AIM: To report a case of choroidal effusion and exudative retinal detachment following a non perforating Yttrium-Aluminium-Garnett (YAG)-laser iridotomy.
DESIGN: Case report.
METHODS: A 53-year-old woman complains of sudden onset of blurred vision in her left eye 15 days after the attempt of YAG-laser peripheral iridotomy. Clinical examination revealed 3+ flare and 1+ cells in the anterior chamber, 2+ vitreous cells, swollen optic nerve, ciliochoroidal effusion, and exudative retinal detachment involving macular area in the left eye. After starting treatment with prednisone 25 mg once daily,

Surgical emphysema is defined as gas or air trapped in the subcutaneous tissue plane. Here, we report a rare case of bilateral periorbital and cervicofacial subcutaneous emphysema following a vitreoretinal surgery for inadvertent globe perforation during the administration of peribulbar anesthesia. This condition, although self-resolving when restricted to the subcutaneous plane has the potential to spread into deeper tissue planes such as the retropharyngeal space. The presence of crepitus helps to distinguish it from angioneurotic edema. Ophthalmologists must be sensitive to the fact that su

In 1977-1987 the authors operated by the cryosurgical method 69 patients on account of detachment of the retina. The detachment occurred in 37 patients after simple perforation of the eye and in 32 patients after perforation with a foreign intraocular body. In all patients the perforation penetrated as far as the vitreous body. In the first part of the paper the authors analyze in detail anamnestic data related to the injury: age, mechanism of injury, rapidity and method of surgical treatment, chemical nature of foreign body and its size, rapidity and method of extraction of the foreign body.

PURPOSE: We describe the management of Hallermann-Streiff syndrome in monozygotic female twins with congenital cataracts, exudative retinal detachments, and 1 case of corneal descemetocele with associated dellen and subsequent perforation.
METHODS: This study was a case report and review of the literature.
RESULTS: Twins 1 and 2 exhibited all 7 cardinal characteristics of Hallermann-Streiff syndrome, presenting with spontaneous lenticular resorption, anterior uveitis, and glaucoma. They underwent bilateral cataract extraction with near total capsulectomy. Both twins experienced recurrent gla

Globe perforation following peribulbar anesthetic injection is a rare but dreaded complication that often results in poor visual outcomes. This case report is on a female patient who sustained vitreous hemorrhage, retinal detachment, and macular breaks due to a peribulbar block administered in the setting of cataract extraction. The retina was repaired with pars plana vitrectomy, endolaser of the peripheral retinal break only, and an internal limiting membrane inverted flap for the macular breaks to avoid the endolaser on the macular area, achieving stable visual outcomes. The authors discusse

Inadvertent perforation of the sclera is a widely recognized complication of eye muscle surgery. In the 1960s and 1970s, it was estimated to occur in 9% to 12% of patients operated. Fortunately, dreaded vision-threatening sequelae have been much less common. To better define the occurrence and sequelae of this complication, we sent a questionnaire to all 342 members of the American Association for Pediatric Ophthalmology and Strabismus. Scleral perforations, defined to include known retinal damage, occurred in 728 of nearly 554,000 eye muscle procedures performed by 223 surgeons. Perforations

OBJECTIVE: To evaluate the incidence and identify associated risk factors of scleral penetrations and perforations resulting from strabismus surgery.
METHODS: A prospective observational study of patients undergoing strabismus surgery by residents or fellows was conducted. The surgical techniques employed included recessions, resections, advancements, posterior fixation sutures, and transpositions. Indirect ophthalmoscopy was performed on each operated eye following surgery to identify any retinal changes indicative of a penetration or perforation.
RESULTS: Of the 144 patients and 217 operat

BACKGROUND: Asymptomatic retinal breaks and lattice degeneration are visible lesions that are risk factors for later retinal detachment. Retinal detachments occur when fluid in the vitreous cavity passes through tears or holes in the retina and separates the retina from the underlying retinal pigment epithelium. Creation of an adhesion surrounding retinal breaks and lattice degeneration, with laser photocoagulation or cryotherapy, has been recommended as an effective means of preventing retinal detachment. This therapy is of value in the management of retinal tears associated with the symptoms

OBJECTIVE/AIM: Corneal perforations can result from a wide variety of disorders and can lead to devastating visual sequelae. Various surgical procedures have been described to manage nontraumatic corneal perforation. Conjunctival flaps offer an important technique in dealing with such corneal emergencies. We report a modified conjunctival flap procedure referred to as superior forniceal conjunctival advancement pedicle (SFCAP) in the successful management of corneal perforation and impending corneal perforation.
PATIENTS AND METHODS: Out of 20 patients who underwent SFCAP for non-traumatic co

Perforation of the globe is a rare but specific risk of strabismus surgery, as proven by a review of the literature and statistic analyses of the authors. While in the 1960s and 1970s perforations occurred in about 10% of the operations, improved operation techniques and better material have reduced the perforation rate to 1%° at present. This tendency is confirmed by the authors' studies of the periods 1966-1970 (eight clinics; 10,349 strabismus operations; 37 perforations = 3.6%°) and 1989-1993 (23 clinics; 44,211 operations; 67 perforations = 1.5%°). The personal perforation rate of the aut

Rhegmatogenous retinal detachment (RRD) is a potentially blinding separation of neurosensory retina from retinal pigment epithelium by fluid passing from the vitreous cavity into the subretinal space through one or more retinal breaks, holes or tears. Although most RRDs can be repaired surgically with one operation, some visual loss often results, especially if the macula has been detached, and a small fraction of RRDs require multiple surgeries with significant morbidity, including permanent severe visual loss. Vitrectomy, the most popular RRD surgery, is associated with temporary visual loss

A technique has been developed which produces an experimental posterior perforating eye injury, that reproducibly results in traction retinal detachment. The standard injury is an incision through the pars plana with vitreous prolapse and incarceration; the wound is then carefully closed with microsurgical techniques. It appears that blood in the vitreous is an essential factor in the development of vitreous traction and of traction retinal detachment. The model is considered useful for further histological, electron microscopic, electrophysiological, and ultrasonic studies of posterior perfor

PURPOSE: This retrospective study was undertaken to review the clinical signs and management of patients with perforating eye injuries associated with regional anaesthesia for ophthalmic surgery.
METHODS: Fifteen consecutive patients presenting with inadvertent perforation of the globe were evaluated. Eleven required vitreoretinal surgery. The indications were retinal detachment (six eyes), vitreous haemorrhage (four) and an epiretinal membrane (one eye).
RESULTS: Those eyes with retinal detachments generally had a poor functional outcome in spite of the retinae being attached in five of the

Five patients had trauma to the retina that occurred during cataract surgery. In two of the cases the injury was caused by perforation of the globe by the retrobulbar injection needle and in one case the retinal injury occurred when the superior rectus muscle bridle suture perforated the sclera. A stab incision for release of blood in an expulsive choroidal hemorrhage caused a retinal dialysis. Another case of retinal dialysis was caused by aspiration of solid vitreous after vitreous loss.
