The impact of public health service on rural children's health: Quasi-experimental evidence from China.

OBJECTIVES: This study evaluates the impact of a major maternal and child health service program on the health outcomes of children in rural China. STUDY DESIGN: Quasi-experimental study. METHODS: We utilize data from the China Family Panel Studies (CFPS) spanning 2012 to 2018, leveraging a regression discontinuity (RD) design with March 2006 as the cutoff to identify the program's causal effects. Our empirical strategy includes a series of robustness checks, such as RD validity tests, alternative bandwidths and functional forms, placebo cutoffs, exclusion of a confounding policy, and a comb

Ethnic Disparities in Utilization of Maternal and Child Health Services in Rural Southwest China.

BACKGROUND: Studies in China on ethnic disparities in access to health care in remote and rural population remain insufficient. This study aimed to assess the disparities in utilization of maternal and child health (MCH) services, including antenatal care (ANC), hospital birth, child growth monitoring, and immunization compliance between Han and ethnic minority women in Yunnan Province. METHODS: A multi-stage sampling scheme was used to randomly recruit women from 40 townships in 14 remote prefectures of extremely remote areas in Yunnan. From birth records, we identified and recruited 303 Han

Perceptions and use of maternal health services by women in rural coastal Madang Province.

Maternal mortality remains exceptionally high in Papua New Guinea (PNG) at 733 per 100,000 live births. There has been little, if any, improvement in maternal mortality or maternity services since the 1980s. In 1992-1993 a survey of 550 women in rural coastal areas of Madang Province was undertaken to investigate the prevalence of maternal risk factors and parous women's utilization of and attitudes towards the existing health services. Women were classified as at risk on the basis of previous obstetric complications, parity, stillbirths and neonatal deaths. On this basis 67% of women were cla

The Perceived Stigma of Mental Health Services Among Rural Parents of Children With Psychosocial Concerns.

OBJECTIVE: To examine parents' perceptions of stigma regarding mental health services for their child, consider stigma in the context of novel service delivery settings (e.g., telehealth, primary care, and schools), and evaluate stigma with other factors known to influence service access. METHODS: 347 caregivers of children with psychosocial concerns completed surveys regarding their perceptions of stigma, service delivery settings, and barriers to care. RESULTS: Parents endorsed low levels of stigma around services. Greater perceived stigma was related to less willingness to seek services i

Has India's national rural health mission reduced inequities in maternal health services? A pre-post repeated cross-sectional study.

BACKGROUND: In 2005, India launched the National Rural Health Mission (NRHM) to strengthen the primary healthcare system. NRHM also aims to encourage pregnant women, particularly of low socioeconomic backgrounds, to use institutional maternal healthcare. We evaluated the impacts of NRHM on socioeconomic inequities in the uptake of institutional delivery and antenatal care (ANC) across high-focus (deprived) Indian states. METHODS: Data from District Level Household and Facility Surveys (DLHS) Rounds 1 (1995-99) and 2 (2000-04) from the pre-NRHM period, and Round 3 (2007-08), Round 4 and Annual

Creating a Feasible, Safe and Sustainable Penicillin Oral Challenge Program in a Rural Health Service.

PROBLEM: Programs that identify people with a low-risk allergy who can be safely delabeled by a direct oral challenge have emerged but are mostly reported in tertiary centres. There is limited information in the rural setting where resources are more constrained. SETTING: A medium-sized rural health service in Victoria (Australia). KEY MEASURES FOR IMPROVEMENT: The outcome measure was the number of patients with a low-risk penicillin allergy who completed a penicillin oral challenge. STRATEGIES FOR CHANGE: The penicillin oral challenge program developed considered four critical workflows: c

Health Literacy, Service Readiness, and Community Reinforcement of Rabies-Prevention Behaviors in Rural Thailand.

Background: Rabies is almost invariably fatal once clinical symptoms develop, yet it is preventable through canine vaccination and timely post-exposure prophylaxis (PEP). In rural Thailand, preventive behaviors likely depend on health literacy and contextual conditions that enable and reinforce protective action, but structural pathways remain unclear. Methods: We conducted a cross-sectional study among 750 adults in rabies-risk areas of Si Sa Ket Province, Thailand. A socio-ecological, One Health-informed structural equation model (SEM) examined associations among rabies-related health litera

Maternal-child health and family planning: user perspectives and service constraints in rural Bangladesh.

This article presents a microanalysis of interactions between female fieldworkers and women in rural Bangladesh, and a discussion of the broader organizational constraints that hamper service delivery. It is argued that the fieldworker, herself a rural woman, is faced with considerable demand for both maternal-child health (MCH) and reproductive health care services, but that operational constraints prevent her from realizing her potential in both of these areas. Qualitative data show that, in the eyes of rural women, contraceptive use and health care are intricately intertwined, and that this

Bridging the service gap: health facility challenges in HPV vaccine uptake in rural Uganda.

BACKGROUND: Cervical cancer remains a major cause of morbidity and mortality among women in Uganda. Although the Human Papillomavirus (HPV) vaccine is highly effective in preventing cervical cancer, completion of the recommended two-dose schedule remains low, particularly in rural settings. Rukiga District was selected for this study due to persistently low HPV second-dose (HPV2) completion rates compared with national targets. This study assessed health facility-level barriers influencing HPV vaccine completion among adolescent girls aged 9-14 years in rural Uganda. METHODS: A mixed-methods

Elective abortion as a primary health service in rural India: experience with manual vacuum aspiration.

India allows abortion up to 20 weeks of pregnancy but places restrictions on abortion facilities and providers. Abortion services are especially deficient in rural areas. Although vacuum aspiration is safer, sharp curettage continues to be used by providers as they lack relevant training. This paper describes the provision of first trimester abortion services using manual vacuum aspiration (MVA) in a rural clinic in the state of Rajasthan over a four-year period. Non-use of anaesthesia increased safety and allowed women to return early to a normal routine. Of 534 women, none suffered major com

Assessing health-regulating ecosystem services in rural landscapes: An integrated approach using physiological and visual semantic data.

To support health-oriented rural revitalization, effective management of rural green infrastructure requires robust tools for quantifying restorative ecosystem services and informing evidence-based design. This study develops and tests a multimodal evaluation framework that integrates subjective mood assessment (POMS), physiological stress indicators (GSR and HRV), and deep-learning-based visual semantic analysis (SegFormer) to examine how landscape structure relates to health-relevant restorative responses. The framework was applied in four rural landscape typologies-flat-farmland, hilly-fore

Can Traditional Health Practitioners Deliver HIV Counseling and Testing Services? A Pilot Study in Rural South Africa.

INTRODUCTION: In 2021, 7.2 million South Africans were living with HIV. Uptake of HIV testing is undermined by a combination of factors, including HIV stigma, distrust in the health system, skepticism toward allopathic medicine, preference for traditional medicine, and long distances to the nearest health facility. METHODS: We conducted a single arm pilot trial to determine the acceptability and feasibility of training traditional healers to conduct rapid HIV counseling and testing in rural South Africa. Seventeen healers were enrolled in a nationally recognized training program with the goal

Determinants affecting utilisation of health services and treatment for children under-5 in rural Nepali health centres: a cross-sectional study.

BACKGROUND: Large inequalities in child health remain in Nepal, with caste, ethnicity and sex being major determinants of deprivation and negative outcomes. The purpose of this study was to explore whether key demographics of under 5s were associated with health seeking behaviours, utilisation of health care, and treatment received. METHODS: Data came from Integrated Management of Neonatal & Childhood Illness (IMNCI) records of 23 health centres across five districts. After digitising the paper records, the data was analysed by district, caste/ethnicity, sex, and age to investigate difference
