The rumination severity index: Development and evaluation of a scoring tool for rumination syndrome.

OBJECTIVES: Rumination syndrome (RS) is increasingly recognized in pediatrics; however, no tool exists to measure RS severity in children. Thus, the rumination severity index (RumSI) was developed and validated to measure rumination severity in children. METHODS: We developed a seven-item patient-reported questionnaire, the RumSI, to evaluate RS symptoms and impact on functioning based on clinical experience of pediatric gastroenterologists and psychologists who specialize in caring for children with RS. Patients with RS evaluated at our institution or their parents/caregivers were asked to c

The prevalence of rumination syndrome and rumination disorder: A systematic review and meta-analysis.

BACKGROUND: Rumination is characterized by the repeated regurgitation of food. Rumination syndrome is a disorder of gut-brain interaction diagnosed by Rome criteria, whereas rumination disorder is a feeding and eating disorder diagnosed by DSM-5 criteria. We aimed to determine the global prevalence of rumination according to these criteria across all age groups. METHODS: We performed a systematic review and meta-analysis of studies reporting the prevalence of rumination syndrome according to Rome III and Rome IV and rumination disorder according to the following validated DSM-5 assessments: P

How to recognize and treat rumination syndrome.

PURPOSE OF REVIEW: Rumination syndrome (RS) is a functional gastroduodenal disorder characterized by repeated effortless regurgitation or vomiting of recently ingested food without retching. RS generally has been considered a rare entity. However, it has been increasingly recognized that many RS patients are likely to be underdiagnosed. This review discusses how to recognize and manage RS patients in clinical practice. RECENT FINDINGS: A recent epidemiological study that included over 50,000 individuals found that the prevalence of RS around the world is 3.1%. In patients with proton pump inh

Rumination Syndrome and Dental Erosions in Children.

Rumination syndrome is the effortless regurgitation of recently ingested food with subsequent reswallowing or spitting out. Dental erosion (DE) affects 2% to 5% of the population. DE is defined as loss of tooth structure by a chemical process that does not involve bacteria. Our objective was to compare the frequency of DE among children with rumination syndrome with healthy controls. We enrolled 30 patients 4 to 21 years of age diagnosed with rumination syndrome, and 30 age- and sex-matched healthy control subjects. Patients were evaluated by pediatric dentists for presence of DE with Taji et

Rumination and vomiting in Prader-Willi syndrome.

Inability to vomit has been cited as characteristic of Prader-Willi syndrome (PWS). Although post-prandial vomiting after gastric by-pass surgery has been reported, neither vomiting under "typical" circumstances or rumination have been described. Prompted by the discovery of several cases of vomiting and rumination, a questionnaire was sent to members of the PWS Association. Approximately 36% (113/313) of affected individuals reportedly experienced at least one episode of vomiting. Induced vomiting was unsuccessful in 9 of 14 cases in whom results were known. However, no complications of Ipeca

Global Prevalence and Impact of Rumination Syndrome.

BACKGROUND & AIMS: Rumination syndrome is a Disorder of Gut-Brain Interaction (DGBI) of unknown etiology. We aimed to assess its global prevalence and potential associations with other medical conditions. METHODS: Data were collected via the Internet in 26 countries. Subjects were evenly distributed by country, sex, and age groups and were invited for a "health survey" using the Rome IV diagnostic questionnaire and a supplementary questionnaire addressing factors potentially associated with DGBI. RESULTS: In all, 54,127 subjects completed the survey (51% male; mean age, 44.3 years). The over

Worry and anger rumination in fibromyalgia syndrome.

The aim of the present study was twofold: 1) to investigate the psychological profile of patients with fibromyalgia syndrome (FS) as compared to patients with other chronic pain syndromes (CP) and healthy subjects (HS); 2) to examine the associations between anxiety, depression, worry and angry rumination in FS patients. FS patients (N=30), CP patients (N=30) and HS (N=30) completed measurements of anxiety, depression, worry and angry rumination. FS patients showed higher levels of state and trait anxiety, worry and angry rumination than CP patients and HS, and higher levels of depression than

Rumination syndrome in ethiopia: a case study.

Eating disorders are commonly believed to be rare or nonexistent in Africa. However, due to exposure to Western culture, a rise in eating disorders among African women is reported in the literature. This case study describes a 17-year-old Ethiopian girl who meets the DSM-IV-TR and DSM-5 diagnostic criteria for bulimia nervosa and the Rome III Diagnostic Criteria for Functional Gastrointestinal Disorders criteria for rumination syndrome. The article discusses the diagnostic delays, the difficulties in terms of therapy, and the context determinants that-combined with individual psychopathologica

Objective manometric criteria for the rumination syndrome.

OBJECTIVES: The rumination syndrome is a behavioral disorder resulting in recurrent regurgitation of undigested food. The diagnosis of this syndrome is currently based on clinical features. We aimed to determine criteria for the rumination syndrome based on physiological measurements. METHODS: We studied patients with clinically confirmed rumination syndrome and gastroesophageal reflux disease (GERD) patients with predominant symptoms of regurgitation. All patients underwent combined high-resolution manometry and pH-impedance measurement after a standardized meal. All reflux events extending

Evaluating Trends In Rumination Syndrome: A Retrospective Analysis.

Rumination syndrome is a functional gastrointestinal disorder marked by effortless postprandial regurgitation, typically occurring 10-15 min after meals. The condition, first noted in 1618, is now diagnosed by the Rome IV criteria through clinical symptoms in the absence of structural abnormalities. Diagnostic confirmation through high-resolution manometry reveals characteristic patterns of gastric pressurization and muscle coordination, while treatment primarily involves behavioral and medical therapies. This study evaluates the epidemiology, symptom profiles, time to diagnosis, and treatment

Belching Disorders and Rumination Syndrome: A Literature Review.

BACKGROUND: Belching disorders and rumination syndrome (RS) are disorders of gut-brain interaction (DGBIs) in Rome IV. Belching disorders are composed of excessive gastric belching (GB) and supragastric belching (SGB). Excessive GB is related to physiological phenomenon whereas excessive SGB and RS are behavioral disorders. SUMMARY: A recent large internet survey found that prevalence of belching disorders and RS were 1% and 2.8%, respectively. It has been recognized that not a few patients with two behavioral disorders, excessive SGB and RS, could be misdiagnosed as proton pump inhibitors (P

Rumination syndrome: when the lower oesophageal sphincter rises.

BACKGROUND: Rumination syndrome is an uncommon condition characterised by the self-induced regurgitation from the stomach to the mouth of recently ingested meal that is chewed and reswallowed. Rumination is caused by a voluntary rise in intra-abdominal and intra-gastric pressure leading to the reflux of the gastric content into the oesophagus. However, the precise mechanisms preventing reflux at the gastro-oesophageal junction during the rise in intra-gastric pressure remains unknown. METHODS: In 5 patients, rumination episodes were monitored using combined multiple intra-luminal impedance mo

Severe pediatric rumination syndrome: successful interdisciplinary inpatient management.

BACKGROUND: Rumination syndrome is a condition that occurs when people constantly regurgitate and expel or reswallow food soon after they eat. The most severe cases of rumination syndrome can be debilitating, requiring total parenteral nutrition or enteral tube feedings. We report our experience with the treatment of children with severe rumination syndrome. PATIENTS AND METHODS: Five patients with severe rumination syndrome received a novel inpatient interdisciplinary approach, which involved pediatric psychology, pediatric gastroenterology, clinical nutrition, child life, therapeutic recrea

[Rumination syndrome in an adult with normal intelligence].

A 32-year-old woman visited our department because of repeated regurgitation of food, and was diagnosed with rumination syndrome based on possession of symptoms typical for it. Although rumination syndrome is classified as a functional esophageal disorder according to the Rome II criteria, it is not well known in Japan, and there have been no previous reports of it in an adult with normal intelligence. Since patients with rumination syndrome are often misdiagnosed and receive unnecessary treatment, awareness of it by physicians is important.

Rumination syndrome in children treated by increased holding.

Rumination syndrome--the frequent regurgitation of previously ingested food into the mouth where it is chewed--is a common, life-threatening disorder of retarded individuals. Four cases are described in which holding a retarded, ruminating child for 10-15 min before, during, and after meals, was associated with remission of rumination. Simple holding was effective in three; in the fourth, it was necessary to punish the child by putting her into a separate room for 3 min immediately after regurgitation. A within-subject reversal experimental design suggested that holding and not simple distract

Current diagnosis and management of the rumination syndrome.

The rumination syndrome is a behavioral condition characterized by postprandial regurgitation. In contrast to what many think, the disorder does not exclusively occur in mentally disabled patients or children but also in otherwise healthy adults. As symptoms of postprandial regurgitation are often mistaken for gastroesophageal reflux disease or vomiting, the rumination syndrome is an underappreciated condition. Rumination episodes are caused by an intragastric pressure increases which forces the gastric content into the esophagus and mouth and occurs during 3 distinct mechanisms: primary rumin
