Ridership exceedance exposure risk: Novel indicators to assess PM2.5 health exposure of bike sharing riders.

Identifying the fine particulate matter (PM2.5) exposure risk for bicycle riders is crucial for promoting the development of theory and technology in transportation-related air pollution assessment as well as urban health planning. Previous studies have employed daily mean PM2.5 concentrations and designed routes to evaluate air pollution exposure risk. However, because the daily mean PM2.5 concentrations cannot fully illustrate the intra-day variations in PM2.5, which are typically higher than daily mean values, the adverse effects of PM2.5 concentrations remain underestimated. Moreover, the

Retrieval of individual patient data depended on study characteristics: a randomized controlled trial.

OBJECTIVES: The aim of the study was to examine the effect of providing a financial incentive to authors of randomized clinical trials (RCTs) to obtain individual patient data (IPD). STUDY DESIGN AND SETTING: Parallel-group RCT with authors identified in the RCTs eligible for two systematic reviews. The authors were randomly allocated to the intervention (financial incentive with several contact approaches) or control group (using the same contact approaches). Studied outcomes are proportion of authors who provided IPD, time to obtain IPD, and completeness of IPD received. RESULTS: Of the 12

Catastrophic Health Expenditures Associated With Open Reduction Internal Fixation of Distal Radius Fractures.

PURPOSE: Distal radius fracture is one of the most common upper-extremity traumatic injuries. These injuries result in time off work and potential financial consequences for patients. Therefore, we aimed to understand the risk of catastrophic health expenditures (CHEs) after open reduction and internal fixation of distal radius fractures and examine the association between patient characteristics and the risk of CHE. METHODS: We used data from patients undergoing open reduction and internal fixation of a distal radius fracture from a large, urban, level I trauma center (2018-2020). The risk o

The role of innovative financing to bridge the gap between implementation and commercialisation.

The deployment of novel medical products requires more than one party willing and able to shoulder the risk of failure. While GSK's RTS,S/AS01E malaria vaccine received a positive scientific opinion from the European Medicines Agency, further evidence was needed for the World Health Organization (WHO) to be willing to recommend the vaccine for broad use. This increased uncertainty around whether WHO would ultimately issue a recommendation and when and who might buy vaccines to enable the wider scale-up to occur. With no large-scale demand outside of Gavi-financed markets, GSK was faced with a

Beyond binary measures: introducing a safety net hospital index.

INTRODUCTION: Safety net hospitals (SNHs) disproportionately serve uninsured, Medicaid, and low-income Medicare patients, yet no universally accepted SNH definition exists. Existing binary measures frequently identify divergent hospital sets, creating research inconsistency and risking misaligned policy protections, concerns heightened by recent federal Medicaid cuts and site-neutral payment reforms. METHODS: Using Medicare cost report data for community hospitals, we introduce and validate a continuous, multidimensional SNH Index constructed as a composite of Medicaid revenue share, uncompen

"Sharing is my only option": an ethnographic analysis of the underlying contexts of needle and syringe sharing among people who inject drugs using the socio-ecological framework.

BACKGROUND: People who inject drugs (PWID) in Bangladesh are part of a long-standing needle syringe program, which faced challenges in containing the spread of HIV, thus warranting exploration of the drivers of needle and syringe sharing. This article aimed to explore the underlying reasons for needle and syringe sharing among PWID through ethnographic lenses in Dhaka. METHODS: We adopted peer-driven Participatory Ethnographic and Evaluation Research, entailing 6,000 h of observations at service delivery points and drug-injecting spots, 66 in-depth interviews and seven focus groups with PWID

Improving Patient Access to New Drugs in South Korea: Evaluation of the National Drug Formulary System.

This study reviews and evaluates the national drug formulary system used to improve patient access to new drugs by making reimbursement decisions for new drugs as part of the South Korean national health insurance system. The national health insurance utilizes three methods for improving patient access to costly drugs: risk-sharing agreements, designation of essential drugs, and a waiver of cost-effectiveness analysis. Patients want reimbursement for new drugs to be processed quickly to improve their access to these drugs, whereas payers are careful about listing them given the associated fina

High Deductibles are Associated With Severe Disease, Catastrophic Out-of-Pocket Payments for Emergency Surgical Conditions.

BACKGROUND: Out-of-pocket spending has risen for individuals with private health insurance, yet little is known about the unintended consequences that high levels of cost-sharing may have on delayed clinical presentation and financial outcomes for common emergency surgical conditions. METHODS: In this retrospective analysis of claims data from a large commercial insurer (2016-2019), we identified adult inpatient admissions following emergency department presentation for common emergency surgical conditions (eg, appendicitis, cholecystitis, diverticulitis, and intestinal obstruction). Cryptocurrency Trading, Day Trading, and Gambling Behavior: Examining the Moderating Effects of Financially Focused Self-Concept and Gambling Motives.

Cryptocurrency and day trading have grown in popularity over the past decade following the creation of the first cryptocurrency, Bitcoin, in 2009. These activities share important features with gambling, including risking money on an uncertain outcome, a chance of monetary rewards, and the potential to experience harm (e.g., financial or relationship problems). However, little is known about cryptocurrency and day trading engagement in the adult population, including associations with gambling behavior, harm, and psychological factors that might moderate these relationships. We analyzed cross-

Disaggregating catastrophic health expenditure by disease area: cross-country estimates based on the World Health Surveys.

BACKGROUND: Financial risk protection (FRP) is a key objective of national health systems and a core pillar of universal health coverage (UHC). Yet, little is known about the disease-specific distribution of catastrophic health expenditure (CHE) at the national level. METHODS: Using the World Health Surveys (WHS) from 39 countries, we quantified CHE, or household health spending that surpasses 40% of capacity-to-pay by key disease areas. We restricted our analysis to households in which the respondent used health care in the last 30 days and categorized CHE into disease areas included as WHS

Managing the Planned Cessation of a Global Supply Market: Lessons Learned From the Global Cessation of the Trivalent Oral Poliovirus Vaccine Market.

The Polio Eradication and Endgame Strategic Plan 2013-2018 calls for the phased withdrawal of OPV, beginning with the globally synchronized cessation of tOPV by mid 2016. From a global vaccine supply management perspective, the strategy provided two key challenges; (1) the planned cessation of a high volume vaccine market; and (2) the uncertainty of demand leading and timeline as total vaccine requirements were contingent on epidemiology. The withdrawal of trivalent OPV provided a number of useful lessons that could be applied for the final OPV cessation. If carefully planned for and based on

Impact of out of pocket payments on financial risk protection indicators in a setting with no user fees: the case of Mauritius.

BACKGROUND: Mauritius embraces principles of a welfare state with free health care at point of use in any public facilities. However, the health financing landscape changed in 2007 when Private Health Expenditure (PvtHE) surpassed General Government Health Expenditure. PvtHE is predominately out of pocket (OOP) with only 3.4% related to premiums for private insurance. In 2014, Household OOP Expenditure on health accounted for 52.8% of total health expenditure. OOP is known to be regressive and to impact negatively on households' living standards. OBJECTIVES: This paper aims to examine trends

Evaluating air pollution exposure among cyclists: Real-time levels of PM2.5 and NO2 and POI impact.

Although cycling has numerous health benefits, the increased breathing volume and lack of protection from exposure to the environment while cycling poses health risks that cannot be disregarded. Previous studies evaluating the exposure of cyclists to air pollution have typically focused on assessing exposure to a single pollutant or exposure concentrations on specific urban routes, and have not performed a comprehensive assessment considering the distribution of cyclists. The present study used bicycle-sharing big data to conduct a more comprehensive and refined real-time population weighted e

Catastrophic pharmaceutical expenditure in patients with type 2 diabetes in Iran.

OBJECTIVES: This study aimed to assess the financial burden of out-of-pocket (OOP) payments to purchase antidiabetic medicines for type 2 patients in Iran. METHOD: The "budget share" and "capacity to pay" approaches were employed to assess the catastrophic pharmaceutical expenditures of antidiabetic medication therapies. The catastrophic thresholds were adjusted for pharmaceutical sectors. The data was 2019 monthly household expenditures in rural and urban areas, insurance coverages of antidiabetic medicines and patients' out-of-pocket (OOP) payments in 30-day treatment schedules. RESULTS: T

Health-economic evaluation of a novel personalized digital avatar-based anemia management software in hemodialysis patients.

This study aimed to evaluate the cost-effectiveness and financial impact of an anemia management tool (AMT)-a software system that uses real-time blood volume and hemoglobin monitoring data-for adult patients receiving in-center hemodialysis (HD) in the United States. A Markov cohort model was developed to estimate lifetime costs and health outcomes for 1000 in-center HD patients with and without use of AMT. Clinical input parameters, including hemoglobin stability and dose reduction of erythropoiesis-stimulating agents (ESAs), were derived from a randomized controlled trial. The net monetary

Connecting psychosocial and personality characteristics with mental health outcomes. An Italian co-twin control study.

Exposure to stressful life events is common, and it is linked to increased psychological issues. As most likely people respond to stressors depending on environmental and genetic factors, we assessed in a twin study the association of some personal characteristics such as resilience and self-perception with anxiety, depression and stress in the late Covid pandemic period, to verify the underlying genetic and shared familial components. With this design, the strength of the associations was compared between individual-level and intrapair-level analyses. From June 2020 to December 2021, the Ital

Do Gamers Play for Money? A Moderated Mediation of Gaming Motives, Relative Deprivation, and Upward Mobility.

Past research indicates strong monetary motives for gambling often elevate an individual's risk of experiencing symptoms of gambling disorder, with personal relative deprivation (PRD) and upward mobility (UM) identified as key factors in this relationship. Nevertheless, few studies have examined how financial motives, PRD, and UM might interact for people playing modern video games-many of which offer financial incentives to encourage participation. Due to the overlap between gambling and (video) gaming, evidence suggests disordered gambling and disordered gaming might also share similarities.

Out-of-pocket expenditure on childhood infections and its financial burden on Indian households: Evidence from nationally representative household survey (2017-18).

The key objective of this research was to estimate out of pocket expenditure (OOPE) incurred by the Indian households for the treatment of childhood infections. We estimated OOPE estimates on outpatient care and hospitalization by disease conditions and type of health facilities. In addition, we also estimated OOPE as a share of households' total consumption expenditure (TCE) by MPCE quintile groups to assess the quantum of the financial burden on the households. We analyzed the Social Consumption: Health (SCH) data from National Sample Survey Organization (NSSO) 75th round (2017-18). Outcome
