Are hospitalized Parkinson's disease patients more likely to carry a do-not-resuscitate order?

While DNR utilization is a complex subjective phenomenon, the effect of such a decision can collectively influence attitudes of care. The role of palliative care in advanced PD has been under appreciated. We reviewed the Healthcare Cost and Utilization Project's National Inpatient Sample (NIS) database from 2012 for all hospitalizations ⩾65years. We identified PD by using ICD-9-CM code 332.0 and DNR status with ICD code - V49.86 entered during the same admission as a secondary diagnosis. We estimated risk of mortality by the 3M™ All Patient Refined DRG (APR DRG) classification System and gener

Use of do-not-resuscitate orders in patients with kidney disease hospitalized with acute myocardial infarction.

BACKGROUND: Patients with kidney disease are at increased risk for adverse health outcomes in comparison to patients without kidney disease. Therefore, patients with kidney disease may have greater use of do-not-resuscitate (DNR) orders than patients without kidney disease in the setting of an acute illness. We examined the association between advanced kidney disease and use of DNR orders in patients admitted with an acute myocardial infarction (AMI) to all greater Worcester, MA, hospitals as part of an epidemiological study. METHODS: Use of DNR orders in 4,033 Worcester residents hospitalize

Prevalence and related factors of do-not-resuscitate orders among in-hospital cardiac arrest patients.

PURPOSE: Studies concerning do-not-resuscitate (DNR) orders in mainland China are rather scarce. We explored the prevalence and related factors of DNR orders among in-hospital cardiac arrest (IHCA) patients at a general tertiary hospital in mainland China. MATERIALS AND METHODS: We identified all IHCA patients hospital-wide between July 2019 and September 2020. Data regarding DNR status were collected from medical records. We investigated the frequency of DNR orders and explored the determinant factors of DNR establishment using logistic regression. RESULTS: A total of 1154 IHCA patients wer

How--and when--to obtain consent for do-not-resuscitate orders. Clinical guidelines and strategies for resolving conflicts.

Consider a do-not-resuscitate (DNR) order when a patient's presumed consent for cardiopulmonary resuscitation (CPR) is in question, the patient has an illness that is terminal or severe and irreversible, or he or she is permanently unconscious or likely to have cardiac or respiratory arrest. The patient with decisional capacity has the right to give or withhold consent for a DNR order. State law may limit a surrogate's authority to request that CPR be withheld. Remember, a DNR order does not restrict a patient's access to intensive care. Nurses, patient advocates, social workers, and clergy me

Family satisfaction with the end-of-life discussions and resuscitation orders for patients with severe COVID-19: a multicenter study in Saudi Arabia.

BACKGROUND: Despite the significant mortality, data on end-of-life (EoL) discussions during COVID-19 pandemic are scarce. Health care providers had to cope with significant challenges, particularly overcoming barriers to effective communication with patients and their families, and lack of information on therapy and prognosis. This study was undertaken to assess the satisfaction of close family members (CFMs) with EoL discussions for patients hospitalized with severe COVID-19 infection. METHODS: The study was conducted in five major hospitals in Saudi Arabia. Medical charts of consecutive pat

Impact of the COVID-19 pandemic on the care of hospitalised patients with do-not-resuscitate orders: a retrospective cohort study.

OBJECTIVES: Do-not-resuscitate (DNR) orders limit the use of cardiopulmonary resuscitation but are often interpreted more broadly. During COVID-19 surges, resource constraints may have increased variability in application, potentially affecting patients with DNR orders. We aimed to determine whether admission during COVID-19 surge months in 2020 versus non-surge months was associated with differences in use of life-sustaining treatments among hospitalised patients with DNR orders. METHODS: Using data from five states (Arizona, California, Iowa, Mississippi and New York), we identified non-sur

Associations between Patient Characteristics and a New, Early Do-Not-Attempt Resuscitation Order after Intracerebral Hemorrhage.

BACKGROUND: Decisions to limit care, including use of a do-not-resuscitate (DNR) order, are associated with increased risk of death after intracerebral hemorrhage (ICH). Given the value that patient surrogates place on the physician's perception of prognosis, understanding prognostic indicators that influence clinical judgment of outcomes is critical. OBJECTIVE: The purpose of this study was to understand the patient variables and comorbid illnesses associated with DNR orders placed on patients within 72 hours after ICH. DESIGN: Single-center, retrospective review of medical records of 198 c

Experiences of bereaved family caregivers of patients with Alzheimer's disease regarding do-not-resuscitate orders: A qualitative study.

BACKGROUND: This study aimed to explore the end-of-life decision-making experiences of bereaved family caregivers of Alzheimer's disease (AD) patients, focusing on do-not-resuscitate orders. Given the high emotional and ethical burden on caregivers, understanding their challenges and needs is crucial to enhancing palliative care for AD patients. METHODS: A qualitative, exploratory study was conducted using semi-structured interviews with 22 family caregivers recruited through purposive sampling in central Taiwan. Participants were primary caregivers for AD patients who had been bedridden for

Guidelines for the appropriate use of do-not-resuscitate orders. Council on Ethical and Judicial Affairs, American Medical Association.

Cardiopulmonary resuscitation (CPR) is routinely performed on hospitalized patients who suffer cardiac or respiratory arrest. Consent to administer CPR is presumed since the patient is incapable at the moment of arrest of communicating his or her treatment preference, and failure to act immediately is certain to result in the patient's death. Two exceptions to the presumption favoring CPR have been recognized, however. First, a patient may express in advance his or her preference that CPR be withheld. If the patient is incapable of expressing a preference, the decision to forgo resuscitation m

Going Beyond the Do-Not-Resuscitate Order: Comparing the Care Received by Cancer Patients with Respect to Hospice Care Needs.

OBJECTIVES: The purpose of this study is to compare the treatment given to cancer patients in intensive care units (ICUs) who require or do not require hospice care. MATERIALS AND METHODS: A descriptive retrospective design was used. A total of 193 files were reviewed for admitted cancer patients. RESULTS: Individuals requiring hospice care had longer hospital stays (t=1.22, p<.05) and were less likely to be sent to palliative care (X2= 183.5, p<.05). The majority of patients were intubated (72.4%), got intravenous hydration (84.5%), and received antibiotics (81.3%). There was a statisticall

Death and do-not-resuscitate order in the emergency department: A single-center three-year retrospective study in the Chinese mainland.

BACKGROUND: Consenting to do-not-resuscitate (DNR) orders is an important and complex medical decision-making process in the treatment of patients at the end-of-life in emergency departments (EDs). The DNR decision in EDs has not been extensively studied, especially in the Chinese mainland. METHODS: This retrospective chart study of all deceased patients in the ED of a university hospital was conducted from January 2017 to December 2019. The patients with out-of-hospital cardiac arrest were excluded. RESULTS: There were 214 patients' deaths in the ED in the three years. Among them, 132 patie

An Electronic Health Record-Assisted Patient Safety Intervention to Identify Missing Code Status Orders in Patients with Prior Do-not-Attempt-Resuscitation or Partial Code Status.

BACKGROUND: Hospitalized patients with serious illnesses often lack code status documentation, increasing the risk of goal-discordant care during hospitalization. LOCAL PROBLEM: Patients with a prior preference for do-not-attempt-resuscitation (DNAR) sometimes lacked a code status order during subsequent encounters, creating a patient safety risk of undesired resuscitation in the event of cardiac arrest. INTERVENTION: In May 2025, we developed an electronic health record (EHR) alert to prompt code status discussion for hospitalized medicine patients with prior DNAR or partial code. METHODS:

Early do-not-resuscitate orders in intracerebral haemorrhage; frequency and predictive value for death and functional outcome. A retrospective cohort study.

INTRODUCTION: In former studies from North America early Do-Not-Resuscitate orders (DNR orders) in patients with intracerebral haemorrhage (ICH) had negative prognostic impact on mortality. The influence of DNR orders on functional outcome and whether DNR orders are grounded on relevant patient characteristics is unknown. We aimed to determine the frequency and predictive factors of DNR-orders and its association to prognosis, in ICH patients, in Scandinavia. METHODS: In 197 consecutive ICH patients admitted to Skåne University Hospital, Malmö, Sweden, between January 2007 and June 2009, info
