Evaluating large-scale propensity score adjustment when sample size is small.

BACKGROUND: Large-scale propensity score (LSPS) models are increasingly used to control confounding in observational studies, but their reliability in small-sample settings is unclear. Small samples can limit a study's ability to estimate propensity scores accurately and achieve adequate covariate balance, raising concerns about insufficient confounding adjustment. Resulting in small datasets being excluded, despite potentially containing valid information. METHODS: We evaluated the balancing and bias reduction performance of LSPS models using six target-comparator pairs. To emulate a distrib

Randomization in phase II trials: No exemption based on sample size.

Conducting randomized clinical trials (RCTs) is challenging. For this reason, in earlier phases of drug development and rare diseases with limited sample sizes, there is a tendency to omit control groups and to replace them with historical information for perceived feasibility arguments. This has significant implications for interpretation of trial results. We present headline results from a trial in stem cell transplantation, where fludarabine and clofarabine were investigated for induction chemotherapy in high-risk acute myeloid leukaemia or advanced myelodysplastic syndrome. The trial was i

EVALUATING SAMPLE-SIZE EFFICIENCY AND SENSITIVITY OF TRACTOMETRY IN ALZHEIMER'S DISEASE.

Tractometry allows quantitative analysis of white matter microstructure along the brain's fiber tracts, but the impact of study design parameters-such as sample size and along-tract resolution-on sensitivity and specificity is not well understood. In this study, we conducted tractometry bootstrap analysis using linear-mixed models across four diffusion tensor imaging (DTI) metrics to systematically evaluate how these factors affect the detection of dementia- and amyloid-related effects. While coarser along-tract segments yield greater sensitivity and higher mean effect sizes, finer segments te

Systematic review of shoulder arthroplasty outcomes: what sample size is meaningful?

BACKGROUND: Shoulder arthroplasty is increasingly performed for shoulder conditions such as arthritis, rotator cuff arthropathy, and traumatic injuries. Registries and other compilations of patient data provide the opportunity to detect meaningful differences in outcomes between alternative techniques and implants. A wide range of outcome measurements are reported after shoulder arthroplasty, but the sample sizes needed to identify meaningful differences have not been studied systematically. This review systematically analyzes common clinical outcomes reported for shoulder arthroplasty and rep

How Sample Size Impacts Probabilistic Stimulation Maps in Deep Brain Stimulation.

Probabilistic stimulation maps of deep brain stimulation (DBS) effect based on voxel-wise statistics (p-maps) have increased in literature over the last decade. These p-maps require correction for Type-1 errors due to multiple testing based on the same data. Some analyses do not reach overall significance, and this study aims to evaluate the impact of sample size on p-map computation. A dataset of 61 essential tremor patients treated with DBS was used for the investigation. Each patient contributed with four stimulation settings, one for each contact. From the dataset, 5 to 61 patients were ra

Sample size and power determination when limited preliminary information is available.

BACKGROUND: We describe a novel strategy for power and sample size determination developed for studies utilizing investigational technologies with limited available preliminary data, specifically of imaging biomarkers. We evaluated diffuse optical spectroscopic imaging (DOSI), an experimental noninvasive imaging technique that may be capable of assessing changes in mammographic density. Because there is significant evidence that tamoxifen treatment is more effective at reducing breast cancer risk when accompanied by a reduction of breast density, we designed a study to assess the changes from

Do histopathologic findings improve by increasing the sample size in cholecystectomies?

BACKGROUND: Gallbladder diseases present with cholelithiasis in a wide spectrum of adenomas and cancers. Two or three specimens are sampled in cholecystectomies in routine pathology practice. The aim of this study was to investigate the increase in frequency of histologic alterations in cholecystectomies, particularly precancerous lesions, by increasing the sample size to understand the carcinoma pathway. METHODS: Cholecystectomies of 432 patients with pathology records and materials from two medical centers were collected, and two groups were created. Initial data with two or three samples w

Sample size determination for matched-pair equivalence trials using rate ratio.

In this article, we compare Wald-type, logarithmic transformation, and Fieller-type statistics for the classical 2-sided equivalence testing of the rate ratio under matched-pair designs with a binary end point. These statistics can be implemented through sample-based, constrained least squares estimation and constrained maximum likelihood (CML) estimation methods. Sample size formulae based on the CML estimation method are developed. We consider formulae that control a prespecified power or confidence width. Our simulation studies show that statistics based on the CML estimation method general

Usefulness of an increase in size of motor unit potential sample.

OBJECTIVE: It is known that the sensitivity of quantitative electromyographic (EMG) analysis of motor unit potentials (MUPs) improves with an increase in MUP sample size to more than 20. However, no normative data and estimate of sensitivity have been published. METHODS: In the present study sample sizes of 5, 10, 15, 20, 30 and 40 MUPs were obtained from the external anal sphincter (EAS) muscles of 81 controls and 70 patients with cauda equina lesions. For each sample size normative limits and sensitivities for mean values and 'outliers' were calculated for 8 MUP parameters. RESULTS: As the

Approaches to sample size calculation for clinical trials in rare diseases.

We discuss 3 alternative approaches to sample size calculation: traditional sample size calculation based on power to show a statistically significant effect, sample size calculation based on assurance, and sample size based on a decision-theoretic approach. These approaches are compared head-to-head for clinical trial situations in rare diseases. Specifically, we consider 3 case studies of rare diseases (Lyell disease, adult-onset Still disease, and cystic fibrosis) with the aim to plan the sample size for an upcoming clinical trial. We outline in detail the reasonable choice of parameters fo

Sufficient Sample Size and Power in Multilevel Ordinal Logistic Regression Models.

For most of the time, biomedical researchers have been dealing with ordinal outcome variable in multilevel models where patients are nested in doctors. We can justifiably apply multilevel cumulative logit model, where the outcome variable represents the mild, severe, and extremely severe intensity of diseases like malaria and typhoid in the form of ordered categories. Based on our simulation conditions, Maximum Likelihood (ML) method is better than Penalized Quasilikelihood (PQL) method in three-category ordinal outcome variable. PQL method, however, performs equally well as ML method where fi

Sample size determination for a specific region in a multiregional trial.

Recently, geotherapeutics have attracted much attention from sponsors as well as regulatory authorities. A bridging study defined by the International Conference on Harmonisation (ICH) E5 is usually conducted in the new region after the test product has been approved for commercial marketing in the original region due to its proven efficacy and safety. However, extensive duplication of clinical evaluation in the new region not only requires valuable development resources but also delays availability of the test product to the needed patients in the new regions. To shorten the drug lag or the t

Preclinical trials in Alzheimer's disease: Sample size and effect size for behavioural and neuropathological outcomes in 5xFAD mice.

5xFAD transgenic (TG) mice are used widely in AD preclinical trials; however, data on sample sizes are largely unaddressed. We therefore performed estimates of sample sizes and effect sizes for typical behavioural and neuropathological outcome measures in TG 5xFAD mice, based upon data from single-sex (female) groups. Group-size estimates to detect normalisation of TG body weight to WT littermate levels at 5.5m of age were N = 9-15 depending upon algorithm. However, by 1 year of age, group sizes were small (N = 1 -<6), likely reflecting the large difference between genotypes at this age. To de

The effect of sample size on estimates of genetic differentiation and effective population size for Schistosoma mansoni populations.

Eradication or local extinction of the human parasite Schistosoma mansoni is a goal for many control programs. Population genetic analyses are helping to evaluate and guide these efforts, yet what to sample, how to sample and how densely to sample is not well established. We determined the S. mansoni allele frequency profile of nearly all infected inhabitants in two small Brazilian communities and created sub-samples representing 5-50% of all detected human infections (infrapopulations). Samples were selected at random with replacement, and each size class was replicated 100 times. Mean pairwi

On the pursuit of reproducibility: the importance of large sample sizes in psychoimmunology.

Peripheral inflammatory markers (PIMs), such as C-reactive protein (CRP) or white blood cell count (WBC), have been associated with depression severity in meta-analyses and large cohort studies. However, in typically-sized psychoimmunology studies (N < 200) that explore associations between PIMs and neurobiological/psychosocial constructs related to depression and studies that examine less-studied PIMs (e.g., interferon gamma), significant concerns about reproducibility of results exist. For the well-characterized association between PIMs (CRP/WBC) and depression severity, we examined statisti

Sample sizes and population differences in brain template construction.

Spatial normalization or deformation to a standard brain template is routinely used as a key module in various pipelines for the processing of magnetic resonance imaging (MRI) data. Brain templates are often constructed using MRI data from a limited number of subjects. Individual brains show significant variabilities in their morphology; thus, sample sizes and population differences are two key factors that influence brain template construction. To address these influences, we employed two independent groups from the Human Connectome Project (HCP) and the Chinese Human Connectome Project (CHCP

Proposed international conventions for particle size-selective sampling.

Definitions are proposed for the inspirable (also called inhalable), thoracic and respirable fractions of airborne particles. Each definition is expressed as a sampling efficiency (S) which is a function of particle aerodynamic diameter (d) and specifies the fraction of the ambient concentration of airborne particles collected by an ideal sampler. For the inspirable fraction. SI(d) = 0.5 (1 + e-0.06d). For the thoracic fraction, ST(d) = SI(d)[1 - F(x)], where (formula; see text) F(x) is the cumulative probability function of a standardized normal random variable. For the respirable fraction, S
