Rhodococcus equi peritonitis in continuous ambulatory peritoneal dialysis.

The human Rhodococcus equi (R. equi) infection is now emerging, although extrapulmonary manifestation and isolation from patients without human immunodeficiency virus (HIV) infection remains unusual. Considerable effort is required to correctly identify and diagnose this facultative pathogen in patients with peritonitis in end-stage renal failure (ESRF) on continuous ambulatory peritoneal dialysis (CAPD). In the six cases of R. equi CAPD peritonitis reported in this series, diagnoses were made, on average, after 15 days and prolonged antibiotic therapy with morbidity in two patients. A diagnos

Antimicrobial Resistance in Rhodococcus equi and the Promise of Synergistic Therapies.

Rhodococcus equi is an opportunistic intracellular pathogen responsible for severe pneumonia in foals and has emerged as an important cause of infection in immunocompromised humans. The treatment of R. equi infections in foals relies mainly on the combination of macrolides and rifampin. However, the increasing incidence of multidrug-resistant (MDR) isolates has raised significant therapeutic challenges. The mechanisms underlying this resistance include mutations in target genes, activation of efflux pumps, and biofilm formation, which collectively compromise the efficacy of conventional antibi

Coinfection by Mycobacterium tuberculosis and Rhodococcus equi in advanced HIV.

A 28-year-old man with HIV (diagnosed eight years prior, last CD4 count 23 cells/mm3) and prior tuberculosis (cured, then confirmedly reinfected with treatment default) presented with cachexia, diarrhea, chest pain, and tachypnea with increased work of breathing that required orotracheal intubation. Admission chest CT showed extensive bilateral consolidations with air bronchograms, suggestive of previous mycobacterial infection and/or possible new bacterial infection. Initially managed as sepsis, blood cultures grew Rhodococcus equi (four samples). Molecular testing confirmed Mycobacterium tub

Comparison of the efficacies of Rhodococcus equi recombinant vaccine in mice.

Rhodococcus equi is an important bacterial pathogen and causes severe chronic granulomatous pneumonia in foals below 6 months of age. It has also become an opportunistic and emerging pathogen in immunocompromised humans. Vaccination is the most cost-effective strategy for controlling and preventing this infection. Although several potential virulence genes and candidate immunogens have been identified over the years, no effective vaccine is currently available to prevent R. equi disease in horses. Recently, bacterial vector vaccines have been shown to be promising for R. equi. In this study, t

Infective Endocarditis Caused by Rhodococcus equi in an Immunocompetent Patient.

Rhodococcus equi (R. equi) is a rare zoonotic organism that is found in the feces of grazing animals and in farm soil. It typically causes pulmonary disease, but it can also cause extrapulmonary disease. Immunocompromised patients are at a higher risk of developing the infection, but it has been reported in individuals with competent immune system as well. We present a unique case of infectious endocarditis (IE) due to a R. equi infection in an immunocompetent patient. A 77-year-old male with a history of coronary artery disease, prior myocardial infarction, systolic heart failure, hypertensio

Development of a multilocus sequence typing scheme for Rhodococcus equi.

Rhodococcus equi causes pulmonary and extrapulmonary infections in animals and humans, with endemic situations and significant young foal mortality in stud farms worldwide. Despite its economic impact in the horse-breeding industry, the broad geographic and host distribution, global diversity and population structure of R. equi remain poorly characterised. In this context, we developed a multilocus sequence typing (MLST) scheme using 89 clinical and environmental R. equi of various origins and eight Rhodococcus sp. Data can be accessed at http://pubmlst.org/rhodococcus/. A clonal R. equi popul

Rhodococcus equi pericarditis in a patient living with HIV/AIDS.

INTRODUCTION: Rhodococcus equi, previously called Corynebacterium equi, is known to cause pneumonia in foals and swine. Although it was known to cause infection rarely in humans, R equi infection in humans has increased with the advent of HIV and increased use of immunosuppressants. CASE: We report a case of a 48-year-old male patient with newly diagnosed HIV/AIDS presenting with recurrent R equi bacteremia, pericardial effusion, and pericardial cyst. The infection was treated with drainage of the pericardial effusion and cyst and 2 weeks of intravenous vancomycin and 6 months of oral azithro

Less Typical Courses of Rhodococcus equi Infections in Foals.

This article aims to present several interesting and less typical courses of Rhodococcus equi infections in foals, collected during the 2019-2021 foaling seasons in some Polish studs. The study was conducted by the Division of Veterinary Epidemiology and Economics, Warsaw University of Life Sciences-SGGW, and concentrated on ultrasonographic contribution to diagnostics and treatment of the disease. Among many standard cases of rhodococcal pneumonia, some rare ones occurred. The aforementioned issues include the potential contribution of rhodococcal infection to a grave outcome in a prematurely

Rhodococcus (R). equi, a recognized pathogen in horses, is emerging as a human opportunistic pathogen, especially in immunocompromized people. It affects also New World camelids, but there are no reports of R. equi infection in Old World camelids yet. Four cases of disseminated R. equi infection in adult breeding dromedaries occurred at one camel farm near Dubai within 16 months of each other. At necropsy the lungs were diffusely consolidated with large caseous areas. Histology revealed severe suppurative to necrotising pneumonia with multiple encapsulated abscesses. Rhodococcus equi infection after reduction mammaplasty in an immunocompetent patient.

The majority of infections caused by R. equi occur in hosts with some degree of cell-mediated immunodeficiency. Immunocompetent individuals are infrequently affected and usually present with localized disease. Infections of the skin or related structures are uncommon and are usually related to environmental contamination. The microbiology laboratory plays a key role in the identification of the organism since it may be mistaken for common skin flora. We describe a 31 year-old woman without medical problems who presented nine weeks after breast reduction with right breast cellulitis and purulen

Effect of Macrolide and Rifampin Resistance on the Fitness of Rhodococcus equi.

Rhodococcus equi is a leading cause of severe pneumonia in foals. Standard treatment is dual antimicrobial therapy with a macrolide and rifampin, but the emergence of macrolide- and rifampin-resistant R. equi isolates is an increasing problem. The objective of this study was to determine the effect of macrolide and/or rifampin resistance on fitness of R. equi Three unique isogenic sets were created, each consisting of four R. equi strains, as follows: a susceptible parent isolate, strains resistant to macrolides or rifampin, and a dual macrolide- and rifampin-resistant strain. Each isogenic se

Diagnosis and management of pulmonary infection due to Rhodococcus equi.

BACKGROUND: Rhodococcus equi is a recognized cause of disease in humans, especially in individuals who are immunocompromised. Because diphtheroids are regarded as part of normal respiratory flora, the importance of R. equi as a pulmonary pathogen may not be fully appreciated and its prevalence may be underestimated. Most treatment recommendations for R. equi infection were established before antiretroviral drugs became available for human immunodeficiency virus/AIDS therapy, and therapeutic strategies may need to be updated. OBJECTIVES: To review the role of R. equi as a cause of pulmonary in

First Detection of Rhodococcus equi in a Foal in Bulgaria-A Case Report.

This case report describes the following scenario: A 56-day-old Arabian filly presented with classical symptoms of respiratory distress and a rapidly deteriorating condition, despite intensive antimicrobial treatment, resulting in death. The post-mortem examination revealed severe bronchopneumonia with characteristic disseminated pus-containing nodules. Microbiological tests and PCR confirmed the presence of R. equi carrying the virulence-associated protein A (VapA) gene. Rhodococcus Equi: Challenges to Treat Infections and to Mitigate Antimicrobial Resistance.

Rhodococcus equi, a gram-positive facultative intracellular pathogen and a soil saprophyte, is one of the most common causes of pneumonia in young foals. It poses a threat to the economy in endemic horse-breeding farms and to animal welfare annually. Many farms use thoracic ultrasonographic screening and antimicrobial treatment of subclinically affected foals as a preventive measure against severe R. equi infections. The wide use antimicrobials to treat subclinically affected foals has contributed to the emergence of multidrug resistant (MDR)-R. equi in both clinical isolates from sick foals a
