Patient safety and diagnostic quality are central to modern histopathology practice. Current incident investigation approaches often rely on root cause analysis; however, its routine use as a universal method is insufficient to address the highly variable, cognitive, sociotechnical and multifactorial nature of histopathology errors. In practice, incidents range from simple technical failures to complex system-level events requiring distinct investigative strategies. A key limitation of current practice is the lack of proportionality, where similar methods are applied irrespective of case compl

BACKGROUND AND STUDY AIMS: Post-endoscopy upper gastrointestinal cancer (PEUGIC) is a key performance indicator for endoscopy. PEUGIC represents a delay in diagnosis: a patient has an endoscopy that did not diagnose cancer, and another investigation, usually endoscopy, 3 to 36 months later that diagnoses upper gastrointestinal (UGI) cancer. We describe a national system to identify PEUGIC and undertake root cause analysis.
METHODS: This retrospective national study was undertaken in the English National Health Service (NHS) and consisted of: 1) identification of PEUGIC; 2) development of an o

Cough syncope, an infrequent condition causing sudden transient unconsciousness from coughing, elevates hospitalized patients' fall risk with severe complications. A 59-year-old male admitted for pneumonia and suspected cough syncope fell post-cough in the restroom, resulting in subarachnoid hemorrhage. Root cause analysis revealed that there are systemic gaps in clinical nursing: lack of targeted nursing protocols, training, educational materials, poor medical and nursing staff communication, and suboptimal environment. Interventions included developing nursing guidelines, enhancing staff tra

OBJECTIVES: Analyze a multicenter cohort of deceased patients after pancreatectomy in high-volume centers in France by performing a root-cause analysis (RCA) to define the avoidable mortality rate.
BACKGROUND: Despite undeniable progress in pancreatic surgery for over a century, postoperative outcome remains particularly worse and could be further improved.
METHODS: All patients undergoing pancreatectomy between January 2015 and December 2018 and died postoperatively within 90 days after were included. RCA was performed in 2 stages: the first being the exhaustive collection of data concernin

OBJECTIVE: The Veterans Health Administration (VHA) serves a population with compounding risk factors for opioid misuse, including chronic pain, substance use disorders, and mental health conditions. The objective of this study was to analyze opioid-related adverse events and root causes to inform mitigation strategies associated with opioid prescribing and administration.
METHODS: The researchers conducted a retrospective analysis of root cause analysis reports of opioid overdose events between August 1, 2012, and September 30, 2019. These adverse events were investigated locally by multidis

OBJECTIVE: To describe and assess the implementation and effectiveness of a RCA based program in Urology residency. The Accreditation Council for Graduate Medical Education states programs should encourage patient safety (PS) and provide formal PS education. However, data suggests most programs fall short. Root Cause Analysis (RCA) has been established as an effective method for event analysis and PS.
METHODS: A RCA program was designed to analyze and discuss PS events in our department. This began with an educational session about RCA methodology. Subsequently, teams composed of faculty and

This quality improvement project describes 22 OR patient falls reported in the Veterans Health Administration between January 2010 and February 2016. Most (n = 15; 68%) involved patient falls from the OR bed. Other patient falls (n = 6; 27%) occurred when the patient was transferred to or from the OR bed, and one fall (5%) occurred at another time. Root causes of the falls included tilting of the OR bed, issues with safety restraints, malfunctioning OR bed or gurney locks, inadequate patient sedation, and poor communication among team members. One fall (5%) resulted in a major injury, four fal

BACKGROUND: Advances in molecular biology and changes in microbial nomenclature may subject diagnostic microbiology to errors. A patient diagnosed with Pneumocystis jiroveci pneumonia and then with AIDS had received a negative "AIDS test"--"negative for antibodies to HTLV 1 and 2." The test requisition showed that the physician had requested HTLV-I/II testing but not an HIV-1/2 test. A root cause analysis was performed to determine if the erroneous testing represented a systemic problem. A study was conducted to identify and address such testing errors.
METHODS: For the 1,952 HTLV-I/II test r

BACKGROUND: Medical errors threaten patient safety. Although root cause analysis (RCA) is commonly used to investigate medication errors, such errors often recur, posing ongoing risks to patient safety. The RCA2 framework, introduced by the National Patient Safety Foundation, aimed to strengthen traditional RCA by emphasizing action-oriented outcomes. To further enhance sustainability, we implemented RCA3-an iterative process adding follow-up audits to evaluate long-term effectiveness of corrective actions. We aim to highlight RCA3's ability to maintain the effectiveness of interventions in pr

Significant progress has been made in the past two decades understanding how child molesters gain access to children and molest them while manipulating others to not get caught. However, incidents of child sexual abuse in schools by educators, and by other children, continue. This manuscript suggests that a comprehensive solution involves two components: (1) using root cause analysis methodology to systematically identify and integrate repetitive causes; (2) to use the findings from voluminous occupational safety research focusing on low-frequency but high-intensity events. Additionally, this

Efficient allocation of deceased donor organs depends upon effective prediction of immunologic compatibility based on donor HLA genotype and recipient alloantibody profile, referred to as virtual crossmatching (VCXM). VCXM has demonstrated utility in predicting compatibility, though there is reduced efficacy for patients highly sensitized against allogeneic HLA antigens. The recently revised deceased donor kidney allocation system (KAS) has increased transplantation for this group, but with an increased burden for histocompatibility testing and organ sharing. Given the limitations of VCXM, we

Delays in diagnosis and treatment are widely considered to be threats to outpatient safety. However, few studies have identified and described what factors contribute to delays that might result in patient harm in the outpatient setting. We analyzed 111 root cause analysis reports that investigated such delays and were submitted to the Veterans Affairs National Center for Patient Safety in the period 2005-12. The most common contributing factors noted in the reports included coordination problems resulting from inadequate follow-up planning, delayed scheduling for unspecified reasons, inadequa

INTRODUCTION: Many hospital settings are adopting a zero-tolerance policy towards pressure injury (PI) development; this requires good planning and the implementation of care, as the incidence of PIs reflects the quality of care given in a hospital or facility.
AIM: To identify common contributing factors towards the development of PIs in a geriatric rehabilitation hospital and improve patient safety through the reduction of hospital-acquired PIs.
METHOD: This was done using root cause analysis (RCA). All patients who developed a Stage 3 or 4 deep tissue injuries or unstageable hospital-acqu

Cushing's disease (CD) is a condition characterized by excess glucocorticoid from an ACTH pituitary adenoma. Identifying surgical candidates represents a major diagnostic challenge. We performed a root cause analysis (RCA) of treatment failures for patients with suspected CD. The present study aims to categorize failures in treatment. Medical records were reviewed from 2008 to 2017 for all patients treated surgically for suspected CD. Demographics, past medical history, endocrine outcomes, imaging findings, laboratory studies and clinical features were collected. Eighty-five patients were iden

Endotracheal tube fire during laryngeal surgery is a rare complication but one that has serious consequences. Surgeons, anesthesiologists and others involved with this type of surgery should become familiar with how to manage this difficult situation, which should be considered a sentinel event requiring prompt analysis of the root cause and surrounding circumstances. Measures to improve management should be implemented and training provided in order to prevent the recurrence of a similar unfortunate event. We report a case in which a patient's airway caught fire during use of an electrocauter

OBJECTIVE: A panel of experts in pain medicine and public policy convened to examine root causes and risk factors for opioid-related poisoning deaths and to propose recommendations to reduce death rates.
METHODS: Panelists reviewed results from a search of PubMed and state and federal government sources to assess frequency, demographics, and risk factors for opioid-related overdose deaths over the past decade. They also reviewed results from a Utah Department of Health study and a summary of malpractice lawsuits involving opioid-related deaths.
RESULTS: National data demonstrate a pattern of
