardiologists, who often manage latent COPD in patients with HF, remains underexplored. This study aimed to evaluate the validity and safety of BGF therapy managed by cardiologists compared to respiratory specialists and to identify prognostic factors associated with acute exacerbations. Methods This retrospective pilot study analyze

[Care of patients with exacerbation of chronic obstructive lung disease].

Exacerbations of chronic obstructive pulmonary disease are a common cause of hospital admission. They have a profound effect on the quality of life, disease progression and healthcare costs. The central event is amplified inflammation with many biomarkers who are not in clinical use. Severe dyspnea and large volume of purulent sputum are typical clinical presentation of exacerbation due to viral or bacterial infections. Bronchodilators and corticosteroids are the mainstay of pharmacologic therapy of exacerbation, antibiotics are useful in the presence of purulent sputum and other signs of resp

Noninvasive ventilation in cardiogenic pulmonary edema: a multicenter randomized trial.

Studies employing noninvasive pressure support ventilation in cardiogenic pulmonary edema have been performed in the intensive care unit when overt respiratory failure is already present and in small groups of patients. In this multicenter study, performed in emergency departments, 130 patients with acute respiratory failure were randomized to receive medical therapy plus O2 (65 patients) or noninvasive pressure support ventilation (65 patients). The primary end point was the need for intubation; secondary end points were in-hospital mortality and changes in some physiological variables. Nonin

High-Flow Nasal Cannula Use for Respiratory Support Outside the Paediatric Intensive Care Unit in Children Over 1 Year: A Narrative Review.

AIMS: High-flow nasal cannula (HFNC) therapy is increasingly used outside paediatric intensive care units (PICUs) for children with acute respiratory distress, despite limited evidence beyond bronchiolitis and neonatal populations. No existing review has synthesised evidence in children over 1 year without bronchiolitis managed in emergency department and ward settings. This narrative review aims to address this gap. METHODS: PubMed, Scopus and Medline (OVID) were searched to October 2025 for studies of HFNC in children aged over 12 months with acute respiratory distress, managed in emergency

Anti-MuSK-Positive Myasthenic Crisis in a 7-Year-Old Female.

A seven-year-old African American female with anti-MuSK-positive Juvenile Myasthenia Gravis collapsed while at school from progressively worsening weakness and dyspnea. On initial emergency department presentation, she required 15 liters per minute of supplemental oxygen to maintain oxygen saturation above 92%. Initial pulmonary function tests and venous blood gas led to the decision to place her on noninvasive positive pressure ventilation (NPPV) with BiPAP in the emergency department. Due to worsening hypercarbia, she later required mechanical intubation in the PICU and underwent IVIG therap

Anhydrous ammonia injuries.

Anhydrous ammonia, a pungent, colorless gas or liquid, is commonly used around the world as a refrigerant and fertilizer in agriculture. It is an alkali causing liquefaction or freeze-dry lesions with skin contact. Initial emergency response involves evacuation, decontamination, first aid and alerting proper authorities. Decontamination must be complete and rescuers must avoid being overcome by fumes. Emergency department care is directed at respiratory, ocular, skin and gastrointestinal treatment. Respiratory and ocular lesions tend to be the most severe and can be used as a triage guide. The

Nasal high flow therapy use in wards in patients with chronic obstructive pulmonary disease may spare ICU resources.

Nasal high flow therapy has been previously studied for the management of acute hypoxic respiratory failure in patients with chronic obstructive pulmonary disease but the data regarding its use outside of the intensive care unit are sparse. We aimed to evaluate safety and efficacy of nasal high flow therapy outside of the intensive care unit in patients with acute hypoxic respiratory failure and known chronic obstructive pulmonary disease. We conducted a retrospective matched historic cohort study of adult patients with diagnosed chronic obstructive pulmonary disease presenting with acute hypo

Updates on respiratory syncytial virus prophylaxis: the past, present and future.

PURPOSE OF REVIEW: Human respiratory syncytial virus (RSV) causes acute respiratory infection in individuals of all age groups. It is the most common pathogen identified in infants and young children with acute lower respiratory infection. Although most infants and young children experience mild, cold-like symptoms, some infants develop severe lower respiratory tract disease such as pneumonia and bronchiolitis that may result in an emergency department visit or hospitalization. RECENT FINDINGS: In the United States, hospitalization rates have risen during the last decades, and while premature

[A case of anti-IgLON5 disease with recurrent respiratory failure].

A 72-year-old woman with a seven-year history of Parkinson's disease, characterized by gait instability, rigidity, and postural instability, was admitted to our department for evaluation of recurrent episodes of hypoxemia and altered consciousness. During hospitalization, she experienced recurrent episodes of respiratory failure, prompting the measurement of anti-IgLON5 antibodies, which were found to be positive, leading to a diagnosis of anti-IgLON5 disease. The respiratory failure could not be attributed to vocal cord paralysis or respiratory muscle weakness, leading to the hypothesis that
