Rheumatic diseases in China.

Prevalence and clinical manifestations of 5 common rheumatic diseases in China are discussed. Systemic lupus erythematosus appears to be more prevalent in Chinese than in Caucasians with possibly different clinical manifestations, in particular less serositis. Rheumatoid arthritis may be less aggressive and seronegative arthridities are less common. Behçet's and aortitis are frequently seen but not temporal arteritis and polymyalgia rheumatica.

[Rheumatic diseases and vaccination].

Recommendations for vaccination of patients with rheumatic Diseases. Patients with rheumatic diseases are particularly at risk from infections. Vaccinations are suitable to reduce this risk. Current Recommendations of national (STIKO and Wagner et al.) and international committees (EULAR) provide a basis for the secure and effective use of vaccinations.Important new developments, such as sequential vaccination against pneumococcus or the approval of a vaccine against Herpes Zoster, improve the possibilities of an effective infection protection. Special attention must be paid to the current rec

Calprotectin in rheumatic diseases.

Calprotectin is a heterodimer formed by two proteins, S100A8 and S100A9, which are mainly produced by activated monocytes and neutrophils in the circulation and in inflamed tissues. The implication of calprotectin in the inflammatory process has already been demonstrated, but its role in the pathogenesis, diagnosis, and monitoring of rheumatic diseases has gained great attention in recent years. Calprotectin, being stable at room temperature, is a candidate biomarker for the follow-up of disease activity in many autoimmune disorders, where it can predict response to treatment or disease relaps

[Eye in rheumatic diseases].

Ocular inflammatory diseases are frequently a part of clinical features of numerous systemic rheumatic disorders. Furthermore, appearance of ophthalmic disease can precede the other manifestation of rheumatic disorders and can be the first sign of systemic disease. At the alredy known rheumatic disease, ocular inflammation may demonstrate the grade of systemic disorder. Therefore multidisciplinary approaches, timely and correct diagnosis, as well as appropriate therapy these ocular inflammatory conditions enable visual function protection, health improvement and life saving.

Opioids in rheumatic diseases.

Opioids are the most potent analgesics available and are well established for the treatment of severe acute, surgical, and cancer pain. However, their use to ameliorate chronic noncancer pain (CNCP) is still controversial because of the side effects, such as tolerance, somnolence, confusion, constipation, addiction, withdrawal, and anxiety about disapproval by regulatory bodies. Chronic rheumatic diseases along with low-back pain and arthritis are among the leading causes of CNCP. This article will focus on the role of opioids in chronic rheumatic diseases. Furthermore, the peripheral effects

Osteoimmunology in rheumatic diseases.

This review summarizes the recent advances of osteoimmunology, a new research field that investigates the interaction of the immune system with the skeleton. Osteoimmunology has contributed significantly to the understanding of joint destruction in rheumatoid arthritis and other forms of arthropathies. In particular, the molecular regulation of osteoclast formation and its control by proinflammatory cytokines have helped investigators to understand the mechanisms of bone erosion in rheumatic diseases. Osteoimmunology has also allowed an improvement in our knowledge of the structure-sparing eff

Rheumatic diseases in China.

INTRODUCTION: Epidemiological studies of rheumatic diseases have been conducted during the past 20 years in China. The aim of this study was to clarify prevalence rates of common rheumatic diseases in China. METHODS: Relevant reports of population-based surveys conducted from 1980 to 2006 were retrieved. Studies using the World Health Organization-International League of Associations for Rheumatology COPCORD (Community Oriented Program for Control of Rheumatic Diseases) protocol and those that did not employ this protocol but were published in recognized journals were identified and analyzed.

Autoimmune rheumatic diseases: One or many diseases?

Until the etiopathogenic factor(s) of autoimmune and autoinflammatory rheumatic disorders will be identified, their classification into entities will continue. However, their similar clinical manifestations, overlapping syndromes, evolution from one entity into another, as well as common autoantibody responses, suggest that autoimmune and autoinflammatory disorders may constitute distinct pathophysiologic processes on the basis of a different genetic background. Prognosis and effective therapeutic regimens are mostly based on the clinico-pathologic severity of the involved tissues or organs an

Autoimmune liver diseases in systemic rheumatic diseases.

Systemic rheumatic diseases (SRDs) are chronic, inflammatory, autoimmune disorders with the presence of autoantibodies that may affect any organ or system. Liver dysfunction in SRDs can be associated with prescribed drugs, viral hepatitis, alternative hepatic comorbidities and coexisting autoimmune liver diseases (AILDs), requiring an exclusion of secondary conditions before considering liver involvement. The patterns of overlap diseases depend predominantly on genetic determinants with common susceptible loci widely distributing in both disorders. In AILDs, it is important to identify the ove

Ocular diseases in patients with rheumatic diseases.

To study the distribution of ocular involvement among persons with rheumatic disease, a cross-sectional survey was performed in 224 patients attending the Division of Rheumatology, Department of Medicine, Maharaj Nakorn Chiang Mai Hospital. Of these patients, 102 presented with rheumatoid arthritis, 74 systemic lupus erythematosus, 39 systemic sclerosis, 6 mixed connective tissue disease, 2 polymyositis and 1 juvenile rheumatoid arthritis. It was found that the ocular involvement probably related to diseases including dry eye (19.9%) and uveitis (0.4%). The ocular involvement was presumably re

Psychsomatic aspects of rheumatic, especially soft tissue rheumatic diseases.

Rheumatic diseases may be connected to varying degrees with psychic factors. On the one hand, rheumatic diseases may produce psychic reactions, while on the other hand rheumatic symptoms--especially in extra-articular rheumatism--may be induced by psychosomatic hysteric pararheumatic diseases and depressive mechanisms. Special reference is made to psychosomatically induced extra-articular rheumatic diseases in which not only the complex genesis but also the clinical symptomatology are discussed. During therapy more attention should be directed to psychic factors than in the past, as only an in

Iron metabolism in rheumatic diseases.

Iron is a crucial element for living organism in terms of oxygen transport, hematopoiesis, enzymatic activity, mitochondrial respiratory chain function and also immune system function. The human being has evolved a mechanism to regulate body iron. In some rheumatic diseases such as rheumatoid arthritis (RA), systemic lupus erythematous (SLE), systemic sclerosis (SSc), ankylosing spondylitis (AS), and gout, this balanced iron regulation is impaired. Altered iron homeostasis can contribute to disease progression through ROS production, fibrosis, inflammation, abnormal bone homeostasis, NETosis a

Environmental factors and rheumatic diseases.

The pathogenesis and pathophysiology of rheumatic diseases is complex and relies on the interaction of different factors. The common view is that the pathological autoimmunity develops in genetically predisposed individuals upon exposure to an environmental trigger. This highlights the importance of recognizing and deconstructing the effects of environmental agents in rheumatic diseases. Several factors have been identified in the last decades, with detrimental or protective effects, impacting not only on disease onset, but also on its natural history. Cigarette smoking has been identified as

Myocardial infarction in rheumatic diseases.

Rheumatic diseases, including rheumatoid arthritis, systemic lupus erythematosus, systemic sclerosis, and spondyloarthritides, are chronic systemic disorders marked by persistent inflammation and immune dysregulation. These diseases confer an elevated risk of cardiovascular disease, with myocardial infarction (MI) as a leading cause of increased morbidity and premature mortality. Accumulating evidence suggests that patients with rheumatic diseases experience a 1.5- to 3-fold higher incidence of MI compared with the general population. Chronic systemic inflammation, endothelial dysfunction, oxi

Parvovirus B19 in Rheumatic Diseases.

Parvovirus B19 (B19V) is a human pathogen belonging to the Parvoviridae family. It is widely diffused in the population and responsible for a wide range of diseases, diverse in pathogenetic mechanisms, clinical course, and severity. B19V infects and replicates in erythroid progenitor cells (EPCs) in the bone marrow leading to their apoptosis. Moreover, it can also infect, in an abortive manner, a wide set of different cell types, normally non-permissive, and modify their normal physiology. Differences in the characteristics of virus-cell interaction may translate into different pathogenetic me
