[Is it correct to use neonatal intensive care units as operating rooms?].

AIM: The neonatal intensive care unit (NICU) is used in many centres as operating room in order to avoid the co-morbidities that there produces the movement of critical patients. The motive of this work is to analyze the advantages and disadvantages of this type of surgical interventions. METHODOLOGY: Between January, 2004 and December, 2007 405 newborns were operated in the NICU. The most frequent surgical realized interventions were: deferred closing breastbone (172), placement of ECMO (42), ligation of patent ductus arteriosus (45), laparotomies for necrotising enterocolitis (27), repair o

Audit of intensive care unit admissions from the operating room.

An audit of 265 intensive care unit (ICU) admissions from the operating room was performed for the year 1991. In a quality assurance exercise we identified 34 unanticipated ICU admissions (UIAs) by a retrospective peer review of the medical charts. Of these UIAs, 16 were deemed predictable and seven preventable. Five of the seven potentially preventable UIAs were judged to have had inappropriate intravenous fluid management. This has prompted changes in our education programme. In an assessment of our resource management, we evaluated prospectively collected data on the Apache II scores on the

Health Care Sustainability: Strategies for Operating Rooms and Urogynecologic Procedures.

In 2023, the World Health Organization (WHO) concluded that climate change is the biggest threat to public health. Health care has a large carbon footprint with operating rooms contributing an inordinate amount to this issue. Surgeons have an opportunity to lead sustainability efforts by changing individual practices and collaborating with other stakeholders. Some of these changes that have been suggested include using reusable devices, including gowns, drapes, and specula; updating instrument trays and surgeon preference cards to minimize opening unused instruments; using clean energy sources

Intensive Care Unit Room Characteristics and Association with the Development of Delirium.

INTRODUCTION: Delirium is associated with worse outcomes in critically ill patients. Factors that could reduce delirium are patients' environment and maintaining circadian cycles, but whether certain room characteristics improve the incidence of delirium is unclear. Our objective was to investigate whether the presence of windows or doors in patients' rooms is associated with lower rates of delirium. METHODS: In this retrospective, cohort study, adult, medical patients admitted to the Intensive Care Unit (ICU) between 1 January 2024 and 1 July 2024 were identified. Clinical and room character

In Too Deep: A Point-of-Care Ultrasound (POCUS) Escape Room.

AUDIENCE: Emergency medicine residents and emergency ultrasound fellows. INTRODUCTION: Point-of-care ultrasound (POCUS) is an essential emergency medicine skill that requires hands-on practice and an understanding of anatomy in three-dimensional space.1 Experientially, some common POCUS challenges are identifying foreign bodies in soft tissue, recognizing nerves, and identifying lower extremity veins in relation to other anatomic landmarks. But finding novel ways to challenge and engage advanced learners who have mastered basic POCUS content can be difficult, and this was the impetus of the c

From Operating Room to Intensive Care: A Case of Enterococcus Endocarditis.

Infective endocarditis is a serious infection of the endocardial surface of the heart, most commonly involving the heart valves. Enterococcus species, particularly Enterococcus faecalis, are significant but less common causes of infective endocarditis. Enterococcus can become pathogenic, especially in immunocompromised patients or those with recent surgical interventions. Due to the bacteria's intrinsic resistance to many antibiotics, including cephalosporins, and low-level resistance to aminoglycosides, the treatment can be complicated. This case illustrates the progression of a patient from

Referrals from primary care with foreskin symptoms: Room for improvement.

AIMS: Referrals to secondary care for boys with foreskin symptoms require face-to-face review, resulting in time out of school / work and costs to the family. This study aimed to review outcomes of referrals to ascertain if there was scope to reduce referrals. METHODS: New patients referred to a UK regional paediatric surgery clinic during 2019 were identified and screened retrospectively. Medical records for boys over one year of age referred due to foreskin symptoms were reviewed. RESULTS: Of 2598 referrals, 1939 (75%) were boys & 1094 were > 1 yr; 398 (21%) were referred with foreskin sym

Effect of the COVID-19 Pandemic on Musculoskeletal Care in the Emergency Room.

OBJECTIVE: Study the effect of the coronavirus disease (COVID-19) pandemic on emergency room (ER) utilization for musculoskeletal (MSK) complaints. METHODS: Data from ER encounters in a teaching hospital for years 2019 and 2020 were extracted from an electronic database. The ICD-10-CM codes were used to assign the primary diagnosis. Joinpoint regression was applied to a time-series to detect changes in daily encounters (all-cause and MSK). Total number and median daily encounters were calculated by year, sex, age groups, and diagnostic codes for each year. RESULTS: Overlapping intervals with

Single-centre prospective observational study on postdelivery room care.

OBJECTIVES: As quality of care in the delivery room has major impact on outcome of preterm infants, multiple guidelines have been established in recent years. There is, however, little evidence on how to proceed during postdelivery room care, the time of transfer and admission to the neonatal intensive care unit (NICU). The aim of this study was to identify processes taking place during this period with potential impact on outcome. STUDY DESIGN: Prospective observational study. SETTING: Single-centre German tertiary NICU. PATIENTS: 40 inborn preterm infants undergoing postdelivery room care

Congenital diaphragmatic hernia: intensive care unit or operating room?

Despite improvements in prenatal diagnosis and neonatal intensive care, the Congenital Diaphragmatic Hernia (CDH) Registry still records a 64% survival rate. Many reports demonstrate, however, that approximately 80% of CDH patients with no other malformations may survive if managed with permissive hypercapnia, gentle ventilation, high-frequency oscillatory ventilation (HFOV), surfactant, inhaled nitric oxide (iNO) and extracorporeal membrane oxygenation (ECMO), and delayed surgical repair. We wished to define the evolving outcome of CDH newborns using a protocol approach to management, which i

Transforming Postoperative Care: Advanced Recovery Room Care for Intermediate-risk Noncardiac Surgical Patients.

The risk of death in the first 30 days after noncardiac surgery is 140 times higher than during the procedure itself. While advances in surgery and anesthesia care have reduced intraoperative mortality 10-fold, improvements in postoperative care have lagged behind. Although high-risk patients may be electively admitted to an intensive care unit, most intermediate-risk patients (2 to 4% mortality) are usually transferred from the postanesthesia care unit to general surgical wards, where vital signs are typically assessed every 4 to 6 h, which may delay early recognition of clinical deterioratio

Subspecialty emergency room as alternative model for otolaryngologic care: implications for emergency health care delivery.

PURPOSE: A dedicated otolaryngology emergency room (ER) represents a specialized surgical evaluation and treatment setting that may be an alternative triage pathway for acute otolaryngologic complaints. We aim to characterize practice patterns in this setting and to provide insight into the epidemiology of all-comer, urgent otolaryngologic complaints in the United States. METHODS AND METHODS: Electronic medical records were reviewed for all patients who registered for otolaryngologic care and received a diagnosis in the Massachusetts Eye and Ear Infirmary ER between January 2011 and September

Development and Implementation of a Protocol for In-Room Surgical Procedures in Paediatric Intensive Care.

The Paediatric Intensive Care Unit (PICU) of a Swiss university hospital routinely performs surgical interventions directly in patient rooms due to certain patients' inability to be transferred to the operating room. Given the variability in practice, organizational delays and increased stress among healthcare professionals stemming from oral transmission of procedural knowledge, an institutional protocol was developed. This protocol aims to enhance care safety, improve interdisciplinary coordination and systematically anticipate material human, and environmental needs. Developed through struc

Weakness acquired in the cardiac intensive care unit: still the elephant in the room?

Over the past two decades, the cardiac critical care population has shifted to increasingly comorbid and elderly patients often presenting with nonprimary cardiac conditions that exacerbate underlying advanced cardiac disease. Consequently, the modern cardiac intensive care unit (CICU) patient has poor outcome regardless of left ventricular ejection fraction. Importantly, delayed liberation from organ support, independent from premorbid health status and admission severity of illness, has been associated with increased morbidity and mortality up to years post-general critical care. Although a

Ethical competence: the cornerstone of care in the cardiac operating room, a phenomenological study.

BACKGROUND: Ethical competence leads to the provision of principled and ethical care, which subsequently improves health and increases patient satisfaction. Among these, the cardiac operating room is one of the most challenging and stressful therapeutic environments, in which the ethical competencies of healthcare professionals can significantly impact the performance and quality of care. Therefore, it is critical to describe healthcare professionals' experiences in the operating room regarding ethical competency. The present study aimed to investigate the experiences and perceptions of ethica

Point-of-care high-sensitivity assay on PATHFAST as the backup in the emergency room.

AIM: Although the levels of cardiac troponin I (cTnI) have proved to be a useful diagnostic biomarker of acute myocardial infarction, there are a wide variety of point-of-care (POC) analysers, which provide measurements of cTnI. The aim of this study was to compare the results obtained by the ADVIA Centaur ultra-assay cTnI assay (us-cTnI), ADVIA Centaur high-sensitive cTnI assay (hs-cTnI) and a POC high-sensitivity assay using PATHFAST. We also aimed to explore total turnaround time (TAT) for laboratory results using the POC PATHFAST analyser. METHODS: Samples from 161 patients were taken. Of

Medical management of status epilepticus: Emergency room to intensive care unit.

In convulsive status epilepticus (SE), achieving seizure control within the first 1-2 hours after onset is a significant determinant of outcome. Treatment is also more likely to work and be cost effective the earlier it is given. Initial first aid measures should be accompanied by establishing intravenous access if possible and administering thiamine and glucose if required. Calling for help will support efficient management, and also the potential for video-recording the events. This can be done as a best interests investigation to inform later management, provided adequate steps to protect d
